Abstract

A case of ileum endometriosis presented with ileus is reported. The case is a 33-year-old woman, who has had pain all over the abdominal region at menstruation and has been receiving medication under a diagnosis of chronic pancreatitis. On February 14, 1990, she was hospitalized urgently because of accentuated abdominal pain. Since gastrographin revealed a stenotic shadow at the terminal ileum, she was introduced to the surgical unit under the ileus.Surgically, an inflammatory tumor was palpable about 10 cm orally from the terminal part of the ileum; on the oral side from the tumor, ileum dilated and serosa partially presented a coarse, fine granular appearance. Ileocecalresection was performed. Ileal endometriosis is so rare that only 17 cases have so far been reported in Japan. It commonly occurs in one's thirties, with not more than unipara, complicated intrapelvic endometriosis-incidence of 50%, ileus symptom-bearing rate of 80%, and without apprecialble differences between Japan and the West.Usually, we should take medical care of patients complaining of any menstruation-related abdominal pain not only by dealing with it as a mere “menstrual pain”, but also bearing a possible intestinal endometriosis in mind.
