Abstract

Abstract Background: The study aims to examine the mediating role of physical activity attitudes in the relationship between university students' e-health literacy levels and obesity awareness. Methodology: The study was designed with a relational research model, one of the quantitative research methods. A total of 333 university students, 159 males and 174 females, participated in the study. Pearson correlation analysis was applied to examine the relationship between e-health literacy, obesity awareness, and physical activity attitudes. In the study, e-health literacy was employed as the independent variable; obesity awareness was used as the dependent variable, and positive and negative attitudes toward physical activity were used as the mediating variables. The bias-corrected bootstrapping 5000 method was employed to ascertain the significance of the mediating variable, with the lower and upper limit intervals being checked to ensure that no zero values were present. Results: The mediating effect of positive physical activity attitudes between e-health literacy and obesity awareness was found to be positive and significant (β=0.0112, t=4.259, p=<.001, CI=[0.060-0.163]. The mediating effect of negative attitudes toward physical activity between e-health literacy and obesity awareness was insignificant (β=0.0162, t=0.913, p=0.361, CI=[-0.018-0.049]). A predictive effect of e-health literacy and physical activity attitudes on obesity awareness was identified, with a value of 57%. The study revealed that e-health literacy accounted for 25% of the negative attitudes toward physical activity and 42% of the positive attitudes toward physical activity. As e-health literacy levels increase, so too does obesity awareness. Figures Figure 1 INTRODUCTION Obesity, a growing global public health issue, is becoming crucial day by day [ 1 ]. In societies where access to food is limited and living standards are relatively low, the obesity problem is as prevalent as in societies where access to food is abundant and living standards are higher [ 2 ]. Obesity and overweight lead to many health problems such as hypertension, Alzheimer's, cardiovascular diseases, gallbladder diseases, liver fattening, arthrolith, sleep disorders, high cholesterol, type 2 diabetes, musculoskeletal disorders, respiratory, skin, and reproductive problems [ 3 , 4 ] and as a result, it creates a huge burden on health expenditures [ 5 ]. Obesity arises from a complex interplay of environmental factors, genetic predisposition, and human behavior [ 6 ]. The potential factors influencing and causing obesity are pretty diverse. Several factors cause obesity, including decreased physical activity, short sleep duration, increased television viewing time [ 7 ], an unbalanced diet, socio-economic status, gender [ 8 ], education level, stress/depression intensity, annual household income [ 9 ], smoking and alcohol consumption, bad habits [ 10 ], and biological factors such as genetics, prenatal determinants, pregnancy and menopause [ 11 ]. In order to reduce the prevalence of obesity, individuals' levels of knowledge about obesity, their attitudes toward it, and their awareness of obesity should be considered [ 12 ]. Increasing the awareness of individuals against obesity and fostering a more positive attitude toward physical activity play a pivotal role in reducing and eliminating health risks [ 13 ]. Increased obesity awareness also helps individuals review their daily life choices and avoid potential triggers that may contribute to the emergence of obesity [ 14 ]. It was observed that the implementation of intervention programs within the context of the change in individuals' perceptions of obesity awareness led to an increase in awareness levels; indeed, positively reflected on dietary behaviors [ 15 , 16 ]. In this respect, it can be stated that health literacy-based intervention programs play a vital role in enhancing students' obesity awareness. Across the community, through the programs to be implemented, it is possible to facilitate access to physical activity, expand opportunities for physical activity, and increase awareness and attitudes toward physical activity [ 17 ]. Increasing the attitudes and participation of individuals toward physical activity in their daily lives positively increases life satisfaction, mental health, physical fitness, and self-perception while dramatically reducing the risks of obesity, asthma, diabetes, heart disease, and cognitive decline [ 18 – 20 ]. In this context, while some practices aim to foster an attitude toward physical activity and obesity awareness, health literacy can play a crucial role in this process [ 21 ]. Raising individuals' awareness about the negative consequences of obesity can facilitate health literacy and a greater interest in the issue [ 22 ]. In recent years, the accessibility of information from electronic sources, particularly in light of the advancements in internet technology by digitalization, has elevated the significance of the e-health literacy concept. E-health literacy refers to the individual's capacity to access, evaluate, and use accurate information using health technologies [ 23 ]. It is thought that making healthy decisions thanks to the individual's acquisition of these skills may positively affect obesity awareness and attitudes toward physical activity. In the context of raising obesity awareness and attitudes toward physical activity, e-health literacy is becoming increasingly evident. It is stated that e-health literacy is mutually related to participation in physical activity or attitude toward physical activity [ 21 ]. Chrissini and Panagiotakos [ 24 ] reported that individuals with high levels of health literacy have superior knowledge and skills in obesity management. It was stated that knowledge about health issues effectively increased obesity awareness and physical activity levels [ 25 ]. Another study reported that young people who read articles and newspapers exhibited higher levels of obesity awareness [ 26 ]. In this context, obesity awareness, attitudes toward physical activity, and e-health literacy are important intertwined health issues [ 23 ]. Within the scope of the study, the study aims to examine the mediating role of e-health literacy in obesity awareness and attitude toward physical activity among university students. The present study sought to determine whether e-health literacy is a mediating variable in the relationship between obesity awareness and attitude toward physical activity. METHODS Research Group The G-Power 3.0.1 program was used to determine the sample size. α value was calculated as 0.05, medium effect size as 0.15, and Power (1-β) as 0.95. According to the result of Critical F = 19.3500000 and Actual power = 0.9505747, it was detected that at least 129 students should participate in the study. The research group consisted of 333 university students, 159 males (47.7%) and 174 females (52.3%). Of the participants, 95 were 1st-grade students (28.5%), 119 were 2nd-grade students (35.7%), 59 were 3rd-grade students (17.7%), and 60 were 4th-grade students (18.1%). While 97 participants (29.1%) in the research group stated they participated in extra-curricular sportive activities, 236 participants (70.9%) said they did not. The height average of the participants was x = 178.58/Ss = 11.103, and that of weight was x = 68.34/Ss = 16.155. Data Collection Tools E-Health Literacy Scale The scale, whose validity and reliability study was conducted to determine the e-health literacy levels of individuals, consisted of a total of eight items and a single dimension [ 27 ]. The internal consistency coefficient of the scale was determined as 0.97. There were no reverse scoring items in the scale, and it was designed in a five-point Likert type. Confirmatory factor analysis (x2 = 55.261/Df = 16, CFI = 0.988, TLI = 0.980, NFI = 0.984, IFI = 0.988, RMSEA = 0.086, SRMR = 0.014) for the application of the e-health literacy scale on the current research sample shows that the scale has acceptable fit values [ 28 ]. Obesity Awareness Scale The scale, which was developed to determine the obesity awareness levels of individuals and adapted into Turkish by Kafkas and Özen [ 29 ], consists of three sub-dimensions and a total of twenty items. Within the scope of the study, only the obesity awareness sub-dimension of the scale was included in the research. The obesity awareness sub-scale consists of nine items in total. The scale was designed as a four-point Likert type (29). Confirmatory factor analysis (x2 = 81.16/Df = 23, CFI = 0.981, TLI = 0.970, NFI = 0.973, IFI = 0.981, RMSEA = 0.087, SRMR = 0.022) for the application of the Obesity Awareness Scale on the current research sample shows that it has acceptable fit values [ 28 ]. Physical Activity Attitude Scale In Children And Adolescents : The scale, whose validity and reliability study were conducted to determine the attitudes of children and adolescents toward physical activity, consists of twelve items in total and two dimensions: positive attitude and negative attitude. The positive attitude sub-dimension comprises seven items, with an internal consistency coefficient of 0.85. The negative attitude sub-dimension includes five items, with an internal consistency coefficient of 0.80. The scale was designed as a five-point Likert type [ 30 ]. Confirmatory factor analysis (x2 = 112.467/Df = 42, CFI = 0.978, TLI = 0.971, NFI = 0.966, IFI = 0.978, RMSEA = 0.071, SRMR = 0.032) for the application of the physical activity attitude scale in children and adolescents on the current study sample shows that it has acceptable fit values [ 28 ]. Data Analysis The Jasp 0.18.3.0 statistical program was used for data analysis. Skewness and kurtosis values were examined to assess the normality distribution of the data. Kline stated that the kurtosis and skewness values should be less than three for the data set to be normally distributed [ 31 ]. Pearson correlation analysis was applied to examine the relationship between e-health literacy, obesity awareness, and attitudes toward physical activity. In the research, the independent variable was e-health literacy, the dependent variable was obesity awareness, and the mediating variables were positive and negative attitudes toward physical activity. The bias-corrected bootstrapping 5000 method was employed to ascertain the significance of the mediating variable. For the significance of the mediation effect, it was checked that there was no zero value between the confidence intervals showing the lower and upper limits [ 32 ]. There was a significant positive relationship between e-health literacy and obesity awareness (r=,730, P = 001). In addition, there was a significant positive relationship between e-health literacy and positive attitudes toward physical activity (r=,648). There was a significant negative relationship between e-health literacy and negative attitudes toward physical activity (r=-,498). There was a significant positive relationship between obesity awareness and positive attitudes toward physical activity (r=,622), while there was a significant negative relationship between obesity awareness and negative attitudes toward physical activity (r=-,467) (Table 1 ). The CFA results indicate that the construct validity of the four-factor measurement model holds acceptable values (χ²=1205.510/Df = 367, p = < .001, CFI = 0,922, IFI = 0,922 TLI = 0,914, RMSEA = 0.083, NFI = 0.892, SRMR = 0.060, GFI = 0,969) (Table 2 ). It was detected that e-health literacy had a positive and significant effect on obesity awareness (β = 0.408, t = 11.696, p = < .001, CI=[0.340–0.477]). It was found that e-health literacy had a negative and significant effect (β=-0.462, t=-10.482, p = < .001, CI=[-0.548, -0.376]) on negative attitudes toward physical activity. E-health literacy was found to have a positive and significant effect (β = 0.643, t = 15.546, p = < .001, CI=[0.562–0.724]) on positive attitudes toward physical activity. The mediating effect of positive attitudes toward physical activity between e-health literacy and obesity awareness was found to be positive and significant (β = 0.0112, t = 4.259, p = < .001, CI=[0.060–0.163]. It was determined that the mediating effect of negative attitudes toward physical activity between e-health literacy and obesity awareness was not significant (β = 0.0162, t = 0.913, p = 0.361, CI=[-0.018-0.049]. The predictive effect of e-health literacy and attitudes toward physical activity on obesity awareness was found to be 57%. It was identified that e-health literacy explained 25% of the negative attitudes toward physical activity and 42% of the positive attitudes toward physical activity (Table 3 ). EHL = E-Health Literacy, OA = Obesity Awareness, PA + = Positive attitudes toward physical activity, PA-=Negative attitudes toward physical activity DISCUSSION It was found that e-health literacy has a direct positive and significant effect on obesity awareness. Studies have shown that lack of health knowledge is associated with health literacy and obesity [ 33 , 34 ]. In their study on adolescents, Chari et al. [ 35 ] associated health literacy with obesity in their study on adolescents and stated that obese adolescents have low health literacy. The findings of this research show that low health literacy is associated with obesity. As a result of their research, Sansom-Daly [ 36 ] et al. remarked that inadequate health literacy is associated with adverse health behaviors. In support of these results, studies showing that obesity is more common in cases of low health literacy reveal the relationship between health literacy and obesity. The common conclusion drawn from the studies in the literature is that low health literacy increases the risk of obesity [ 37 , 38 ]. In this context, the significance of health literacy becomes evident. In other words, it can be stated that as the level of e-health literacy increases, effects such as obesity awareness and physical activity behavior also occur. The literature generally supports the findings of the research. The study revealed that e-health literacy has a positive and significant effect on positive attitudes toward physical activity. It has been seen in the literature that people with adequate health literacy adopt regular physical activity, health-promoting behaviors, and healthy eating habits, and their awareness of obesity increases [ 39 , 40 ]. In this context, it is evident that health literacy encompasses more than the ability to understand basic health information and make the right health-related decisions [ 41 ]. People may engage in physical activity to improve their health and protect themselves from non-communicable health problems. In terms of participation in physical activity, the health literacy of adolescents was higher among participants who were physically active enough than among those with insufficient physical activity levels [ 42 , 43 ]. As proof of this case, there are findings in the literature showing that low health literacy is associated with physical inactivity and sedentary behaviors [ 44 , 45 ]. These results align with the current research findings, as individuals with high physical activity levels can participate in physical activity for different purposes. The fact that physical activity levels of individuals with high health literacy can also be attributed to factors such as awareness of healthy living, learning accurate information about the prevention and effects of diseases, and their tendency toward less problematic behaviors. From a different perspective, it can be said that there is a relationship between low health literacy and obesity [ 46 ]. A healthy lifestyle is a component of the pathway linking health literacy and health outcomes [ 47 ]. Physical activity is among the factors contributing to a healthy lifestyle. The positive correlation between adequate health literacy and physical activity in adults is an outstanding example of this situation [ 48 , 49 ]. It was found that a positive attitude toward physical activity has a mediating effect on e-health literacy and obesity awareness. When the literature is examined, it is seen that factors such as lack of physical activity in adult obese people are associated with low health literacy [ 50 ]. This finding shows that individuals with low health literacy have low obesity awareness and negative attitudes toward physical activity. On the other hand, the negative relationship between e-health literacy and negative attitudes toward physical activity, which are among the current research findings, indirectly supports the lack of mediating effect of negative attitudes toward physical activity between e-health literacy and obesity awareness. Health behaviors such as physical activity strongly predict health outcomes [ 51 , 52 ]. This finding indirectly supports the mediating role of positive physical activity attitudes in the relationship between e-health literacy and obesity awareness. Because physical activity can create a good health status for individuals and protect them from poor health status such as obesity, in another study, it was reported that physical activity promoted good health and played an essential role in preventing non-communicable health problems [ 53 ]. The findings obtained in the current study support this situation. In their research, Yüksel and Akil underline that as the level of physical activity increases, obesity awareness also increases [ 54 ]. This finding supports the interpretation made on the subject. Nevertheless, the literature reveals that poor eating habits and lack of physical activity are among the top factors predisposing adolescents to obesity [ 55 , 56 ]. This situation reveals the importance of adequate physical activity, which helps to prevent non-communicable diseases such as obesity, in the creation of health literacy and healthy lifestyle, which are among health-promoting behaviors. CONCLUSION In conclusion, e-health literacy and health promotion are positive developmental variables that can lead to good health outcomes. By having good health literacy skills in today's technology, individuals can increase their healthy life quality and have knowledge and competence about preventable diseases and health-protective conditions thanks to easy and fast access to health information in an electronic environment. This study concluded that e-health literacy is associated with obesity awareness, and as the level of e-health literacy increases, obesity awareness also increases. On the other side, the current study concluded that there was a positive and significant relationship between e-health literacy and positive attitudes toward physical activity. Last but not least, it was concluded that positive physical activity attitudes mediated the relationship between e-health literacy and obesity awareness. These results show that awareness can be raised about preventable health problems such as obesity by promoting e-health literacy, adding it to programs, and transferring it to students comprehensively. From a different perspective, regular physical activity can reveal healthy life behaviors. In this respect, public health can be protected by encouraging regular physical activity to prevent avoidable health problems such as obesity, which is one of the general public health problems. This research is reproducible by being applied to different sample groups. Limitations The study participants were limited to university students. Since the study has cross-sectional research characteristics, it can be repeated on different sample groups; for example, the moderating effect of different variables on the relationship between e-health literacy and obesity awareness can be tested. The study is limited to quantitative research. As a matter of fact, different findings can be revealed for the current problem by addressing different aspects of the research and adopting mixed research methods. A total of 333 university students, 159 males and 174 females, participated in the study. Pearson correlation analysis was applied to examine the relationship between e-health literacy, obesity awareness, and physical activity attitudes. In the study, e-health literacy was employed as the independent variable; obesity awareness was used as the dependent variable, and positive and negative attitudes toward physical activity were used as the mediating variables. The bias-corrected bootstrapping 5000 method was employed to ascertain the significance of the mediating variable, with the lower and upper limit intervals being checked to ensure that no zero values were present.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults: \u003c/strong\u003eThe mediating effect of positive physical activity attitudes between e-health literacy and obesity awareness was found to be positive and significant (β=0.0112, t=4.259, p=\u0026lt;.001, CI=[0.060-0.163]. The mediating effect of negative attitudes toward physical activity between e-health literacy and obesity awareness was insignificant (β=0.0162, t=0.913, p=0.361, CI=[-0.018-0.049]). A predictive effect of e-health literacy and physical activity attitudes on obesity awareness was identified, with a value of 57%.
