Abstract

To examine the frequency of differential diagnoses among patients with suspected endometriosis. This was a retrospective observational study based on data collected at a university hospital with a gynecological ultrasound unit between 2018 and 2022. Women with a diagnostic suspicion of endometriosis due to symptoms such as dyschezia, dyspareunia, pelvic pain, dysmenorrhea, urinary symptoms, and abnormal uterine bleeding were included. Findings identified on transvaginal ultrasound using the International Deep Endometriosis Analysis (IDEA) group protocol were assessed. A descriptive analysis of the differential diagnoses reported in absolute frequencies was performed. A total of 319 patients with clinical suspicion of endometriosis were included in the study. Ultrasound allowed for a differential diagnosis in 67% (80/119) of cases. Among these, myomas were identified in 30% (24/80), while adhesions and ovarian cysts accounted for 26% (21/80) and 20% (16/80) of the cases, respectively. Polycystic Ovarian Syndrome (PCOS) was found in 10% (8/80) of the patients, while abnormalities in the endometrial cavity were seen in 8% (6/80) of cases. The remaining 6% (5/80) of cases had other causes, including pelvic congestion, hydrosalpinx, and the presence of uterine devices compromising endometrial integrity. Transvaginal ultrasound using the IDEA group protocol can assist in the differential diagnosis of endometriosis.
