EP31.07: Preoperative staging of deep endometriosis with volume ultrasound

Abstract
Diagnostic and staging of deep infiltration endometriosis (DIE) before operation is serious problem. The aim was to study preparation volume ultrasound in patients with suspected DIE. A retrospective review of 57 patients with suspected deep infiltration endometriosis (DIE). The mean age of was 36 yrs. We used combination of volume transabdominal, transvaginal, transrectal, translabial sonography and sonovaginography for mapping of DIE lesions at 18-22 days LMP. Results of testing correlated with findings at laparoscopic surgery (LS), histological examination. The indications for LS were abnormal uterine bleeding in 27 patients, lower abdominal pain-28, infertility-18, dyspareunia-16, adnexal masses in 14. In uterosacral ligaments- sensitivity (47 cases), specificity, positive predictive value (PPV) and negative predictive value (NPV) were 88%, 92%, 94% and 80%, respectively. Vaginal fornix (12) specificity, PPV& NPV - 78%, 94%, 90% and 78%. Rectosigmoid (6) specificity, PPV& NPV were 87%, 95%, 93% and 81%. Urine bladder (8) specificity, PPV& NPV were 76%, 90%, 80% and 87%. Bowel- 1 case-positive. Combination of volume transabdominal, transvaginal, transrectal, translabial sonography and sonovaginography have great correlation with surgical findings in patients with suspected deep infiltration endometriosis.
