Analysis of clinical therapeutic effect after laparoscope excision treatment of ovarian endometrial implantation cyst for 266 patients

Abstract
Objective: To evaluate the clinical therapeutic effect after laparoscope excision treatment of ovarian endometrial implantation cyst. Methods: Retrospectively analysis was 266 cases who underwent laparoscope excision of ovarian endometrial implantation cyst from January 1996 to January 2003 , all cases after surgery were follow -up form 1 month to 7 years , all relative factos that possible influence recurrence rate and pregnancy rate of ovarian endometrial implantation cyst were summarized in this paper . Results: (1) the recurrence rates of cumulative 7 years were : the previous surgery vs the no previous surgery-65. 25% vs 42. 63% , P = 0. 000 2; the stage Ⅲ vs the stage Ⅳ -35. 71% vs 56. 88% , F = 0. 040 56; the preoperative medical tretment vs the no preoperative medical tretment - 50. 91%vs 41. 98%, P = 0. 449 3; the postoperative medical therapy vs the no postoperative medical therapy -50. 36%vs 38. 28% ,P = 0. 714 1. (2)the cumulative pregnancy rates over 7 years were:the previous surgery vs the no previous surgery -36. 47% vs 58. 46%, P = 0. 045 2; the stage Ⅲ vs the stage Ⅳ-59. 56% vs 49. 12%, P = 0. 020 7; the preoperative medical tretment vs the no preoperative medical tretment-47. 26% vs 57. 73%, P= 0. 415 2; the postoperative medical therapy vs the no postoperative medical therapy-54. 85% vs 54. 06%, P =0. 459 1. From the Multi - factors logistic model analysis , the previous surgery for endometriosis and stages r -AFS are two covariates that can not be deleted. (3) Ameliorated dysmenorrhea after surgery: 45. 6% cases pain free, 33. 0% cases decreased pain, 21. 4% cases no deteriorated pain. Conclusion: Laparoscope excision treatment of ovarian endometrial implantation cyst seems to be effective and reliable, the adjuvant medical treatment before and after surgery is effective method,but should consider concrete situation for every person. The stage Ⅳ disease and previous surgery for endometriosis are two distinctly prognostic factors that influence the recurrence rate after surgery.
