Abstract

Abstract Postnatal care within the first 48 hours after delivery is critical for the health and well-being of both women and their newborns. Despite the benefits of early postnatal care, the 2022 Ghana Demographic and Health Survey reported that only 45% of women received all the recommended postnatal health checks within the period. This study analysed the 2022 Ghana Demographic and Health Survey. A multi-level logistic regression analysis was employed to identify the factors influencing postnatal care completeness. The data was weighted and analyzed using STATA 17.0. The results shows that 53% of women did not receive complete postnatal care within 48 hours. Married women were 57% more likely to receive complete postnatal care compared to unmarried women (OR = 1.57* [95% CI: 1.02, 2.42]). Similarly, women enrolled in the National Health Insurance Scheme were 56% more likely to receive full postnatal care (OR = 1.56** [95% CI: 1.20, 2.04]).

Figures Figure 1 INTRODUCTION Maternal mortality continues to present as a significant public health concern, especially in developing and resource-constrained settings despite the ongoing efforts by national and international health organizations. In 2015, there were approximately 303,000 maternal deaths globally, with Sub-Saharan Africa (SSA) reporting more than half (201,000) of these deaths [ 1 ] Similarly, in 2020, about 287, 000 women died during pregnancy and immediately after childbirth, with most (87%) of these deaths reported in Sub-Saharan Africa (SSA) and Southern Asia [ 2 ]. Consistently, childbearing women in the SSA region continue to face challenges with access to equitable and quality care [ 3 ]. Postnatal assessment has proven over the years to be a major approach to preventing maternal and child mortality [ 4 ]. Despite this, access and use of postnatal services remain low in SSA. The United Nations for instance noted that, in 2013, only about 13% of the mothers in sub-Saharan Africa received the WHO recommended postnatal assessment [ 5 ]. Postnatal care, especially in the first 48 hours post-delivery is a critical component of the maternal health continuum, as it ensures the well-being of both the mother and the newborn in the vulnerable period after childbirth through recognizing and treating complications associated with childbirth [ 6 , 7 ]. Available evidence suggests that women who undergo at least 3 postnatal assessments between 48–72 hours after delivery experience a good quality of life post-delivery [ 8 , 9 ]. Maternal and child health services in Ghana form an integral part of the primary healthcare system. According to the Ghana Statistical Services (GSS), however, there is a great disparity between the utilization of Antenatal and Postnatal services. Whereas about 98% of women attended Antenatal clinics, less than half of the women received complete postnatal assessment visits [ 10 ]. Similarly, postnatal care service utilization within the critical 48-hour period after childbirth remains low in Ghana despite continuous efforts to integrate the need for post-natal care into antenatal services in Ghana [ 10 ]. This can lead to complications that have the potential to distort the happiness that is often associated with childbirth. Several studies have examined the utilization and determinants of postnatal care in Ghana. However, these studies have primarily focused on the factors associated with the uptake and completion of postnatal care [ 6 , 11 ]. Very little is known about the factors associated with the utilization of immediate (first 48 hours) post-natal care services in Ghana. The aim of this paper, therefore, is to examine the factors associated with postnatal care service utilization within the first 48 hours post-delivery in Ghana. The findings will help identify key barriers and facilitators to accessing timely postnatal care, which can inform targeted interventions to improve maternal and neonatal health outcomes. Additionally, the findings can guide policymakers and healthcare providers in addressing gaps in service delivery to ensure that women have access to quality healthcare within the first 48 hours after delivery in Ghana. METHODS AND MATERIAL Study design, study setting The study used secondary data from the 2022 Ghana Demographic and Health Survey (GDHS). The GDHS collected data on several indicators regarding children, women, and men across the country. The 2022 GDHS used a quantitative cross-sectional survey to collect data from women who had live births three years before the conduct of the survey. The women's file of the GDHS was used for this study. The study deemed women who delivered within the past two years preceding the study as qualified to be included in the study. This selection is necessary to avoid recall bias, particularly among multiparous women. Ghana is a West African country located along the Atlantic coast with a population of approximately 30.8 million people. Ghana is divided into 16 regions, each with capitals, and made up of metropolitans, municipalities, and districts. Ghana shares borders with Togo to the east, Burkina Faso to the north, Cote d'Ivoire to the west, and the Gulf of Guinea to the south. Measurement of variables The outcome variable for this study was Post Natal Care (PNC) completeness. A composite variable was generated using three variables from the 2022 DHS women's file. The variables include; blood pressure discussed vaginal bleeding, and discussed family planning. A total of eleven variables (age, marital status, educational level, religion, place of delivery, NHIS, antenatal attendance, household head, residence, wealth index, and region) were selected from the women's file and modeled as determinants of PNC completion. A pre-categorized age variable with six categories (15 to 19 years, 20 to 24 years, 25 to 29 years, 30 to 34 years, 35 to 39 years, and 40 to 49 years) was used for this study. Also, marital status was categorized as never married, married, living with a partner as well as widowed, divorced, and separated which were grouped as a single category to increase the power of the models that were used. The educational level of women was also used as a predictor and the categories include having no education and having primary, secondary, or tertiary education. Religion was categorized into no religion, Christian, Islam, and Traditionalist. In addition, the place of delivery (home, public, or private facility) was selected as a possible determinant of completing PNC. Regarding having NHIS, the women were categorized into having NHIS (Yes) and not having NHIS (No). The required ANC attendance of four or more by the WHO was used to categorize ANC into less than 4 and greater or equal to 4. Other variables included are household heads (Male or Female), Residence (Urban or rural), and wealth index measured as poorest, or poorer. Finally, the region of residence which included the sixteen administrative regions of Ghana was also analyzed as a determinant. Data analysis All data were analyzed using STATA version 17.0. The results are presented in the form of tables, charts, and graphs to ensure a comprehensive visual and statistical understanding of the findings. The data analysis was conducted as follows: Univariate Analysis: Descriptive statistics were calculated for all variables, with results displayed in tables, charts, and graphs. This analysis highlighted the proportions and percentages of participants' socio-demographic characteristics. In addition to descriptive statistics, the independent variables and the dependent variable (ANC initiation) were measured and visualized through bar charts. The univariate analysis also evaluated the individual effects of the independent variables on the dependent variable, providing a foundational understanding of the data. Multivariate Analysis: A binary logistic regression model was applied to assess the relationship between ANC initiation (the dependent variable) and the independent variables. This model allowed for a deeper understanding of the factors that influence ANC initiation when controlling for multiple variables. RESULTS Completeness of postnatal care within 48 hours The study explored the completeness of the required postnatal care services within 48 hours. It was found that 46% of women had completed postnatal care services within this period. Demographic and household characteristics of respondents The results indicated that 49.2% of respondents aged 40–49 completed their postnatal care within 24 hours, while 49.0% of married women did so. Among those with tertiary education, 51.9% completed their postnatal care within 24 hours, and 50.2% of women who delivered in public facilities achieved the same. Additionally, 48.8% of women with National Health Insurance (NHIS) and 47.7% of those who attended antenatal care (ANC) more than three times completed their postnatal care within 24 hours. Postnatal care completion within 48 hours was also higher among women in households with male heads (46.3%) and those residing in urban areas (49.2%). Furthermore, 51.1% of women from the wealthiest group on the wealth index completed their postnatal care within 24 hours. Except for women’s age, religion, household head, and wealth index variables, all other factors were statistically significant as shown in Table 1 . Factors associated with postnatal care within 48 hours After controlling for confounding variables in Model III, it was found that the odds of completing postnatal care were 57% higher among married women compared to those who were never married [aOR = 1.57, 95% CI: 1.02, 2.42]. Additionally, women who delivered in public hospitals [aOR = 3.71, 95% CI: 2.67, 5.15] and private facilities [aOR = 3.13, 95% CI: 1.77, 5.53] had significantly higher odds of completing postnatal care compared to those who delivered at home. Women with National Health Insurance (NHIS) had 57% higher odds of postnatal care completion, while those who attended antenatal care more than three times had 56% higher odds compared to those without NHIS or fewer than four antenatal visits. All these variables were statistically significant in relation to postnatal care completeness. Random effect models The interclass correlation (ICC) reported in Table 2 showed that showed that the ICC in model O was 41%. This indicates that 41% of the variation in postnatal care completeness is found across the cluster. The variation between the clusters decreases in Model I by 40%, model II by 37%, and model III by 37%. Model III was selected for interpretation and discussion because it has the least AIC of 15375.79 among the other models indicating a good fit and avoiding unnecessary complexities. The results indicate that approximately half of the women completed their postnatal care during this critical period, which is essential for both maternal and newborn health. Previous studies in Ghana and other sub-Saharan African countries have highlighted the importance of early postnatal care, with varying rates of utilization. [ 12 ] found that 51.7% of women in Ghana received postnatal care within two days, a finding that closely aligns with the present study, suggesting consistency in postnatal care utilization over time. In contrast, [ 13 ] reported a higher percentage (61.8%) of women receiving postnatal care within the first week in the Greater Accra Region. This discrepancy is likely due to their longer time frame compared to the 48-hour focus in this study. Similarly, [ 14 ] found that 81% of facility-based deliveries in Ghana received postnatal care within 48 hours, underscoring the role of delivery location in determining postnatal care utilization. In a broader sub-Saharan African context, [ 15 ] analyzed data from 30 countries and reported postnatal care utilization rates ranging from 9.9–73.6%, with Ghana at 68.3%. While the present study’s findings fall towards the lower end of this range, the variation may reflect differences in healthcare access, policies, and data collection methods across different settings. For instance, [ 16 ] Ethiopia reported only 20.2% of women received postnatal care within 42 days, highlighting the significant variability in utilization across countries. These variations emphasize the need for targeted interventions to increase postnatal care utilization in Ghana, particularly in the critical first 48 hours after delivery, as the current rates suggest room for improvement in maternal health services. Marital status was found to be a significant factor in postnatal care utilization. Married women had 57% higher odds of completing postnatal care compared to unmarried women. This finding is consistent with existing literature, including studies by [ 6 ] and [ 17 ], which reported that married women were more likely to utilize maternal health services. The social support, financial stability, and shared decision-making that often come with marriage may explain this association. However, some studies, such as [ 18 ], did not find a significant association between marital status and postnatal care, suggesting that the impact of marital status may vary depending on the study context and other confounding factors like education and economic status [ 19 ]. These results highlight the importance of considering marital status when designing interventions to improve postnatal care utilization, particularly for unmarried women who may be more vulnerable to missing out on essential care. Place of delivery was also a significant predictor of postnatal care completion. Women who delivered in public or private health facilities were significantly more likely to complete postnatal care compared to those who delivered at home. This finding aligns with previous research, including studies by [ 14 ] and [ 13 ], which demonstrated that institutional delivery increases the likelihood of receiving postnatal care. Facility-based deliveries offer immediate access to healthcare providers, established referral systems, and greater awareness of postnatal care’s importance, contributing to higher postnatal care completion rates. However, as noted by [ 20 ] facility delivery alone does not always guarantee postnatal care completion, suggesting that the quality of care and follow-up mechanisms may also play a role. These findings underscore the need for interventions that not only promote institutional deliveries but also ensure that women who deliver at home are not left behind in accessing postnatal services. Health insurance coverage (NHIS) and antenatal care (ANC) attendance were also key factors influencing postnatal care completion. Women with NHIS who attended ANC more than three times had significantly higher odds of completing postnatal care compared to uninsured women or those with fewer ANC visits. This finding is supported by studies from Ghana, such as those by [ 21 ] and [ 22 ], which showed that NHIS coverage increases maternal health service utilization, including ANC and postnatal care. The financial barriers reduced by NHIS, combined with increased health education during ANC visits, may explain this positive relationship. However, as noted by [ 23 ], health insurance alone may not guarantee postnatal care utilization in all regions, as other factors such as distance to health facilities and cultural practices may also play a role. These findings suggest that strategies to increase NHIS coverage and encourage multiple ANC visits could enhance postnatal care utilization, but further efforts are needed to address barriers that persist despite insurance coverage. CONCLUSION In conclusion, this study highlights the importance of marital status, place of delivery, health insurance, and antenatal care attendance in determining postnatal care completion within the first 48 hours after delivery in Ghana. The findings underscore the need for targeted interventions to improve postnatal care utilization, particularly among unmarried women and those delivering at home. Increasing NHIS coverage and promoting institutional deliveries could enhance postnatal care services, ultimately contributing to better maternal and neonatal health outcomes. RECOMMENDATIONS Based on the findings from this study, we recommend that women who are not married should be given special education during their antenatal care period to ensure to utilize the complete postnatal care services within the critical hours after birth. Also, women should be encouraged to deliver at health facilities to ensure automatic postnatal care at birth. As such, the TBAs who deliver at home should be incorporated into the health facility system to ensure all women receive care after delivery, In addition, the free maternal health policy should be well implemented to ensure all women have active NHIS cards to ensure that there is no barrier in finance. All regions should get equal distribution of health care resources in terms of both human and capital resources to help bridge the gap of inequity in service access. Religion was categorized into no religion, Christian, Islam, and Traditionalist. In addition, the place of delivery (home, public, or private facility) was selected as a possible determinant of completing PNC. Regarding having NHIS, the women were categorized into having NHIS (Yes) and not having NHIS (No). The required ANC attendance of four or more by the WHO was used to categorize ANC into less than 4 and greater or equal to 4. Despite the benefits of early postnatal care, the 2022 Ghana Demographic and Health Survey reported that only 45% of women received all the recommended postnatal health checks within the period. This study analysed the 2022 Ghana Demographic and Health Survey. A multi-level logistic regression analysis was employed to identify the factors influencing postnatal care completeness. The data was weighted and analyzed using STATA 17.0. The results shows that 53% of women did not receive complete postnatal care within 48 hours. Married women were 57% more likely to receive complete postnatal care compared to unmarried women (OR\u0026thinsp;=\u0026thinsp;1.57* [95% CI: 1.02, 2.42]).
