Insomnia diminishes attention, memory, visuospatial capacities, and executive functioning. The association between insomnia and age-related cognitive deterioration has been previously established. Aim Assessing the effect of insomnia on cognitive functioning in older persons was the aim of this study. Method This case-control study recruited 110 participants from the Department of Psychiatry, Cairo University's Faculty of Medicine. Patients aged 60 and above were divided into two groups: 55 cases and 55 controls. All participants were given the Mini Neuropsychiatric Interview, the Insomnia Severity Index (ISI), the Pittsburgh Sleep Quality Index (PSQI), and the Montreal Cognitive Assessment Basic Version (MOCA-B) as evaluation instruments. Results The patients and controls showed notable variations in terms of executive functioning, calculation, visuoperception, naming, attention, and the overall score of the MOCA-B test. Furthermore, comparison between patients with early insomnia and other types of insomnia revealed a statistically significant impairment in the total MOCA-B test, delayed recall, and attention subtests. Conclusion When compared to matched controls, elderly people with insomnia did worse on cognitive tests. Calculation, attention, executive skills, visuoperception, and naming were among the cognitive processes that were impaired. [ 1 , 2 ]. Insomnia affects daily cognitive or work-related efficacy and mood [ 3 , 4 ]. Insomnia was found to increase susceptibility to Alzheimer's disease, cognitive dysfunction, diminished ability in attention, memory (e.g.working and episodic memory), visuospatial capacities, executive functioning such as problem solving and manipulation, and verbal memory [ 5 ], along with difficulty in thinking clearly [ 6 ]. Along with the elevated risk of cerebrovascular and cardiovascular disorders [ 7 ]. In Egypt, one study assessing cognitive health among older people in geriatric homes revealed a higher incidence of insomnia among males and married elderly. Moreover, chronic insomnia was found to be common with extended stays in geriatric households. Predictors for insomnia among Egyptian geriatrics were chronic pain, nocturia, and chronic diseases [ 8 ]. Although a large body of evidence has revealed the strong correlation between insomnia and cognitive decline among elderly people, To our knowledge, this is the first study to evaluate this important disorder in the elderly population of Egypt. Accordingly, Investigating the relationship between insomnia and cognitive function impairments in older persons was the aim of this study. This study hypothesised that the elderly with insomnia may be prone to developing cognitive impairment when compared to normal subjects. Methods Study design and population Patients aged 60 or older who were referred from Cairo University's Faculty of Medicine, Psychiatry and Addiction Medicine Hospital's sleep outpatient clinic, and members of geriatric clubs participated in a case-control study between May 2023 and June 2024. The Geriatric Social Center at Cairo University served as the source of the control group. The inclusion criteria Individuals who were over 60, of either sex, had clinically average IQs, consented to participate, and satisfied the DSM-5 criteria for insomnia [ 9 ]. Exclusion criteria included patients who met the diagnostic criteria of sleep disorders excluding insomnia, such as breathing-related sleep disorders, patients fulfilling the DSM-5 diagnostic criteria for Major neurocognitive disorders, such as dementia, patients fulfilling the diagnostic criteria of psychiatric disorders i.e. Patients with psychotic disorders or mood disorders, who meet the diagnostic criteria for substance use disorders, those with co-morbid medical or neurological conditions that may impair cognition (e.g., terminal organ failure, stroke), or individuals using hypnotics or other psychotropic medications. Study procedure: Patients were subjected to the following assessment tools; 1-Mini Neuro-Psychiatric Interview (MINI 7.0.2) [ 10 ] for the assessment of Psychiatric disorders. 2- Both its verified Arabic version [ 12 ] and the Insomnia Severity Index (ISI) [ 11 ] were used. The ISI is a seven-item self-report questionnaire that evaluates the type, intensity, and consequences of insomnia. The overall score ranged from zero to twenty-eight. A 5-point Likert scale was used to score each item, with 0 denoting no difficulty and 4 denoting a very serious issue. Overall score was interpreted as nonclinically relevant (0–7), subthreshold (8–14), moderate (15–21), and severe (22–28) insomnia. 3. We used the validated Pittsburgh Sleep Quality Index (PSQI) [ 13 ] and its Arabic counterpart [ 14 ]. The PSQI was created to evaluate the quality of sleep that individuals with severe sleep disorders had throughout the previous month. Seven component scores are produced from a total of 19 elements. A single global score is the sum of these seven component scores. A 4-point Likert scale was used to rate the others. Poor sleep quality is indicated by a global score of 6 or higher, with a range of 0 to 21. 4- The Montreal Cognitive Assessment (MOCA-B) [ 15 ] and its validated Arabic version [ 16 ] were used to assess executive functions, orientation, calculations, language, conceptual thinking, attention and concentration, memory, and visuoperception in older subjects with low educational attainment. The test scores 30 points. The cut-off is 21/22 for assessing mild Neurocognitive Disorders (NCD) and 16/17 for detecting major NCD (dementia). Participants with scores below 17 were excluded. Statistical Analysis The data were coded and entered using the Statistical Package for the Social Sciences (SPSS) version 28 (IBM Corp., Armonk, NY, USA). The data were summarized through the calculation of mean, standard deviation, median, minimum, and maximum for quantitative variables, alongside frequencies (counts) and relative frequencies (percentages) for categorical variables. The Mann-Whitney test, a non-parametric method, was used for data not adhering to a normal distribution, while the unpaired t-test was applied for comparisons of normally distributed quantitative variables between groups. The Chi-square test was utilized to analyze categorical data. An exact test was utilized when the expected frequency was less than 5. The Spearman correlation coefficient was employed to evaluate correlations among quantitative variables. P-values below 0.05 were deemed statistically significant. Sample size calculation According to Lovato et al.'s study [ 17 ]A sample size of 55 per group was required to detect a significant difference, assuming an alpha error of 0.05 and a power of 80%. Results Analysis of the patients' sociodemographic information revealed no statistically significant differences between the case and control groups regarding gender, marital status, place of residence, or occupation. Additionally, no statistically significant differences were observed in age or formal years of schooling between patients and controls (Table 1 ). Furthermore, no significant alteration was observed in the body mass index (BMI) or the smoking index. The ISI (cases: mean ± SD 13.58 ± 1.87; controls: mean ± SD not specified) and PSQI (cases: mean ± SD 8.67 ± 2.14; controls: mean ± SD not specified) exhibited a statistically significant difference between cases and controls (p < 0.001) (Table 1 ). A high percentage of poor sleep hygiene (65.5%) was observed among cases, and almost half reported restorative sleep issues. Insomnia not only increases the likelihood of depression, diabetes, and hypertension, and worsens the quality of their life [ 18 ], but also affects cognitive function and increases the risk for Alzheimer’s disease [ 19 ]. In this study, we hypothesized that older insomniac people have a higher risk of cognitive impairment when compared to normal subjects. Assessing cognitive function among our elderly insomniac patients revealed significantly lower performance compared with the normal group, particularly in total MOCA-B score, executive functions, calculation, visuoperception, naming, and attention. In this study, approximately 66% of cases exhibited poor sleep hygiene, and nearly half experienced restorative sleep. Similarly, other studies revealed that more than 50% of geriatric patients demonstrated poor sleep hygiene and restorative sleep [ 20 – 22 ]. Furthermore, In terms of the MOCA-B test's overall score, our study showed a substantial difference between patients and controls., executive functions, calculation, visuoperception, naming, and attention. On the same line, a decline in attention, memory, visuospatial abilities, executive function, and verbal memory among older people with insomnia was detected in one study [ 5 ], as well as another that showed a decrease in cognitive subdomains, including MoCA-B and delayed recall, among older people with insomnia [ 23 ]. This study demonstrated a significant difference in late insomnia between the insomniac group and the control group regarding visuoperception,, with no statistically significant difference in other cognitive function scores. Although no studies were found on the effects of late insomnia and visual perception impairment, a large body of evidence demonstrates that visual impairment and insomnia have a bidirectional relationship [ 24 , 25 ]. This aligns with another large Chinese cohort study, which found that older people with fewer than 5 hours of sleep per night had a higher risk of developing sensory impairment, including visual perception [ 26 ]. Extensive evidence indicates a significant correlation between insomnia and the risk of dementia in the elderly population.. A US national registry followed elderly patients above 65 years old for ten years, revealing an escalation of dementia 1.5 times in patients with insomnia compared to those without insomnia, while those with sleep-maintenance insomnia showed a significant decrease in dementia by 40% [ 27 ]. Similarly, a meta-analysis has demonstrated the association between all-cause dementia and the presence of insomnia [ 28 ]. All previous evidence indicates the strong correlation between severe insomnia and dementia, which contradicts our study findings, where a weak correlation between deprived sleep and longer insomnia and poor recall was detected. Conclusion Insomnia impairs cognitive functions in elderly patients, particularly in areas such as calculation, attention, executive functions, visuoperception, and naming. Initial and late insomnia may cause attention and visual impairments, while the duration and severity of insomnia significantly impact short-term memory. Strengths and limitations In this study, the MOCA-B, a standardized and validated tool; clinician-rated sleep interviews; and DSM-5-TR diagnostic criteria were used to assess cognitive performance in insomniac patients. Nevertheless, limitations of this study include the inability to use polysomnographic investigations to objectively evaluate sleep, the measurement of insomnia symptoms only once, and a limited sample size that restricted the generalisability of the findings. The study also highlighted the need for more precise sleep assessment instruments. Patients aged 60 and above were divided into two groups: 55 cases and 55 controls. All participants were given the Mini Neuropsychiatric Interview, the Insomnia Severity Index (ISI), the Pittsburgh Sleep Quality Index (PSQI), and the Montreal Cognitive Assessment Basic Version (MOCA-B) as evaluation instruments.\u003c/p\u003e\u003cp\u003e\u003cb\u003eResults\u003c/b\u003e\u003c/p\u003e\u003cp\u003eThe patients and controls showed notable variations in terms of executive functioning, calculation, visuoperception, naming, attention, and the overall score of the MOCA-B test. Furthermore, comparison between patients with early insomnia and other types of insomnia revealed a statistically significant impairment in the total MOCA-B test, delayed recall, and attention subtests.\u003c/p\u003e\u003cp\u003e\u003cb\u003eConclusion\u003c/b\u003e\u003c/p\u003e\u003cp\u003eWhen compared to matched controls, elderly people with insomnia did worse on cognitive tests.
