Background Digestive endoscopy nurses play a crucial role in the process of endoscopic diagnosis and treatment, and specially trained nurses can improve the safety of digestive endoscopy surgery. Methods Following a thorough review of pertinent guidelines, expert opinions, and literature pertaining to the diagnosis and treatment of digestive endoscopy, a preliminary set of core competency evaluation indicators for digestive endoscopy nurses was established through group deliberation. Subsequent refinement of these indicators and their respective weights was achieved through two rounds of delphi expert consultation and the application of the analytic hierarchy process, resulting in the finalization of the evaluation index system. Results In the two rounds of letter inquiry, the authority coefficient of experts was 0.865 and 0.875 respectively, and the coordination coefficient of Kendall of the second round was 0.152,0.243 and 0.334 ( P <0.05).The final core competence evaluation indexes included 5 first-level indexes, 11 second-level indexes and 48 third-level indexes. Conclusion The core competence evaluation index system is characterized by a high degree of scientific rigor and reliability, effectively capturing the essential competencies necessary for the role of digestive endoscopy nurses. This system serves as a valuable tool for enhancing post-competence and elevating the overall quality of nursing care, offering a sound basis for improvement in clinical practice. digestive endoscopy talent training specialist nurse core competence Background The trend of specialized nursing development globally is towards a more specialized and skilled workforce, as the complexity of healthcare needs increases and the need for highly trained nurses becomes ever more crucial. Additionally, with the rise of technology and the integration of digital health tools, the role of specialty nurses is evolving to embrace new competencies and deliver patient-centered care efficiently and effectively[ 1 ].With the continuous expansion and innovation of endoscopic diagnosis and treatment technology, and gradually advancing to the goal of comprehensive development, innovative development and standardized development, higher requirements are put forward for the theoretical knowledge and operation skills of nursing staff [ 2 ]. Several studies have pointed out that digestive endoscopy nurses play a vital role in the process of endoscopic diagnosis and treatment, and specialized trained nurses can improve the safety of digestive endoscopy operation[ 3 , 4 ].If the core competencies of specialized nurses in digestive endoscopy are inadequate, it can lead to a range of risks.These include decreased diagnostic accuracy, poor patient outcomes, increased complication rates during endoscopic procedures, and a potential for legal liability[ 5 ]. Currently, the core competency evaluation indicator systems for specialized nurses exist in various fields such as critical care, geriatrics, pediatrics, oncology, mental health, and emergency nursing[ 6 ]. These systems aim to assess nurses' clinical skills, patient outcomes, professional development, and interdisciplinary collaboration.Specialized nurses in digestive endoscopy differ significantly from those in other fields, as they focus on the unique challenges and complexities of endoscopic procedures, including patient preparation, equipment handling, intraoperative assistance, and post-procedure care. This specialization requires a nuanced understanding of anatomy, physiology, and endoscopy-specific pathophysiology[ 5 ].The emphasis for digestive endoscopy nurses lies in their proficiency in endoscopic techniques, their ability to manage intraoperative risks and complications, and their expertise in patient education and support throughout the endoscopic procedure[ 7 ].Therefore, it is very necessary to formulate the core competence evaluation system of digestive endoscopy specialized nurses and standardize the training of specialized nurses. At present, all provinces and cities in China have trained digestive endoscopy nurses[ 8 – 10 ], But it lacks a unified core competence evaluation index. Through literature review, semi-structured interview and Delphi expert letter inquiry, this study constructed the index system of core competence, and defined the weights of all levels, in order to provide reliable basis for the cultivation and evaluation of core competence of digestive endoscopy nurses. Methods Design The Delphi method is frequently employed in gathering expert opinions on practical issues, utilizing the technique to address issues of replication by aggregating the opinions or judgments of individual experts [ 11 ]. This process involves multiple rounds of questionnaires distributed to a panel of experts [ 12 ].This study consists of two phases: (1) preresearch: Following a thorough review of pertinent guidelines, expert opinions, and literature pertaining to the diagnosis and treatment of digestive endoscopy, a preliminary set of core competency evaluation indicators for digestive endoscopy nurses was established through group deliberation. (2) Delphi stage: On the basis of the first draft, the Delphi expert letter questionnaire was designed, and two rounds of Delphi expert letter consultation were conducted to reach an agreement within the group. The first draft of the evaluation index system for the core competence of nurses specialized in digestive endoscopy To understand the current research status and refine the competency components for digestive endoscopy specialist nurses, we searched PubMed, Medline, Web of Science, Cochrane Library, CNKI, and Wangfang databases from inception to September 2023. We focused on relevant systematic evaluations, meta-analyses, and original papers published in English or Chinese. Using the iceberg model as a theoretical basis, we extracted five main dimensions of competency: clinical nursing, judgmental thinking, professional development, communication and management, and clinical teaching and scientific research. Four primary indicators, eleven secondary indicators, and forty-seven tertiary indicators comprised the first draft of the competency indicators for digestive endoscopy specialist nurse, which was compiled using the information obtained from the literature review. Delphi process The expert panel The expert panels participating in the Delphi survey were from different regions and organizations in China. Inclusion criteria included ① education background: Bachelor’s degree and above; ②professional title: intermediate and above;③ work experience and years:engaged in clinical nursing, digestive endoscopsic nursing,nursing management in hospitals for 10 years or more in a department with digestive endoscopic or work experience as a digestive endoscopic nurse;④ All experts agreed to participate in this study voluntarily. Data collection Based on the preliminary draft, a questionnaire was designed to assess the digestive endoscopy competencies of nurses from the perspective of experts. The questionnaire encompassed three sections: general information about the experts, the main body of the survey, and a self-evaluation form specifically for experts. Each indicator's importance was rated using a 5-point Likert Scale, ranging from 5 (very important) to 1 (very unimportant)[ 13 ]. Additionally, a column was included for experts to provide their revision opinions and suggestions for additional indicators. To ensure anonymity, experts did not meet or know each other; communication was limited to the investigator. Each expert had the freedom to propose additions, deletions, modifications, or suggest new indicators that were not included in the original questionnaire. In July and August 2023, two rounds of Delphi expert correspondence were conducted via email, with the consent of the experts. After distributing the questionnaire, we engaged in extensive communication with the experts, explaining the purpose and requirements of the survey, clarifying any questionable entries, and requesting a response via email within two weeks. The competency indicator index weights were calculated using the Delphi method, with indicators scoring a mean value of 4 or higher considered significant. A coefficient of variation of 25% or less and at least 75% consistency among experts were established as screening criteria[ 12 ]. To prevent the deletion of crucial indicators, only one indicator meeting the deletion criteria was removed after consultation with the research team. We took into account expert opinions and made appropriate additions, mergers, and revisions to the items, ultimately developing a comprehensive evaluation index for nurses' digestive endoscopy competencies. Data analysis Statistical analysis was conducted utilizing SPSS 27.0 software, with descriptive analysis incorporating mean values, standard deviation, coefficient of variation, and proportion. The level of experts' engagement was quantified through the effective return rate of the questionnaire[ 14 – 15 ].. Expert authority was evaluated based on judgment and familiarity with the questions, while Kendall's coefficient of concordance was employed to gauge the alignment of expert opinions. The study included indicators scoring an average of 4 or higher from a minimum of 80% of the experts[ 16 ]. Ethical considerations The ethics Committee of Shanxi Bethune Hospital (Shanxi Academy of Medical Sciences) granted approval for this study, with ethics number YXLL-2023-042. The participants have provided their consent to participate, and they have been assured that their data will be kept anonymous and confidential. Results Basic information of experts A total of 20 experts were ultimately included. Among them, 8 were engaged in nursing management, accounting for 40% of experts, 6 were engaged in clinical nursing, accounting for 30.00% of experts, and 7 was engaged in hospital digestive endoscopic nursing, accounting for 35.0% of experts. The average age was 46.40 ± 5.40years, and the average work experience was 23.65 ± 6.26years. The Delphi expert panel demographics(Table 1 ). The respective effective return rates for these rounds were 100% and 95.24.00%, resulting in an overall return rate of 97.62%. Specifically, in the first round, 16 experts (constituting 76.19% of the total expert pool) offered their detailed opinions, while 12 experts (representing 60.00% of the total experts) did the same in the second round. Authority coefficient of experts The authority coefficient (Cr) of experts was determined by combining their ability to make informed judgments (Ca) and their familiarity with the surveyed indicators (Cs) according to the formula: Cr = (Ca + Cs) / 2[ 17 ]. In our study, the Cas scores for the expert panels in the two rounds of surveys were 0.854 and 0.846, respectively, and the Css scores were 0.876 and 0.904, respectively. Consequently, the authority coefficients (Cr) were calculated to be 0.865 and 0.875. Coordination degree of expert opinions The degree of consensus among expert opinions was measured through the coefficient of variation (CV) and the coordination coefficient. In the two rounds of surveys, the CVs ranged from 0.08–0.20 and 0.00-0.28, respectively. The coordination coefficient was assessed using Kendall's coefficient of concordance, which indicated values ranging from 0.130–0.252 and 0.152–0.334 for the indicators in both survey rounds. Furthermore, based on Kendall's coefficient of concordance, the p-values for the second, and third-level indicators were all calculated as 0.000, indicating statistical significance. These findings suggest a high level of agreement among the experts' opinions (Table 2 ). The survey's initial round elicited forty comments, suggesting additions, deletions, subdivisions, consolidations, and first-level wording modifications to the competency indicators. The research team meticulously evaluated and debated the experts' feedback, subsequently revising and refining the indicators accordingly.Three tertiary indicators were added,namely,Use method and precautions of endoscopic accessories, biological monitoring method of digestive endoscopy and cause analysis of nosocomial infection and intervention measures.Two tertiary indicators were deleted: endoscopic ultrasound image recognition and nursing coordination of confocal microendoscopy.Four tertiary indicators are modified: "endoscopic diagnosis and treatment of common diseases of digestive system" is modified to "image recognition of endoscopic diagnosis and treatment of common diseases of digestive system"; "self-protection of endoscopic radiation" is modified to "self-protection knowledge of endoscopic radiation"; "preoperative preparation, nursing cooperation and postoperative precautions" is modified to "preoperative preparation, nursing cooperation and relevant knowledge of digestive endoscopic diagnosis and treatment"; "gastroenterology, capsule endoscopy, amplification and pigment endoscopy" is modified to "nursing cooperation of endoscopy and colonoscopy, capsule endoscopy, small oscopy and duodenoscope". All reserved indicators were included in the second round of the expert survey. Results of the second round of the Delphi survey In the second round of the survey, two tertiary indicators indicators were modified: "ability to start emergency response procedures quickly when a patient has complications" was changed to "ability to start emergency plan quickly when a patient has complications"; "endoscopic cleaning, disinfection and sterilization process" was changed to "knowledge of endoscopic cleaning, disinfection and sterilization". Finally, 5 first-level indicators, 11 second-level indicators, and 48 third-level indicators were determined (Table 3 ). practice 4.16 ± 0.49 0.12 94.74 0.0063 Discussion Reliability of research results The training of specialized nursing talents is an important direction for the development of nursing in the new era, which requires good clinical professional skills and solid professional knowledge of digestive endoscopy[ 1 ]. This study draws on the theoretical framework of nurses' core competence, and adopts various research methods, including literature research, semi-structured interview, Delphi method and hierarchical analysis method, to construct the core competence evaluation index system[ 18 ]. The application of hierarchical analysis helps to determine the weight of core competence indicators and improve the scientificity of research results[ 19 ]. The effectiveness of the Delphi method mainly depends on the representativeness, enthusiasm and authority of the consulting experts: In this study, 15 experts were selected, who are from 10 provinces and cities, with a certain regional representation. The main research fields of experts include clinical medicine, nursing management, clinical nursing, etc[ 20 – 22 ]. Experts are associate senior or above, and more than half of experts have worked in the field of digestive endoscopy for more than 15 years and have rich working experience in this field. The positive coefficient of experts in the two rounds of letter inquiry was 100%, and 12 and 6 experts proposed suggestions in the two rounds, which had high enthusiasm of experts. Expert authority coefficient (Cr) 0.700 can be considered as high expert opinion, The mean coefficient of authority of the two rounds of expert inquiry was 0.870 and the coefficient of variation was 0.094, indicating that the results of the inquiry were reliable.Furthermore, we employed consensus-based percentages to strengthen the reliability of the findings. Specialty characteristics of constructing competency indicators for digestive endoscopy nurse The evaluation index system of core competence of digestive endoscopy nurses includes the evaluation of five core abilities of nursing practice ability, judging thinking ability, professional development ability, communication and management ability, and clinical teaching and scientific research. The core competence of specialized nurses in digestive endoscopy is formulated on the basis of the core competence framework of specialized nurses combined with the current development status and future requirements of the department[ 23 ], The endoscopic equipment maintenance, digestive endoscopy and treatment postoperative precautions, endoscopic biopsy techniques and normative specimen management, endoscopic preoperative preparation and intraoperative nursing cooperation, according to the patient's actual situation to determine endoscopic treatment nursing related problems to solve the best strategy, can accurately predict the risk of endoscopic related complications, can early identification or timely find endoscopic operation related complications, etc, More focused on the cultivation of specialist nurses in digestive endoscopy, More professional, More inclined to cultivate senior clinical nursing specialists, It fully reflects the core competence characteristics of digestive endoscopy nurses. This core competence evaluation index system is helpful for the comprehensive evaluation of digestive endoscopy specialist nurses, and provides reasonable evaluation tools for their selection, training, assessment and management. Content analysis of the developed competency indicators for digestive endoscopy nurse The current work developed a set of core competency indicators for digestive endoscopy nurses in China on the basis of the “iceberg model” and defined a series of knowledge, skills, and personal traits essential for digestive endoscopic nursing competency[ 24 ].Foreign research believes that Specialized nurses play multiple roles in the clinical practice, and the ability evaluation of specialized nurses is of great significance for patients and specialized nurses themselves, specialized nursing practice, and specialized nursing management[ 25 , 26 ]. In the evaluation of the core index system, the top three weights are clinical nursing practice ability (0.453), professional development ability (0.168), judging thinking ability (0.142), and the weights are the theoretical knowledge (0.244), master the endoscopic specialist nursing skills operation (0.209), and the ability to communicate and coordinate with patients (0.084). Among the first level indicators, clinical nursing practice competence has the highest weight, which indicates that clinical nursing practice ability is the core competence of the most important work, which compares with the results of other types of specialized nurses unanimous. The weight of the specialized nursing theoretical knowledge of endoscopic diagnosis and treatment ranks first, further highlighting the importance of nurses' professional ability and accomplishment in the post competence. According to the evaluation index system, digestive endoscopy specialist nurses can analyze their own shortcomings and determine the direction of improvement. Nursing managers or teachers can use the evaluation system to dynamically evaluate the core competence level of digestive endoscopy specialist nurses, and give classified guidance based on individual differences. Limitations Owing to challenges like limited research funding and time restrictions, we were able to distribute the questionnaire to merely 15 experts hailing from diverse regions of China. Although the panel of experts demonstrated exceptional authority, reliability, and representation, their opinions might not accurately reflect the general consensus. Furthermore, the competency indicators for digestive endoscopy, which were developed in this study, need to undergo further clinical trials to confirm their practical utility. Conclusion The current research has established scientific and reliable competency assessment metrics for nurses specializing in digestive endoscopy, drawing inspiration from the iceberg model. These metrics encompass five primary indicators, eleven secondary indicators, and forty-eight tertiary indicators. The implementation of these metrics is expected to significantly impact the selection, training, assessment, and evaluation of nurses in this domain.
