This case report examines a 49-year-old patient presenting with a painful mass in the right labia majora, which mimicked an external tumor and caused severe dyspareunia. The diagnosis was challenging due to the atypical location of the endometrioid tissue near a prior episiotomy scar, requiring a structured questionnaire for chronic pelvic pain assessment to highlight cyclical symptoms before surgical verification. Histological confirmation following excision established the presence of ectopic endometrial tissue in this unusual anatomical site, underscoring the need for high clinical suspicion in complex presentations. This paper is centrally about endometriosis — specifically the diagnostic difficulties associated with perineal and vulvar lesions located at episiotomy scars.

Abstract

Endometriosis is a chronic estrogen-dependent disease characterized by the presence of endometrioid tissue outside the uterine cavity and accompanied by chronic pain syndrome, impaired quality of life, and reproductive disorders. Diagnosis of the disease remains challenging, especially in cases of atypical localization, due to the polymorphism of clinical manifestations and the absence of specific findings during routine gynecological examination. In such cases, surgical treatment with subsequent histological verification often remains the definitive diagnostic approach. Non-standard forms of endometriosis represent a significant diagnostic challenge because they may clinically mimic benign or malignant tumor-like lesions of the external genitalia and perineal region. Perineal endometriosis is considered a rare form of extragenital endometriosis and is frequently associated with obstetric trauma, particularly episiotomy scars. The article presents a clinical case of atypical localization of endometriosis in a 49-year-old patient who complained of a rounded tumor-like formation in the area of the right labia majora, localized in its lower third with extension toward the posterior commissure and perineal region, topographically corresponding to the site of previous obstetric intervention. The patient also reported cyclic pain in the affected area with exacerbation during menstruation, pronounced dyspareunia resulting in near impossibility of sexual activity, sensation of rectal pressure, and constipation. According to the patient, the lesion had been present for approximately 10 years with gradual enlargement and progressive worsening of symptoms. At the preoperative stage, one of the key components of the comprehensive examination was the use of a structured questionnaire for chronic pelvic pain assessment, which allowed detailed evaluation of symptom cyclicality, pain intensity, gastrointestinal manifestations, and the impact of the disease on the patient’s daily and sexual life. Ultrasonography of the perineal soft tissues revealed a heterogeneous hypoechoic lesion without significant vascularization, while proctologic examination and colonoscopy excluded rectal involvement and other pathological lesions. Despite the fact that magnetic resonance imaging is considered the most informative imaging modality for perineal endometriosis, the combination of clinical findings and ultrasound examination in the present case was sufficient to determine further diagnostic and therapeutic management. Definitive diagnosis was established after surgical excision of the lesion with subsequent histological confirmation of endometriosis. Postoperatively, the patient received hormonal therapy, which resulted in significant clinical improvement within 3 months, including reduction of pain syndrome, regression of dyspareunia, improvement of bowel function, and better quality of life. The presented case highlights the importance of clinical awareness regarding atypical forms of endometriosis, the value of symptom-oriented preoperative assessment, and the role of combined surgical and hormonal treatment in achieving favorable clinical outcomes.
