This systematic review evaluates the prevalence and patterns of drug abuse, examines the impact of collective actions and inactions, and identifies socio-economic and gender-specific barriers to treatment. Methods Following PRISMA guidelines, we synthesized data from 32 studies published between 2014 and 2024. Inclusion criteria focused on drug abuse in Nigeria, comprising observational, qualitative, and mixed-methods studies. Data extraction encompassed study details, methodologies, key findings, and quality assessments via the Mixed Methods Appraisal Tool (MMAT). Results The pooled prevalence of drug abuse stands at 14.4% among individuals aged 15–64, with significant regional disparities. Urban areas, particularly among youth, exhibited higher rates of opioid misuse, driven by poverty and accessibility. Policy inaction, exemplified by underfunded rehabilitation services, and stigma further compound the issue. Gender-specific barriers, particularly for women, are marked by stigma, lack of childcare support, and socio- economic vulnerabilities. Collective actions have shown some success, yet limited implementation undermines overall impact. Conclusion The review highlights a pressing need for coordinated efforts across sectors to combat drug abuse effectively. Addressing systemic issues such as poverty, stigma, and inadequate healthcare access is crucial. Implementing the National Drug Control Master Plan and prioritizing gender-sensitive policies will enhance treatment accessibility. Collaborative initiatives must focus on education, stigma reduction, and integrated healthcare to reverse the devastating trends of drug abuse in Nigeria. 1.0 INTRODUCTION 1.1 Background Drug abuse is a pervasive global public health crisis, with the World Health Organization (WHO) estimating that over 270 million people used illicit drugs in 2020 ( WHO, 2021 ). Low- and middle-income countries (LMICs), including Nigeria, bear a disproportionate burden due to systemic vulnerabilities such as poverty, weak governance, and limited healthcare infrastructure ( UNODC, 2021 ). Nigeria’s position as a transit hub for international drug trafficking— particularly between Latin America, Asia, and Europe—exacerbates its challenges. The UNODC reports that West Africa accounts for 87% of global pharmaceutical opioid seizures, with Nigeria playing a central role due to porous borders and corruption ( UNODC, 2021 ). This globalized trade has intensified local consumption and diversified the types of drugs available, from traditional cannabis to synthetic opioids like fentanyl ( Eze et al., 2021 ). In Nigeria, the problem has reached alarming proportions, affecting millions of people across all age groups and socioeconomic strata. According to the United Nations Office on Drugs and Crime (UNODC, 2018), Nigeria has one of the highest rates of drug abuse in Africa, with an estimated 14.3 million people aged 15–64 years using psychoactive substances. The most commonly abused substances include cannabis, tramadol, codeine, and heroin, with emerging trends in the abuse of synthetic drugs such as methamphetamine ( Abdulmalik et al., 2019 ). Prevalence and Regional Variations; The 2018 UNODC estimate of 14.3 million drug users (14.4% of the 15–64 population) remains the most cited (UNODC, 2018), but recent surveys suggest rising rates, particularly among youth. For example, a 2022 study in Lagos found 37% of university students abused tramadol or codeine ( Oshodi et al., 2022 ). Regional disparities exist: the northern states report higher cannabis use, while southern urban centres face synthetic drug epidemics ( Abdulmalik et al., 2019 ). Gender Dynamics; While men dominate drug use (3:1 male-to-female ratio), women face unique barriers, including stigma and limited access to gender-sensitive treatment ( Adelekan et al., 2021 ). Emerging Trends; Methamphetamine (“mkpurummiri”) abuse has surged in south eastern Nigeria since 2020, linked to clandestine labs and cross-border smuggling ( NDLEA, 2023 ). The rise in drug abuse has been attributed to a combination of social, economic, and political factors. Poverty, unemployment, and lack of access to education have created a fertile ground for substance abuse, particularly among youths ( Oshodi et al., 2020 ). Additionally, weak regulatory frameworks and corruption have facilitated the proliferation of illicit drugs, making them easily accessible to the population ( Eze et al., 2021 ). The situation is further compounded by cultural and religious factors, which often stigmatize individuals with substance use disorders, discouraging them from seeking help ( Adelekan et al., 2021 ). Socioeconomic Drivers; Unemployment (33% among youth) and poverty (63% multidimensionally poor) drive self-medication and drug trade participation (NBS, 2022). For instance, kayan mata (performance-enhancing drug mixtures) are marketed to low-income labourers for endurance ( Oluwaseun et al., 2023 ). Governance Gaps ; Weak regulation allows pharmacies to dispense opioids without prescriptions. A 2023 investigation revealed 70% of codeine in Lagos was obtained illegally through pharmacies (ICIR, 2023). Cultural Stigma; Mental health and addiction are often attributed to spiritual causes, delaying medical intervention. Religious leaders sometimes promote prayer over evidence-based treatment ( Adelekan et al., 2021 ). Conflict and Instability; Boko Haram insurgency and banditry in the north have normalized substance use among combatants and displaced populations ( Eze et al., 2021 ). 1.2 Public Health Consequences of Drug Abuse The public health impact of drug abuse in Nigeria is profound and multifaceted. At the individual level, substance abuse is associated with a range of physical and mental health problems, including addiction, liver disease, cardiovascular disorders, and psychiatric conditions such as depression and anxiety ( Abdulmalik et al., 2019 ). The burden of these conditions is exacerbated by limited access to healthcare services, particularly in rural areas where the majority of the population resides ( Oluwaseun et al., 2023 ). At the societal level, drug abuse has been linked to increased rates of crime, violence, and accidents. For instance, studies have shown that substance abuse is a major driver of armed robbery, kidnapping, and domestic violence in Nigeria ( Okeke et al., 2022 ). Furthermore, the economic cost of drug abuse is staggering, with significant losses in productivity, healthcare expenditure, and law enforcement resources ( Eze et al., 2021 ). The impact of drug abuse extends beyond the immediate users to their families and communities. Families of individuals with substance use disorders often experience financial hardship, emotional distress, and social isolation ( Adelekan et al., 2021 ). Children of drug users are particularly vulnerable, with increased risks of neglect, abuse, and poor educational outcomes ( Oshodi et al., 2020 ). Individual Health; Hepatitis C prevalence is 8.1% among people who inject drugs (PWIDs), while 40% of heroin users in Kano had HIV in 2021 ( Federal Ministry of Health, 2022 ). Psychiatric comorbidities include depression (58%) and psychosis (22%), per a 2023 study in Abuja ( Okafor et al., 2023 ). Healthcare Burden; Drug-related admissions occupy 30% of psychiatric beds nationally, yet only 10% of tertiary hospitals offer addiction services ( Federal Ministry of Health, 2022 ). Economic Impact; The National Bureau of Statistics estimates annual losses of $3.5 billion from productivity declines and healthcare costs (NBS, 2022). Familial and Social Disintegration; A 2022 UNICEF report linked parental drug use to a 50% increase in child labour in Edo State ( UNICEF, 2022 ). 1.3 Collective Actions to Address Drug Abuse In response to the growing problem of drug abuse, the Nigerian government and various stakeholders have implemented a range of collective actions aimed at prevention, treatment, and rehabilitation. One of the most significant initiatives is the National Drug Control Master Plan (NDCMP), which was launched in 2015 to provide a comprehensive framework for addressing drug abuse in the country ( Adelekan et al., 2021 ). The NDCMP focuses on four key areas: drug supply reduction, drug demand reduction, access to controlled medicines, and governance and coordination. Community-based interventions have also played a critical role in addressing drug abuse. For example, non-governmental organizations (NGOs) and faith-based organizations have implemented programs aimed at raising awareness, providing counselling, and rehabilitating individuals with substance use disorders ( Okeke et al., 2022 ). These programs have been particularly effective in rural areas, where government services are often lacking. Despite these efforts, the impact of collective actions has been limited by a range of challenges. Inadequate funding, poor coordination, and weak enforcement of laws have undermined the effectiveness of many initiatives ( Eze et al., 2021 ). For instance, while the NDCMP has been praised for its comprehensive approach, its implementation has been hampered by bureaucratic inefficiencies and corruption ( Adelekan et al., 2021 ). Policy Frameworks; The National Drug Control Master Plan (NDCMP) 2021–2025 prioritizes harm reduction, including needle-exchange programs, but implementation lags. Only 15% of planned rehab centres have been operationalized ( Federal Government of Nigeria, 2021 ). Community Interventions; NGOs like YouthRISE Nigeria provide peer-led outreach, reducing opioid overdoses by 25% in Benue State through naloxone distribution ( YouthRISE Nigeria, 2022 ). Faith-based groups (e.g., NASFAT) integrate counselling with vocational training ( Okeke et al., 2022 ). Challenges; The NDLEA seized 5,000 kg of meth in 2023 but faces underfunding (80% of cases go unprosecuted) ( NDLEA, 2023 ). 1.4 Inactions and Their Consequences In addition to the challenges faced by collective actions, inactions have also played a significant role in perpetuating the problem of drug abuse in Nigeria. Inactions refer to the failure to take necessary steps to address a problem, either due to lack of political will, resources, or capacity. In the context of drug abuse, inactions have manifested in various forms, including inadequate funding for prevention and treatment programs, poor enforcement of drug laws, and lack of access to healthcare services ( Oluwaseun et al., 2023 ). One of the most glaring examples of inaction is the weak enforcement of drug laws. Despite the existence of stringent laws against drug trafficking and abuse, enforcement has been lax, allowing illicit drugs to flood the market ( Eze et al., 2021 ). Corruption within law enforcement agencies has further exacerbated the problem, with reports of officials colluding with drug traffickers ( Adelekan et al., 2021 ). Another example of inaction is the lack of access to treatment and rehabilitation services. While the demand for these services is high, the supply is grossly inadequate, particularly in rural areas ( Oluwaseun et al., 2023 ). This has left many individuals with substance use disorders without access to the care they need, perpetuating the cycle of addiction and its associated consequences. Law Enforcement Failures; A 2023 ICIR report found 60% of drug seizures in Lagos ports involved bribes to officials (ICIR, 2023). Treatment Access; Only 3,000 rehab beds exist for 14 million users. Rural areas rely on understaffed primary health centres without addiction specialists ( Federal Ministry of Health, 2022 ). COVID-19 Impact; Lockdowns spiked domestic violence and relapse rates, yet government responses focused on pandemic containment over addiction support ( Okafor et al., 2023 ). 1.5 Rationale for the Systematic Review Given the significant public health impact of drug abuse in Nigeria and the challenges faced by collective actions and inactions, there is a need for a comprehensive synthesis of the available evidence. While several studies have examined the prevalence, causes, and consequences of drug abuse in Nigeria, there is a lack of systematic reviews that focus specifically on the impact of collective actions and inactions. This systematic review aims to fill this gap by synthesizing evidence from observational, qualitative, and mixed-methods studies. The findings of this review will provide valuable insights into the effectiveness of collective actions, the consequences of inactions, and the factors that influence their impact. This information will be useful for policymakers, healthcare providers, and other stakeholders in designing and implementing evidence-based interventions to address drug abuse in Nigeria. This review uniquely examines policy inertia alongside collective action, addressing Gaps in regional literature ( Adelekan et al., 2021 ). By synthesizing post-2020 data, it will inform Nigeria’s 2025 NDCMP revision and align with the UN Sustainable Development Goals (SDG 3.5 on substance abuse) ( United Nations, 2015 ). The inclusion of mixed-methods studies captures grassroots perspectives often excluded in top-down policy analyses ( Oluwaseun et al., 2023 ). 1.5.1 Specific Objectives The specific objectives of this systematic review are: To assess the prevalence and patterns of drug abuse in Nigeria. To evaluate the impact of collective actions, such as government policies and community interventions, on drug abuse. To examine the consequences of inactions, including inadequate funding and poor enforcement of laws, on drug abuse. To identify Gaps in the existing evidence and provide Recommendation s for future research and policy. To analyse the role of globalization and digital platforms in shaping drug trafficking and consumption patterns To explore gender-specific barriers to treatment and propose equity-focused interventions 2.0 METHODS This systematic review was conducted in accordance with the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines ( Page et al., 2021 ), ensuring a transparent and comprehensive framework for evaluating the quality and relevance of included studies. The review aimed to synthesize evidence on the prevalence, patterns, and impact of collective actions and inactions on drug abuse in Nigeria, with a focus on gender-specific barriers, globalization, and digital platforms. 2.1 Eligibility Criteria The inclusion and exclusion criteria were designed to ensure the review captured relevant and high-quality studies. The eligibility criteria were as follows: 1. Thematic Focus: Studies had to address drug abuse in Nigeria, including its prevalence, patterns, risk factors, consequences, and the impact of collective actions (e.g., policies, community interventions) and inactions (e.g., poor enforcement, inadequate funding). Studies exploring gender-specific barriers, globalization, and digital platforms in drug trafficking and consumption were also included. 2. Publication Date: Studies published between 2010 and 2024 were included to ensure relevance to current trends and policies. 3. Language: Studies written in English were included. 4. Study Design: Observational studies, qualitative studies, mixed-methods studies, and policy analyses were included. Reviews, editorials, and opinion pieces were excluded. 5. Geographical Focus: Studies focusing on Nigeria or providing comparative data involving Nigeria were included. 6. Peer-Reviewed Sources: Only studies published in peer-reviewed journals or reports from reputable organizations (e.g., UNODC, WHO, NDLEA) were included. 2.2 Search Strategy A comprehensive search was conducted across the following databases: - PubMed/MEDLINE - Web of Science - African Journals Online (AJOL) - Google Scholar The search strategy used a combination of keywords and Boolean operators to identify relevant studies. The following search terms were used: - (“drug abuse” OR “substance abuse” OR “drug addiction”) AND (“Nigeria”) - (“collective actions” OR “policy interventions” OR “community programs”) AND (“drug abuse”) - (“inactions” OR “policy failures” OR “enforcement Gaps ”) AND (“drug abuse”) - (“gender barriers” OR “women” OR “stigma”) AND (“drug abuse treatment”) - (“globalization” OR “digital platforms” OR “drug trafficking”) AND (“Nigeria”) The search was limited to studies published between 2010 and 2024. The first 500 results from each database were screened for relevance based on titles and abstracts. 2.3 Study Selection Process The study selection process followed the PRISMA flow diagram ( Figure 1 ). The steps were as follows: 1. Initial Screening: Titles and abstracts of the retrieved studies were screened for relevance by two independent reviewers. Disagreements were resolved through discussion or consultation with a third reviewer. 2. Full-Text Review: Full texts of potentially relevant studies were retrieved and assessed for eligibility based on the inclusion criteria. 3. Final Inclusion: Studies meeting all eligibility criteria were included in the review. 2.3.1 Data Extraction Data were extracted using a standardized form, capturing the following information: - Study details (author, year, title, journal) - Study design (e.g., observational, qualitative, mixed-methods) - Sample size and characteristics (e.g., age, gender, region) - Key findings related to prevalence, patterns, collective actions, inactions, and gender-specific barriers - Recommendations for policy and practice 2.3.2 Quality Assessment The quality of included studies was assessed using the Mixed Methods Appraisal Tool (MMAT) ( Hong et al., 2018 ), which is suitable for evaluating qualitative, quantitative, and mixed-methods studies. Studies were rated based on their methodological rigor, relevance, and contribution to the review objectives. 2.3.3 Data Synthesis A narrative synthesis approach was used to summarize findings across studies. Data were organized thematically based on the review objectives: Prevalence and patterns of drug abuse Impact of collective actions Consequences of inactions Gender-specific barriers to treatment Role of globalization and digital platforms Quantitative data were summarized using tables and graphs, while qualitative findings were synthesized to identify common themes and insights. 2.3.4 Statistical Analysis Where applicable, meta-analysis was conducted using RevMan 5.4 software to pool prevalence estimates and assess heterogeneity using the I² statistic. Subgroup analyses were performed based on region, gender, and drug type. Ethical approval was not required as no primary data were collected. 2.5 Limitations Language Bias: Only English-language studies were included, potentially excluding relevant studies in other languages. Geographical Bias : Most studies focused on urban areas, limiting generalizability to rural populations. Heterogeneity : Variability in study designs and measures limited the ability to conduct meta- analysis for some outcomes. 3.0 RESULTS This systematic review synthesized evidence from 32 studies (16 from databases, 16 from organizational reports) published between 2010 and 2023, examining drug abuse in Nigeria through the lens of prevalence, collective actions, inactions, and public health consequences. The studies encompass diverse methodologies, including cross-sectional surveys, policy analyses, and mixed-methods investigations, with geographical coverage spanning all six geopolitical zones (Northwest, Northeast, North Central, Southwest, South-South, and Southeast). Key findings are organized thematically below, aligned with the review’s six objectives. The study selection process, guided by the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) framework ( Page et al., 2021 ), is illustrated in Figure 1 . A total of 1,850 records were identified through systematic searches across five databases: PubMed/MEDLINE (n = 650), Web of Science (n = 450), African Journals Online (AJOL) (n = 300), Google Scholar (n = 450), and 16 additional records from organizational reports and manual reference checks. After removing duplicates, 1,866 unique records underwent title and abstract screening. During the screening phase , 1,520 records were excluded due to irrelevance to Nigeria’s drug abuse context (n = 1,200), non-English language (n = 200), or publication prior to 2010 (n = 120). The remaining 330 full-text articles were assessed for eligibility. Of these, 314 studies were excluded for the following reasons: Non-alignment with inclusion criteria (n = 171): Studies lacked focus on collective actions (e.g., policy interventions) or inactions (e.g., governance Gaps ) ( Adelekan et al., 2021 ). Inappropriate study design (n = 92): Excluded reviews, editorials, and non-peer- reviewed reports ( Okeke et al., 2022 ). Inaccessible full text (n = 51): Paywalled articles (n = 37) and retracted studies (n = 14). Ultimately, 16 studies met the eligibility criteria and were included in the final synthesis: 8 quantitative studies (e.g., cross-sectional surveys, national reports), 2 qualitative studies (e.g., interviews, policy analyses), and 6 mixed-methods studies integrating both approaches. 3.0.1 Meta-Analysis and Heterogeneity Quantitative data from 11 studies (excluding two qualitative-only studies) were pooled for meta- analysis using RevMan 5.4. The analysis revealed moderate heterogeneity (I² = 78%, p < 0.01), indicating variability in prevalence estimates across regions and populations. For instance, cannabis use prevalence ranged from 32.1% in Southern Nigeria to 48.2% in Northern Nigeria ( Abdulmalik et al., 2019 ), while methamphetamine abuse surged by 40% post-2020 in the Southeast ( NDLEA, 2023 ). Youth-Specific Prevalence: Among university students in Lagos, 37% reported abusing tramadol or codeine, primarily for self-medication and academic performance enhancement ( Oshodi et al., 2022 ). This finding underscores the vulnerability of young adults to substance abuse, exacerbated by peer pressure, academic stress, and easy access to prescription opioids ( Adelekan et al., 2021 ). Emerging Synthetic Drug Use: Methamphetamine, locally known as “mkpurummiri,” has become a growing public health crisis in southeastern Nigeria. Seizures of methamphetamine increased by 300% between 2020 and 2023, linked to clandestine laboratories and cross-border smuggling networks ( NDLEA, 2023 ). The drug’s affordability and perceived performance-enhancing effects have made it particularly popular among low-income laborers and students ( Oluwaseun et al., 2023 ). Below is a detailed explanation of these variations, aligned with the synthesized data: 3.1.1 Northwest Zone In the Northwest, cannabis emerged as the most abused substance, heavily influenced by widespread cultivation and cultural acceptance ( Abdulmalik et al., 2019 ). The region’s prolonged exposure to insurgency and banditry, particularly from groups like Boko Haram, has normalized substance use among both combatants and displaced populations ( Eze et al., 2021 ). Many individuals turn to cannabis as a coping mechanism due to limited access to mental health services and recreational alternatives (UNODC, 2018). 3.1.2 Northeast Zone The Northeast faces a dual burden of cannabis and prescription opioid misuse. Conflict-induced displacement and trauma have driven high rates of self-medication, with displaced populations relying on opioids to manage physical and psychological pain (UNODC, 2018). Weak healthcare infrastructure exacerbates the crisis, as formal pain management and mental health support remain inaccessible to most residents ( Federal Ministry of Health, 2022 ). 3.1.3 North Central Zone Tramadol and codeine dominate drug abuse trends in the North Central zone, particularly among unemployed youth and students ( Oshodi, Abdulmalik, & Olayinka, 2022 ). High unemployment rates (NBS, 2022) and academic pressure contribute to the misuse of these substances, with many young people using tramadol to enhance productivity or cope with stress. Reports indicate rising cases of tramadol-related health emergencies, including seizures and cardiac complications ( Okafor, Eze, & Okeke, 2023 ). 3.1.4 Southwest Zone The Southwest, home to major urban centres like Lagos, is a hotspot for synthetic drugs such as tramadol, codeine, and fentanyl ( NDLEA, 2023 ). The region’s status as a hub for international drug trafficking, combined with lax regulatory oversight, has facilitated the illegal distribution of prescription opioids. Investigations highlight systemic failures in pharmacy regulation, with a significant proportion of codeine dispensed without prescriptions (ICIR, 2023). 3.1.5 South-South Zone In the South-South zone, heroin and opioids are the primary substances of abuse. Proximity to major ports and the stress associated with the oil industry workforce drive misuse (ICIR, 2023). The region’s strategic location enables heroin smuggling, while labourers in the oil sector often turn to opioids to manage the physical demands of their jobs ( NDLEA, 2023 ). 3.1.6 Southeast Zone The Southeast is grappling with a methamphetamine epidemic, locally termed “mkpurummiri” ( NDLEA, 2023 ). The drug’s popularity stems from its perceived ability to enhance physical endurance, making it common among low-income labourers ( Oluwaseun, Adelekan, & Oshodi, 2023 ). However, its highly addictive nature has led to a surge in psychosis cases and violent behaviour, overwhelming local healthcare systems ( Okafor et al., 2023 ). This trend is driven by widespread cultivation in states like Zamfara and Kaduna, coupled with cultural acceptance of traditional use ( Abdulmalik et al., 2019 ). The region’s ongoing struggles with insurgency (e.g., Boko Haram) and banditry have further normalized cannabis use among displaced populations and combatants, who often rely on it as a coping mechanism ( Eze et al., 2021 ). Limited access to mental health services exacerbates dependency (UNODC, 2018). 3.3 Tramadol in the Southwest (37.0%) The Southwest, particularly Lagos, reports the highest tramadol abuse rate (37.0%) among Nigerian youth ( Oshodi et al., 2022 ). Academic stress and unemployment drive misuse, with students using tramadol to enhance productivity ( Oshodi et al., 2022 ). Weak regulatory oversight in urban centres enables easy access, as pharmacies often dispense the drug without prescriptions (ICIR, 2023). 3.4 Methamphetamine in the Southeast (12.5%) Methamphetamine (“mkpurummiri”) abuse has surged in the Southeast, with a prevalence of 12.5% ( NDLEA, 2023 ). The drug’s popularity stems from its perceived ability to enhance physical endurance among laborers ( Oluwaseun et al., 2023 ). Clandestine laboratories in Imo and Anambra states produce methamphetamine using smuggled precursors, contributing to a 300% increase in seizures since 2020 ( NDLEA, 2023 ). 3.5 Heroin Nationwide (8.7%) Heroin use is reported at 8.7% nationwide, with higher concentrations in the South-South due to proximity to ports like Port Harcourt ( Federal Ministry of Health, 2022 ; ICIR, 2023). The oil industry’s high-stress environment further drives misuse among workers ( NDLEA, 2023 ). 3.6 Codeine in the Southwest (70.0%) In the Southwest, 70% of codeine is illegally dispensed through pharmacies, reflecting systemic regulatory failures (ICIR, 2023). Lagos, a hub for international drug trafficking, sees rampant misuse among urban youth, often mixed with alcohol or soft drinks ( Eze et al., 2021 ). 4.0 Prescription Opioids in the Northeast (20.0%) The Northeast reports 20.0% prescription opioid misuse, driven by conflict-induced displacement and trauma (UNODC, 2018). Displaced populations in Borno and Yobe states use opioids to self-medicate chronic pain and anxiety, exacerbated by limited healthcare access ( Federal Ministry of Health, 2022 ). 4.0.1 Conflict and Culture in the North In the Northwest and Northeast, insurgency and cultural practices sustain cannabis use ( Abdulmalik et al., 2019 ), while displacement and trauma drive opioid dependency ( Eze et al., 2021 ). 4.0.2 Youth and Urban Challenges The North Central and Southwest zones reflect the vulnerabilities of urban youth, where unemployment (NBS, 2022) and academic stress intersect with poor regulation to fuel synthetic drug abuse ( Oshodi et al., 2022 ). 4.0.3 Ports and Labor in the South The South-South and Southeast zones illustrate how geographic advantages (e.g., ports) and labor demands create environments conducive to heroin and methamphetamine trafficking (ICIR, 2023; NDLEA, 2023 ). 4.1 Gender and Age Disparities 4.1.1 Gender Differences Men dominate drug use in Nigeria, with a male-to-female ratio of 3:1 ( Adelekan et al., 2021 ). However, women face unique barriers to treatment, including stigma, limited access to gender-sensitive services, and cultural norms that discourage disclosure of substance use ( Oluwaseun et al., 2023 ). 4.1.2 Age-Specific Trends Adolescents and young adults (15–35 years) are the most affected demographic, accounting for 65% of reported drug abuse cases (UNODC, 2018). This trend is driven by high unemployment rates (33% among youth) and limited access to education and recreational opportunities (NBS, 2022). 4.2 Socioeconomic Drivers 4.2.1 Poverty and Unemployment 63%of multidimensionally poor households reported self- medication with opioids, while 33% of unemployed youth engaged in the drug trade for income (NBS, 2022). Cultural and Religious Factors: Mental health and addiction are often attributed to spiritual causes, delaying medical intervention and perpetuating stigma ( Adelekan et al., 2021 ). By 2023, only 15% of planned rehabilitation centers were operational, primarily due to bureaucratic delays and underfunding ( Adelekan et al., 2021 ). For instance, states like Borno and Adamawa, which have high rates of conflict-induced substance use, lacked functional rehab facilities, forcing patients to rely on overcrowded psychiatric wards ( Federal Ministry of Health, 2022 ). Despite these Gaps , the NDCMP’s harm reduction pillar saw success in Benue State, where peer-led naloxone distribution by YouthRISE Nigeria reduced opioid overdose deaths by 25% between 2020 and 2022 ( YouthRISE Nigeria, 2022 ). Harm Reduction Initiatives YouthRISE Nigeria’s naloxone program trained 500 community health workers and peers to administer the opioid antagonist, targeting high-risk groups such as commercial drivers and sex workers. This intervention not only saved lives but also increased community awareness of overdose prevention ( YouthRISE Nigeria, 2022 ). However, scaling such programs nationally remains hindered by stigma and limited funding ( Adelekan et al., 2021 ). 4.3.2 Community Interventions Vocational Training Programs Community-based vocational training programs, such as those implemented by NGOs in rural Niger and Kaduna states, reduced drug-related crime by 30% among participants ( Okeke et al., 2022 ). These programs provided alternatives to drug trade participation, targeting unemployed youths with skills in agriculture, tailoring, and carpentry. For example, 60% of participants in a Kaduna program reported sustained income generation, reducing reliance on illicit activities ( Okeke et al., 2022 ). Faith-Based Counselling Faith-based organizations, such as NASFAT in Lagos and the COCIN Church in Plateau State, integrated spiritual counselling with vocational support, improving treatment adherence by 40% ( Okeke et al., 2022 ). These programs leveraged religious leaders to reduce stigma, with 75% of participants in Plateau State citing reduced shame in seeking help ( Adelekan et al., 2021 ). However, reliance on prayer over evidence-based treatments sometimes delayed medical interventions ( Adelekan et al., 2021 ). 4.3.3 Healthcare Integration Psychiatric Bed Occupancy Drug-related admissions occupied 30% of psychiatric beds nationwide in 2022, straining mental health infrastructure ( Federal Ministry of Health, 2022 ). For example, the Neuropsychiatric Hospital in Lagos reported that 45% of its admissions were linked to cannabis-induced psychosis or opioid withdrawal ( Okafor et al., 2023 ). Only 15% of planned rehabilitation centres are operational due to underfunding and inefficiencies, leaving thousands without access to treatment. Compounding this, the National Drug Law Enforcement Agency (NDLEA) can prosecute just 20% of cases due to budgetary constraints, enabling cartels to thrive with impunity ( Adelekan et al., 2021 ; NDLEA, 2023 ). State governments amplify the problem: Lagos’s lack of pharmacy regulations allows 70% of codeine to be sold illegally, feeding a black market (ICIR, 2023). This inertia at all governance levels creates a vacuum where criminal networks flourish. 4.4.2 Law Enforcement Corruption: The Trafficking Enabler The NDLEA’s mission is undermined by internal collusion. A 2023 investigation revealed that 60% of drug seizures at Lagos ports involved bribes, permitting methamphetamine and fentanyl to infiltrate markets (ICIR, 2023). Such corruption cements Nigeria’s status as a West African trafficking hub ( UNODC, 2021 ), as officials prioritize personal gain over public safety. 4.4.3 Healthcare System Gaps: Urban-Rural Divide Nigeria’s healthcare system mirrors societal inequities. Rural areas have 1 addiction specialist per 500,000 people, compared to 1 per 50,000 in cities ( Oluwaseun et al., 2023 ). This complicity highlights how profit motives override public health, as businesses capitalize on weak enforcement. 4.4.6 Educational and Religious Institutions: Missed Opportunities Schools lacking prevention programs see 37% tramadol abuse among students ( Oshodi et al., 2022 ). Meanwhile, faith-based groups prioritize prayer over medical treatment, resulting in relapse rates 25% higher than secular programs ( Adelekan et al., 2021 ). These institutions, pivotal in shaping norms, neglect evidence-based solutions. 4.5 Interconnected Consequences These collective inactions form a self-reinforcing cycle: - Underfunded rehab centres push addicts into overcrowded hospitals. - Corrupt seizures flood communities with drugs, exploited by pharmacies. - Stigma and spiritualization deter treatment, worsening mental health. - Unregulated schools and religious advice normalize risky behaviours. Nigeria’s drug crisis is not a failure of one sector but a collapse of collective responsibility. Only through integrated reforms—combating corruption, investing in healthcare, and dismantling stigma—can this cycle be broken. The UNODC (2018) highlights rural-urban divides, but no studies stratify findings by geopolitical zones, obscuring tailored interventions. For instance, the North’s cultural conservatism may underreport female drug use, while the South’s commercial hubs face opioid trafficking (ICIR, 2023). Longitudinal data is absent, limiting understanding of evolving trends ( Abdulmalik et al., 2019 ). Recommendation : A nationwide survey using WHO (2021) guidelines could address these Gaps , with oversampling in underrepresented regions. 5.0.2 Policy Implementation Failures The National Drug Control Master Plan (NDCMP) remains under-evaluated. However, no studies test interventions like community dialogues to reduce stigma ( Adelekan et al., 2021 ). Recommendation : Independent oversight bodies and INTERPOL partnerships ( UNODC, 2021 ) could disrupt trafficking networks. 5.3 Mental Health Comorbidities Untreated depression among 58% of Abuja drug users ( Okafor et al., 2023 ) underscores the need for integrated care. Child labour linked to parental addiction ( UNICEF, 2022 ) remains unaddressed in policy. Recommendation : Mandate mental health screening in rehab programs and fund trauma- informed care models. Community Interventions Peer-led programs in Benue reduce overdoses by 25% (YouthRISE, 2022), but scalability is unstudied. Redirecting resources to rural areas, such as allocating 30% of NDLEA funds for drone surveillance of trafficking routes, can help address this imbalance. Developing mobile clinics and partnering with NGOs to provide harm reduction services can ensure that rural communities are not left behind. Policymakers must also integrate rural economic empowerment programs into the NDCMP to address the underlying drivers of drug abuse. The urban-rural divide in drug abuse prevalence in Nigeria highlights the need for region- specific interventions and a more equitable distribution of resources. Urban areas require stricter regulation of pharmaceuticals and targeted prevention programs, while rural areas need economic support, healthcare access, and addiction treatment services. Standardizing research methodologies and improving data quality are essential for understanding the full scope of the crisis and informing effective policies. By addressing the unique challenges of both urban and rural regions, Nigeria can develop a more comprehensive and inclusive response to the drug crisis, ensuring that no community is left behind. 6.5 LIMITATIONS While this systematic review provides valuable insights into the prevalence, patterns, and impact of drug abuse in Nigeria, it is not without limitations. These limitations highlight areas for improvement in future research and underscore the challenges of synthesizing evidence from diverse sources. 6.5.1 Language Bias The review included only English-language studies, which may have excluded relevant research published in other languages, such as indigenous Nigerian languages or French (given Nigeria’s proximity to Francophone countries). This language restriction could introduce bias and limit the comprehensiveness of the findings, particularly in capturing region-specific nuances. 6.5.2 Geographical Bias A significant proportion of the included studies focused on urban areas, particularly Lagos and Abuja, while rural regions were underrepresented. This urban-centric focus limits the generalizability of the findings to rural populations, where drug abuse patterns and contributing factors may differ substantially. The lack of rural data also hinders the development of targeted interventions for these underserved communities. 6.5.3 Heterogeneity in Study Designs and Measures The high variability in study designs, methodologies, and measurement tools (e.g., cross- sectional vs. mixed-methods studies) introduced significant heterogeneity, making it challenging to pool data for meta-analysis in some cases. This variability limits the ability to draw definitive conclusions and underscores the need for standardized research protocols in future studies. 7.0 Limited Longitudinal Data The review primarily relied on cross-sectional studies, which provide a snapshot of drug abuse trends but do not capture changes over time. The prevalence of drug abuse, the devastating impact of inaction, and the systemic barriers to treatment reveal a nation at a crossroads. Yet, within this crisis lies an opportunity for transformation, provided we act with urgency, unity, and a shared sense of responsibility. Drug abuse in Nigeria is not an isolated problem; it is a symptom of deeper systemic failures— poverty, unemployment, weak governance, and cultural stigma. It thrives in the shadows of neglect, fuelled by the collective inaction of stakeholders across sectors. From the bustling streets of Lagos to the conflict-ravaged villages of the Northeast, the faces of this crisis are diverse, but the pain is universal.
