Abstract

Abstract Objective This study aimed to evaluate the association between beta-blocker use and all-cause in-hospital mortality in critically ill patients with coexisting chronic obstructive airway disease (COAD) and cardiovascular disease (CVD), particularly during the acute exacerbation phase—a context not previously studied. Methods We conducted a retrospective cohort study using data from three publicly available intensive care databases (MIMIC-IV, eICU-CRD, and NWICU). Adult patients with both COAD and CVD who were admitted to the ICU within 48 hours of hospitalization were included. Patients were categorized into beta-blocker and non-beta-blocker groups. Doubly robust estimation methods, including inverse probability of treatment weighting (IPTW) and propensity score matching (PSM), were used to control for confounders. The primary outcome was all-cause in-hospital mortality, with secondary outcomes of ICU and hospital length of stay. Results Among 2,753 included patients, 1,314 received beta-blockers. After adjustment for confounders, beta-blocker use was associated with a significant reduction in all-cause in-hospital mortality (odds ratio [OR] = 0.55; 95% confidence interval [CI] = 0.35–0.86; P = 0.008). They should be taken into account in all patients with chronic obstructive airways disease, specifically systemic arterial hypertension, heart failure, coronary artery disease, and atrial fibrillation. 1, 4 Patients with chronic obstructive airway disease and cardiovascular disease exhibit an increased risk of acute exacerbations and mortality. 5 – 7 Furthermore, the likelihood of severe cardiovascular events remains persistently elevated following an exacerbation of chronic obstructive airway disease, even in its early and moderate stages. 8 Beta-blockers are a cornerstone of the management of cardiovascular disease, such as heart failure and after myocardial infarction. 9, 10 Although these therapies primarily target distinct receptor subtypes (beta1 versus beta2), concerns regarding the specificity of available pharmacological agents for these receptor subtypes have generated uncertainty in the management of patients with coexisting obstructive airway and cardiovascular conditions. Previous research has demonstrated an association between increased airway hyperresponsiveness (AHR) and airflow limitation in individuals with COAD. 11, 12 However, recent observational studies indicate that the use of beta-blockers, particularly cardioselective beta-blockers, in the treatment of cardiovascular disease is not associated with an increased risk of exacerbations in patients with COAD. Instead, these medications have been linked to a reduction in pulmonary exacerbations and mortality among such patients. 13–21 Although this evidence has proven the safety and efficacy of prescribing Beta-blockers to patients with COAD and CVD, the guidelines also recommend that B-blockers should not be denied if there is a CVD indication. 22 , 23 However, all existing suggestions pertain to the stable phase, with no studies investigating the safety and efficacy of beta-blocker administration during the acute exacerbation phase. To address this gap in clinical research, we designed this study to examine the effect of beta-blockers on all-cause mortality in patients with COAD and CVD, using data from three publicly available clinical databases. 24 – 26 Methods This retrospective cohort study used de-identified data from the Medical Information Mart for Intensive Care IV (MIMIC-IV, version 3.1), the eICU Collaborative Research Database (eICU-CRD, version 2.0.1), and the Northwestern ICU (NWICU, version 0.1.0) database. 24 – 26 These datasets are available through PhysioNet ( https://physionet.org/ ). An author (Ao Liu) obtained credentialed access after completing the Collaborative Institutional Training Initiative (CITI) modules “Conflicts of Interest” and “Data or Specimens Only Research” (ID 68232932). Because only publicly available, de-identified data were analyzed, this study was deemed exempt from Institutional Review Board review. Participants The cohort consisted of adult patients admitted to the intensive care unit (ICU) with concurrent COAD and CVD ( Tables S1 and S2 ). Inclusion criteria included: (1) age of 18 years or older, admitted for the first time; (2) documented presence of COAD and CVD, identified through ICD-9-CM or ICD-10-CM diagnosis codes listed in the supplementary tables; (3) ICU admission within 48 hours of hospital admission; and (4) one of either COAD or CVD as the primary discharge diagnosis. Study groups The cohort was divided into two groups based on beta-blocker exposure ( Table S3 ): a beta-blocker group and a non-beta-blocker group. Patients in the non-exposed group received no beta-blocker at any point during hospital treatment. Data collection Data were extracted from three databases using Structured Query Language (SQL) in PostgreSQL 16.4 and Navicat Premium 17. The collected data encompassed: (1) demographic and admission details—age, gender, race, and severity at admission assessed by APS III and SOFA scores; (2) interventions, including beta-blockers, vasopressors, and types of respiratory support; (3) comorbidities such as malignancy, chronic kidney disease, and diabetes mellitus; (4) initial vital signs, including mean arterial pressure, respiratory rate, and heart rhythm; (5) laboratory findings—pH, PCO2, PO2/FiO2 ratio, potassium, blood urea nitrogen (BUN), hemoglobin (HB), alanine aminotransferase (ALT), aspartate aminotransferase (AST), and creatinine (Cr) levels; and (6) time from hospital admission to admission. Outcomes The primary outcome was all-cause hospital mortality. Secondary outcomes included ICU and hospital length of stay. Statistical analysis Statistical analysis was performed using doubly robust estimation methods to comprehensively evaluate the association between beta-blocker use and clinical outcomes, including mortality and length of stay. 27 Initially, comprehensive data preprocessing was conducted, in which continuous variables underwent conservative winsorization at the 1st and 99th percentiles to mitigate the influence of extreme values, followed by median imputation for missing data. 28 Propensity scores were estimated via multivariable logistic regression incorporating a comprehensive set of demographic characteristics, clinical variables, laboratory values, and severity scores. 29 We applied doubly robust estimation by combining inverse probability of treatment weighting (IPTW) with outcome regression modeling. 30 Specifically, we used stabilized weights calculated from the propensity scores and truncated between 0.01 and 100, then fitted outcome regression models that included all baseline covariates within the weighted cohort. 31 Treatment effects were evaluated using a range of complementary analytical approaches, including crude analysis, multivariable adjustment, doubly robust estimation via propensity score matching (PSM), and IPTW, with all relevant variables being accounted for. 32 Additional sensitivity analyses were performed using the same double robust IPTW method, excluding patients with a hospital length of stay of less than 24 hours. For binary outcomes, we employed logistic regression to estimate odds ratios with corresponding 95% confidence intervals. In contrast, for continuous outcomes, we used linear regression with robust standard errors (HC3 type) to address potential heteroscedasticity. 33 Covariate balance was rigorously assessed using standardized mean differences, with SMD values below 0.1 indicating adequate balance. 34 All statistical analyses were performed using R version 4.4.1 utilizing specialized packages including MatchIt for propensity score matching, survey for weighted analyses, and lmtest and sandwich for robust variance estimation, with statistical significance defined as two-sided p-values < 0.05. Results Baseline characteristics of patients This multicenter retrospective cohort study analyzed data from three intensive care databases (eICU, MIMIC-IV, and NWICU). We included adult patients (≥ 18 years) with a first hospital admission for asthma or COPD who required ICU care within 48 hours and had documented CVD comorbidity. The final cohort included 2,753 patients: 1,314 received beta-blockers (treatment group), with 1,134 receiving only beta-1 blockers, 100 receiving only non-selective beta-blockers, and 80 receiving both; the remaining 1,439 did not (Fig. 1 ). Significant baseline differences were observed between the two groups before adjustment (SMD > 0.1), particularly in respiratory support and PO₂/FiO₂ ratio (Table 1 ). To address these confounders, propensity scores were estimated using a generalized linear model (GLM) with 23 covariates. Key predictors of beta-blocker use included PO₂/FiO₂ ratio, time from hospital to ICU admission, non-invasive/high-flow respiratory support, PCO₂, diabetes, age, and vasopressor treatment ( Figure S1 ). The findings indicated that, across the entire patient cohort, beta-blocker administration was significantly associated with a reduction in all-cause in-hospital mortality (odds ratio [OR] = 0.55; 95% confidence interval [CI] = 0.35–0.86; P = 0.008). This association demonstrated robust consistency across multiple causal inference methodologies, including crude analysis (OR 0.61, 95% CI 0.44–0.84, P = 0.002), multivariable adjustment models (OR 0.51, 95% CI 0.34–0.75, P < 0.001), and PSM analyses (OR 0.59, 95% CI 0.38–0.91, P = 0.017). Sensitivity analysis, employing IPTW while excluding patients with hospital stays less than 24 hours, substantiated these results (OR 0.56, 95% CI 0.36–0.87, P = 0.010). Subgroup analysis stratified by primary diagnosis (Table 2 ): Among patients with COAD and COPD, no statistically significant association between beta-blocker therapy and all-cause in-hospital mortality was observed (all P > 0.05). No significant difference was observed in the COPD subgroup receiving β1-blockers (OR = 1.80; 95% CI, 0.83–3.92; P = 0.138). In patients with CVD, beta-blocker administration was associated with a significant reduction in all-cause in-hospital mortality (OR 0.37, 95% CI 0.21–0.64, P < 0.001). Improvement also persisted among those receiving beta 1-blockers (OR 0.40, 95% CI 0.23–0.68, P < 0.001). For ICU length of stay (Table 2 ): In the primary analysis employing IPTW adjusted for all covariates, no statistically significant difference was observed between the beta-blocker treatment group and the control group (mean difference [MD] 0.34, 95% confidence interval [CI] -0.07 to 0.75, P = 0.101). This finding was corroborated across alternative analytical approaches, with all P-values exceeding 0.05. For hospital length of stay (Table 2 ), IPTW analysis revealed no significant difference in total hospital duration between the two cohorts (MD = 0.41, 95% CI = -0.42 to 1.23, P = 0.336). Complementary analytical methods produced consistent non-significant results. Sensitivity analyses for both ICU and hospital length of stay yielded analogous findings. Discussion This study, which involved a large cohort of critically ill patients hospitalized with COAD and CVD, demonstrated that the risk of mortality was significantly lower among patients receiving beta-blockers than among those not receiving them (OR, 0.55; 95% CI, 0.35–0.86). In subgroup analysis, beta-blockers primarily benefited patients diagnosed with CVD; however, they did not demonstrate benefit or harm in patients mainly hospitalized to COAD. Approximately 86% of patients prescribed beta-blockers were administered selective beta-1 blockers, and roughly 84% of patients hospitalized due to COAD were primarily diagnosed with COPD. Consequently, these findings pertain predominantly to this patient population and their use of beta-1 blockers. As we noted in the introduction, the conclusion from early studies about the benefit or harm of prescribing beta-blockers for patients with COAD and CVD remains debated. While some studies suggest beta-blockers might increase the risk of acute bronchospasm in people with asthma, similar concerns apply to patients with COPD. Evidence indicates decreased forced expiratory volume in one second (FEV1), heightened airway hyperresponsiveness, and increased mortality among patients taking non-selective beta-blockers or high doses of cardioselective beta-blockers. 35 – 38 Compared to non-cardioselective beta-blockers, cardioselective beta-blockers have an affinity for beta-1 receptors that is at least 20 times greater than for beta-2 receptors. This theoretically translates to a significantly lower risk of inducing bronchoconstriction—a characteristic of particular importance for patients with coexisting pulmonary diseases such as COPD and Asthma. 39 While the post hoc analysis of the MISOAC-AF randomized trial found that in recently hospitalized patients with AF, when comorbid with COPD, there was no difference in all-cause mortality, cardiovascular mortality, and hospitalizations between those using cardioselective versus non-cardioselective beta-blockers. 16 However, an observational study that included 14,789 COPD patients post-myocardial infarction comparing bisoprolol to carvedilol shows that the bisoprolol group had lower incidence rates of mortality, major adverse cardiac and cerebrovascular events, heart failure hospitalization, and significant adverse pulmonary events compared to the carvedilol group. 18 The BLOCK-COPD trial, the first randomized controlled trial (RCT) conducted in 2019, evaluated metoprolol in patients with moderate-to-severe COPD. The study was terminated early after the metoprolol group exhibited a higher incidence of severe exacerbations compared to the placebo group, with a rate ratio of 1.51 for severe exacerbations and 3.71 for very severe exacerbations. 40 Importantly, these patients did not indicate beta-blocker therapy for cardiovascular protection. Recent evidence indicates that cardioselective beta-blockers may be well tolerated in patients with asthma. A double-blind, randomized, crossover study demonstrated that the response to salbutamol following mannitol challenge was noninferior in individuals with mild-to-moderate asthma receiving regular bisoprolol compared with placebo. 20 Additionally, a meta-analysis published in 2025 indicates that patients with asthma may tolerate cardioselective beta-blockers better than non-selective ones. When strong cardiovascular reasons are present, it may be cautious to consider cardioselective beta-blockers for patients with asthma. 19 For patients with COPD, some observational studies and meta-analyses show more benefits than in patients with asthma; cardioselective beta-blockers may reduce all-cause mortality and exacerbations without causing harm to pulmonary function. 14 – 20 The latest BICS study, a UK double-blind RCT evaluated the effect of bisoprolol on preventing exacerbations in high-risk COPD patients, which including a total of 515 patients were randomly assigned to receive either bisoprolol or a placebo for one year, showed that the annual exacerbation rates were similar between the bisoprolol and placebo groups (2.03 vs. 2.01), with no significant difference in the incidence of serious adverse events. 41 In summary, during the stable period, cardioselective beta-blockers should not be withheld from patients with COAD if indicated. They should not be used to hope for obtaining additional benefits if not indicated. Beta-blockers reduce morbidity and mortality in people with CVDs, such as ischemic heart disease, heart failure, atrial fibrillation, and hypertension. 9 , 42 – 44 The administration of beta-blockers in critically ill patients strictly adheres to the principle of prioritizing hemodynamic stability in accordance with indication-based criteria, particularly for specific conditions such as the management of atrial fibrillation-related tachyarrhythmias. Their use is contraindicated in cases of acutely decompensated heart failure. Meta-analytic data suggest that beta-blocker therapy is associated with reduced long-term mortality in this patient population. No significant differences were observed in mean arterial pressure or vasopressor requirements between patients receiving beta-blockers and control groups. Additionally, the use of beta-blockers during ICU resuscitation appears hemodynamically safe and does not adversely affect clinical outcomes, even when administered concomitantly with vasopressor agents. 45 , 46 Our findings indicate that beta-blockers can reduce mortality among critically ill patients hospitalized primarily for cardiovascular conditions, consistent with these established perspectives. Implications Through a comprehensive analysis of patients across three clinical databases, this study offers a valuable reference for clinicians evaluating the risks and benefits of beta-blocker therapy in critically ill patients with COAD and CVD. Evidence demonstrates that beta-blockers are safe for this patient population. Subgroup analysis indicates that cardiac-selective beta-blockers do not significantly increase mortality or lengthen hospital stays in patients with COPD who require hospitalization for exacerbations. For patients primarily hospitalized for cardiovascular conditions, the rational administration of beta-blockers can reduce hospital mortality. These findings provide a foundation for future high-quality clinical investigations, such as in patient cohorts with asthma and heart failure or COPD and myocardial infarction. Limitations Despite extensive adjustments for a broad spectrum of known confounders—including demographic factors, clinical variables, laboratory results, and disease severity indices—the observational nature of this study cannot completely exclude the possibility of residual confounding arising from unmeasured variables. Additionally, inherent limitations in data quality, such as inaccuracies or incomplete documentation of smoking history and comorbidities in the database, may further introduce bias into the analyses. Due to the paucity of data, an analysis of the impact of asthma and non-selective beta-blockers on patient prognosis was not conducted. Accordingly, as a cohort study based on a database, the generalizability of these findings may be limited, and establishing definitive causal relationships necessitates validation through RCTs, which are explicitly designed to mitigate confounding and bias. Conclusions This study establishes that beta-blockers are not contraindicated in critically ill patients presenting with concomitant COAD and CVD. When indicated, cardioselective beta-blockers should be administered promptly. Future multicenter prospective investigations are imperative to examine the potential synergistic effects of beta-blockers in patients with COPD and/or asthma comorbidity across various CVDs, to improve patient outcomes, and to refine therapeutic approaches. Adult patients with both COAD and CVD who were admitted to the ICU within 48 hours of hospitalization were included. Patients were categorized into beta-blocker and non-beta-blocker groups. Doubly robust estimation methods, including inverse probability of treatment weighting (IPTW) and propensity score matching (PSM), were used to control for confounders. The primary outcome was all-cause in-hospital mortality, with secondary outcomes of ICU and hospital length of stay.\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e \u003cp\u003eAmong 2,753 included patients, 1,314 received beta-blockers. After adjustment for confounders, beta-blocker use was associated with a significant reduction in all-cause in-hospital mortality (odds ratio [OR]\u0026thinsp;=\u0026thinsp;0.55; 95% confidence interval [CI]\u0026thinsp;=\u0026thinsp;0.35\u0026ndash;0.86; P\u0026thinsp;=\u0026thinsp;0.008). Subgroup analysis indicated that the mortality benefit was observed primarily in patients hospitalized for CVD (OR\u0026thinsp;=\u0026thinsp;0.37; 95% CI\u0026thinsp;=\u0026thinsp;0.21\u0026ndash;0.64; P\u0026thinsp;\u0026lt;\u0026thinsp;0.001), but not in those hospitalized for COAD. No significant differences were found in ICU or hospital length of stay between the two groups.\u003c/p\u003e\u003ch2\u003eConclusion\u003c/h2\u003e \u003cp\u003eBeta-blockers are not contraindicated in critically ill patients with COAD and CVD and may reduce mortality in those hospitalized primarily for cardiovascular indications.
