However, the benefit of adjuvant chemotherapy (aCT) in patients aged ≤ 40 years with luminal A-like BC remains unclear. This study evaluates the impact of aCT on clinical outcomes in this population. Methods: We conducted a retrospective analysis of a multicenter cohort comprising 23,134 eBC patients treated between 1990 and 2014. Eligible patients were aged ≤ 40 years with luminal A-like tumors (SBR grade 1 or 2, endocrine receptor (ER)-positive, HER2-negative). The impact of aCT on disease-free survival (DFS) and overall survival (OS) was assessed using multivariate Cox regression, adjusting for key prognostic factors. Results: A total of 464 patients met the inclusion criteria, of whom 295 received aCT. Patients who underwent aCT had more unfavorable prognostic features, including younger age, larger tumors, higher grade, lymphovascular invasion, and nodal involvement. Multivariate analysis demonstrated a significant OS benefit with aCT (HR = 0.21, 95% CI [0.05–0.84], p = 0.028 ), whereas DFS improvement did not reach statistical significance (HR = 0.57, 95% CI [0.27–1.22], p = 0.147 ). Independent predictors of OS included tumor size, macroscopic lymph node involvement, and radiotherapy, while only tumor size was significantly associated with DFS. Conclusions: In patients aged ≤ 40 years with luminal A-like eBC, aCT confers a significant OS benefit. Although a trend toward improved DFS was observed, statistical significance was not achieved. Further studies are warranted to refine patient selection criteria and optimize therapeutic strategies for this subgroup. early breast cancer young patient luminal A adjuvant chemotherapy Figures Figure 1 Figure 2 Figure 3 INTRODUCTION Breast cancer (BC) is the most common malignancy globally and the leading cause of cancer-related death in women ( 1 ). Although BC incidence increases with age, approximately 7% of cases occur in women aged ≤ 40 years ( 2 )( 3 ), where it represents over 40% of all cancer diagnoses in this age group ( 4 ). Interestingly, while the incidence of BC in postmenopausal women varies significantly between developed and developing countries, BC in young women occurs at relatively consistent rates across different populations, highlighting the role of distinct risk factors in this subgroup ( 5 ). In premenopausal women, BC onset is closely linked to non-modifiable risk factors such as family history and genetic mutations ( 6 , 7 ). Furthermore, a nonlinear correlation exists between age and BC survival ( 8 , 9 ): younger women face a more challenging prognosis, whereas patients aged between 45 and 50 years old exhibit the best survival rates, with prognosis gradually declining with increasing age. The adverse impact of young age on BC outcomes has been long acknowledged ( 10 – 12 ) though it is not the primary pejorative factor when considering other clinical parameters ( 13 ). The poorer prognosis in young women's BC can be attributed to multiple factors, including delayed diagnosis ( 14 ), lack of access to organized screening programs (which typically commence at age 50 in France), socioeconomic disparities ( 15 ), higher incidence of recurrence or relapse ( 16 ), and unique biological characteristics ( 17 , 18 ). Notably, young BC patients exhibit a substantial proportion of triple-negative, luminal B-like, and HER2-positive subtypes compared to older patients, who are predominantly affected by luminal A-like carcinomas ( 19 , 20 ). Nevertheless, the Endocrine Receptor (ER)-positive subtype is notable and predominant among young patients ( 21 , 22 ) although it does not always translate to a better prognosis, as observed in older patients ( 23 , 24 ). Young age appears to have a particularly significant impact on patients with luminal BC ( 25 , 26 ). Several European guidelines have been published to support the effective management of these complex patients ( 27 , 28 ). To determine the intensity of treatment, multiple factors should be considered, as is the case with older women. Historically, however, adjuvant chemotherapy (aCT) has been considered indispensable for treating young patients with early breast cancer (eBC). A pivotal Danish study by Kroman et al., which included nearly 10,000 women aged ≤ 50, demonstrated that the negative prognostic impact of young age was primarily observed in patients who did not receive aCT ( 29 ). Based on these findings, young women were classified as high risk and often received cytotoxic treatment primarily due to their age. Other studies have corroborated that, despite poorer survival outcomes, younger patients derive greater benefit from aCT compared to older patients ( 30 ). This advantage may be partly attributed to chemo-induced amenorrhea, regardless of estrogen receptor status ( 31 ). Paradoxically, very young women (≤ 35 years) are less likely to experience chemo-induced amenorrhea ( 32 ), and even when present, the endocrine effect of aCT alone is often insufficient ( 33 ). Striking a balance between the benefits and risks of aCT is crucial, as its long-term consequences include infertility, premature menopause, sexual dysfunction, osteoporosis, weight gain, cardiovascular complications, neurotoxicity, cognitive decline, and secondary malignancies ( 34 )( 35 ). Thus, defining the optimal treatment strategy for young women with eBC remains a complex and unresolved challenge. The question of aCT’s benefit is particularly relevant in the context of luminal A-like eBC, where patients generally have a favorable prognosis. To address this critical gap in evidence, our study aims to comprehensively evaluate the impact of aCT on survival outcomes in young women aged ≤ 40 years with luminal A-like eBC. PATIENTS AND METHODS Study Design and Data Source Our data were retrospectively extracted from a prospective multicentric database comprising 23,134 patients who underwent primary surgery for eBC, from 15 different French academic centers between 1990 and 2014. Patients eligible for analysis had confirmed invasive BC with clinical stage T0, T1, T2 or T3 (TNM breast cancer 8th edition). Patients with neo-adjuvant systemic therapies were excluded. Patients aged ≤ 40 with luminal A-like tumor were selected and divided in two groups: patients treated by aCT as population of interest and patients not treated by aCT as a control group. Luminal A-like tumors were defined based on low Scarff-Bloom-Richardson (SBR) tumor grade (1 or 2), positive ER status, and the absence of HER2 (Human Epidermal Growth Factor Receptor 2) overexpression. The definition of the luminal A-like subtype was pathological and not determined by molecular biology. Figure 1 represent the population flow chart. We analyzed the following histopathological characteristics: diagnostic period, age, tumor size in binary value (< 30 or ≥ 30mm), histological type, SBR grade, lymph node treatment by sentinel lymph node biopsy (SLNB) and/or axillary lymph node dissection (ALND), final lymph node status, lymphovascular invasion (LVI) based on examination of slides stained with hematoxylin-eosin ( 36 ), and estrogen and progesterone receptors (with a positivity threshold of 10%). Final lymph node status was categorized in two groups: no invasion or isolated tumor cells (pN0), and microscopic to macroscopic invasion (pN1). Treatments, including type of surgery, radiation therapy (RT) and endocrine therapy (ET) were analyzed. The adjuvant treatments were not standardized according to the centers but adhered to European standards and the recommendations applicable at the respective times. Disease Free Survival (DFS) defined as time to surgery to first event (relapse, metastasis, or death) and Overall Survival (OS) defined as time to surgery to death were analyzed. All procedures involving human participants were done according to the French ethical standards and the Helsinki declaration. This study was approved by our Clinical Research Department under the reference NAC-TS-IPC 2021-026; (ClinicalTrials.gov-NCT02869607). Statistical Analysis Standard descriptive statistics (mean, standard deviation, median and ranges for quantitative variables, and counts and frequency for categorical variables) were used to describe patient and tumor characteristics. Deaths with no evidence of recurrence were treated as competing events in cumulative incidence analyses. Factors associated with DFS and OS were determined in univariate and multivariate analysis. DFS and OS were estimated using the Kaplan-Meier method and compared with the log-rank test. Binary logistic regression was built to determinate factors independently associated with aCT prescription. Multivariate Cox analysis including aCT, ET, RT, tumor size, SBR grade, and nodal status was built. Hazard ratio (HR) was determined for each parameter with a 95% confidence interval (CI) and a p value whose the level of significance was 0.05 . All statistical tests were two-sided. Statistical analyses were performed with SPSS 16.0 (SPSS Inc., Chicago, Illinois, USA) and R version 3.6.3 (R Foundation for Statistical Computing, Vienna, Austria). RESULTS Characteristics of the study population Of 23,134 patients in the database, 464 patients aged ≤ 40 years with luminal A-like tumor were analyzed. Among them, 295 (64%) were treated by aCT (Fig. 1). The characteristics of these cohorts are described in Table 1 . Median age at diagnosis was 37.87 years. Patients ≤ 40 years with luminal A-like tumors predominantly presented with small tumors < 30mm (83%), ductal histological type (84%), pN0 (62%), without LVI (60%). SBR grade 2 tumors were twice as prevalent (66%) as grade 1. Treatment modalities predominantly consisted of lumpectomy (68%), RT (87%), and ET (89%). Most relapses were metastatic (50%). Patients who received aCT exhibited a higher prevalence of unfavorable prognostic features, including a very young age ≤ 35 (36% compared to 15% in the group without aCT), larger tumor size ≥ 30mm (23% versus 5%), SBR grade 2 (80% versus 43%), LVI (38% versus 15%), and pN1 (54% versus 10%). Patients with aCT also more frequently underwent mastectomy (38% versus 20%), RT (90% versus 82%), and ET (94% versus 80%). Notably, metastatic, and contralateral relapses were more prevalent in the aCT group (76% and 80% of this type of relapse), while axillary relapses were more common in the group without aCT (83%). Nevertheless, there were no significant differences in histological type, estrogen, and progesterone receptor status. Factor associated with aCT prescription Factors independently associated with aCT use in a binary logistic regression ( Table 2 ) included a larger tumor size ≥ 30mm (Odd ratio (OR) 5.29, 95%CI 2.04–13.71, p < 0.001 ), SBR grade 2 (OR 6.11, 95%CI 3.34–11.19, p < 0.001 ), and ET (OR 4.43, 95%CI 1.91–10.27, p < 0.001 ). Notably, the diagnostic period from 2007 was independently associated with a lower likelihood of aCT (HR 0.27, 95%CI 0.15–0.50, p < 0.001 ). While the presence of LVI and RT treatment were not significantly associated with aCT prescription, the HR favored the use of aCT. Univariate survival analysis (Fig. 2) The median follow-up for the entire cohort, estimated using the Kaplan-Meier inverted method, was 72.3 months (95%CI 68.7–76.9). Deaths or relapses, in univariate analysis, were more important in aCT group with 51 events among 295 patients (17%) compared to 22 events among 169 patients without aCT (13%). However, the difference was not statistically significant ( p = 0.836 ). The mean DFS in the aCT group was 169 months (95%CI 154–185), with a 5-year DFS of 92% and a 10-year DFS of 74%. In the group without aCT, the mean DFS was 156 months (95%CI 135–178), with a 5-year DFS of 94% and a 10-year DFS of 73%. OS events were more frequent in the aCT group, with 18 events among 295 patients (6%) versus 5 events among 169 patients without aCT (3%). The 5-year OS in the aCT group was 98%, and the 10-year OS was 91%. In the group without aCT, the 5-year OS was 99% and the 10-year OS was 98%. Overall, univariate analysis did not reveal a significant difference in OS between the two groups ( p = 0.437 ). Multivariate survival analysis (Fig. 3) Cox regression analysis included tumor size, SBR grade, nodal status, ET, RT, and aCT identified persistent independent factors associated with DFS and OS. This multivariate analysis is summarized in Table 3 . We observed a significant beneficial effect of aCT on OS (HR 0.21, 95%CI 0.05–0.84, p = 0.028 ), although its impact on DFS did not reach statistical significance (HR 0.57, 95%CI 0.27–1.22, p = 0.147 ). Large tumor size independently correlated with worse DFS (HR 2.40, 95%CI 1.28–4.52, p = 0.007 ) and worse OS (HR 5.04, 95%CI 1.70-14.95, p = 0.004 ). While SBR grade was not significantly associated with survival, the HR suggested a trend toward worse survival in SBR grade 2 (HR 1.51, 95%CI 0.82–2.76, p = 0.184 for DFS; HR 1.51, 95%CI 0.53–4.25, p = 0.437 for OS). Lymph node involvement was independently associated with worse OS (HR 10.57, 95%CI 2.41–46.40, p = 0.002 ) but not significantly associated with DFS (HR 1.57, 95%CI 0.59–2.31, p = 0.648 ). RT was independently associated with better OS (HR 0.15, 95%CI 0.03–0.62, p = 0.009 ) but not significantly associated with DFS (HR 0.89, 95%CI 0.36–2.16, p = 0.791 ). Finally, ET was not significantly associated with survival in our population luminal A-like (with patients mostly treated by ET). DISCUSSION Our findings support a relatively favorable prognosis of luminal A-like eBC in young patients while highlighting a significant OS benefit associated with aCT in multivariate analysis, after adjusting for key prognostic factors such as tumor size, SBR grade, nodal status, and adjuvant treatments. The use of aCT was independently associated with larger tumor size, SBR grade 2, treatment period, and ET use, suggesting a tendency toward more aggressive disease compared to older patients with the same tumor subtype. These observations align with prior studies indicating that young patients with luminal A-like BC may have a more aggressive disease course than their older counterparts ( 25 ). Disease Characteristics and Treatment Patterns Consistent with existing literature, our cohort exhibited a higher rate of distant metastases, reinforcing the established link between younger age and increased recurrence risk ( 2 , 11 ). Patients treated with aCT had higher rates of distant recurrence, whereas those not receiving aCT were more prone to locoregional relapses. Some studies suggest that intensifying locoregional therapy in younger patients could help mitigate this recurrence risk ( 37 , 38 )( 16 , 39 ). Additionally, patients who received aCT typically presented with more unfavorable prognostic features and were more likely to undergo intensive treatment strategies. The observed decline in aCT use from 2007 onward reflects evolving de-escalation strategies and the refinement of treatment recommendations for young patients with low-risk disease ( 40 ). Survival analysis and impact of aCT Our survival analysis confirmed the overall good prognosis of luminal A-like eBC. In univariate analysis, patients who did not receive aCT exhibited fewer events and better 5-year DFS and OS rates. However, these differences were not statistically significant, and DFS trends appeared to reverse at the 10-year mark. In contrast, multivariate analysis revealed a significant OS advantage for aCT-treated patients, underscoring the survival benefit of chemotherapy in this subgroup despite the absence of a statistically significant improvement in DFS. Independent predictors of OS included large tumor size, lymph node involvement, and RT use, while tumor size was the only independent predictor of DFS. SBR grade 2 showed a trend toward poorer survival outcomes, although statistical significance was not reached. The lack of a significant survival association with ET likely reflects its widespread use in this cohort. Our results align with prior studies suggesting that young women experience a roughly 30% reduction in mortality risk with aCT ( 30 ), regardless of molecular subtype or ER status ( 35 ). The benefit is particularly pronounced in patients with lymph node involvement, further reinforcing the prognostic significance of age and nodal status ( 35 ) ( 41 ). These findings also resonate with the RXPONDER study, which supports the role of aCT in patients with intermediate or low recurrence scores ( 42 ). Consequences of aCT While our study lacked data on treatment-related adverse effects and quality of life, the long-term sequelae of aCT in young women are well-documented. These include cardiovascular complications (e.g., post-anthracycline cardiac dysfunction), neurotoxicity, secondary malignancies, and psychological distress such as anxiety and depression ( 47 , 48 ). Fertility preservation and pregnancy management are crucial considerations for young patients. Although most young patients prioritize oncologic treatment over fertility concerns ( 49 ), it remains essential to offer fertility-preserving strategies. Established guidelines provide clear recommendations on fertility preservation and pregnancy management in cancer survivors ( 50 )( 51 )( 52 ). Chemo-induced amenorrhea, a frequent consequence of aCT, has been suggested as a protective factor across BC subtypes. However, in very young patients, the endocrine effects of CT alone are often insufficient due to higher tumor proliferation rates and differences in ER and PR expression ( 31 )( 22 )( 13 )( 33 ). Information about actual menopausal status of patients included in our database was not present, although most of patients ≤ 40 years were likely be pre-menopausal. In addition, the use of adjuvant LHRH analogs was not informed in this population. CONCLUSIONS Young patients with luminal A-like eBC treated with aCT present with more aggressive clinical features at diagnosis, undergo more intensive treatment strategies, and face a higher risk of recurrence. Nevertheless, multivariate analysis underscores a significant OS benefit from aCT in this population, despite the lack of statistically significant DFS improvement. Tumor size and nodal involvement remain critical factors in guiding treatment decisions. Further studies, including meta-analyses, are needed to refine treatment strategies and better identify subgroups that derive the most benefit from systemic therapies. Optimizing treatment decisions is essential to balance efficacy with the minimization of long-term iatrogenic risks in this young population with an otherwise favorable prognosis. Eligible patients were aged\u0026thinsp;\u0026le;\u0026thinsp;40 years with luminal A-like tumors (SBR grade 1 or 2, endocrine receptor (ER)-positive, HER2-negative). The impact of aCT on disease-free survival (DFS) and overall survival (OS) was assessed using multivariate Cox regression, adjusting for key prognostic factors.\u003c/p\u003e\u003ch2\u003eResults:\u003c/h2\u003e \u003cp\u003eA total of 464 patients met the inclusion criteria, of whom 295 received aCT. Patients who underwent aCT had more unfavorable prognostic features, including younger age, larger tumors, higher grade, lymphovascular invasion, and nodal involvement. Multivariate analysis demonstrated a significant OS benefit with aCT (HR\u0026thinsp;=\u0026thinsp;0.21, 95% CI [0.05\u0026ndash;0.84], \u003cem\u003ep\u0026thinsp;=\u0026thinsp;0.028\u003c/em\u003e), whereas DFS improvement did not reach statistical significance (HR\u0026thinsp;=\u0026thinsp;0.57, 95% CI [0.27\u0026ndash;1.22], \u003cem\u003ep\u0026thinsp;=\u0026thinsp;0.147\u003c/em\u003e).
