Abstract

Abstract Background :: Unilateral mandibular hyperplasia (UMH) often leads to facial asymmetry and occlusal disturbances, with no unified diagnostic or therapeutic guidelines currently available.Therefore, this study aims to establish principles for developing individualized treatment protocol and illustrate some refinements in surgical approach by presenting a series of UMH cases. Case Presentation :: A retrospective analysis was conducted on 8 adult UMH patients (aged 19-39). All patients presented with facial asymmetry and occlusal deviation. Treatment involved three protocols incorporating techniques such as modified intraoral vertical ramus osteotomy(IVRO) with synchronous condylectomy and modified sagittal split ramus osteotomy(SSRO) enabling single-stage nerve dissection. Postoperatively, patients achieved restored facial symmetry, stable occlusion, functional improvement, and no recurrence. Conclusion :: Despite limitations including a small sample size and lack of pediatric cases, this study establishes a systematic framework based on the "surgery-first" principle for adult UMH. Integrating digital technology with refined surgical techniques can effectively achieve facial and functional reconstruction.

Presentation\u003c/strong\u003e:: A retrospective analysis was conducted on 8 adult UMH patients (aged 19-39). All patients presented with facial asymmetry and occlusal deviation. Treatment involved three protocols incorporating techniques such as modified intraoral vertical ramus osteotomy(IVRO) with synchronous condylectomy and modified sagittal split ramus osteotomy(SSRO) enabling single-stage nerve dissection. Postoperatively, patients achieved restored facial symmetry, stable occlusion, functional improvement, and no recurrence.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusion\u003c/strong\u003e:: Despite limitations including a small sample size and lack of pediatric cases, this study establishes a systematic framework based on the \"surgery-first\" principle for adult UMH.
