Введение. Эндометриоз – это распространенное заболевание, основными проявлениями которого являются тазовая боль и бесплодие. Глубокий инфильтрирующий эндометриоз (ГИЭ) как одна из форм эндометриоза вызывает грубое повреждение структур малого таза. Для многих женщин с бесплодием хирургическое лечение ГИЭ, вероятно, является единственным вариантом восстановления фертильности, но при этом его роль в развитии бесплодия не доказана клиническими исследованиями, а лечение связано с высоким риском операционных осложнений. Тем не менее ведущие исследовательские группы рекомендуют хирургическое лечение при ГИЭ у женщин с бесплодием для повышения частоты спонтанных беременностей, а также перед ВРТ. Цель. Оценить роль хирургического лечения ГИЭ у женщин в лечении бесплодия по данным отечественной и зарубежной литературы. Материалы и методы. Проведено изучение публикаций, касающихся хирургического лечения ГИЭ с целью повышения фертильности, в базе данных медицинских публикаций PubMed, в источниках удаленного доступа, рекомендациях ведущих исследовательских групп. Заключение. Использование хирургического лечения ГИЭ с целью повышения фертильности не подтверждено качественными контролируемыми исследованиями, но может быть применимо для снижения симптомов эндометриоза у женщин перед ЭКО. Оценка индекса фертильности позволяет выбрать оптимальную тактику ведения пациентки. Согласно ASRM лапароскопическое хирургическое вмешательство при ГИЭ является наилучшим подходом для повышения фертильности. Introduction. Endometriosis is a common disease, the main manifestations of which are pelvic pain and infertility. Deep infiltrating endometriosis (DIE), as one of the forms of endometriosis, causes severe damage to the pelvic structures. For many women with infertility, surgical treatment of DIE is probably the only option for restoring fertility, but its role in the development of infertility has not been proven by clinical studies, and treatment is associated with a high risk of surgical complications. Nevertheless, leading research groups recommend surgical treatment for DIE in women with infertility, to increase the frequency of spontaneous pregnancies, as well as a pre-IVF stage. Purpose. To study the current state of the problem of surgical treatment of GIE in infertility. Materials and methods. A study of reports on the topic of surgical treatment of DIE in order to increase fertility was conducted in the PubMed database of medical publications, in remote access sources, and recommendations from leading research groups. Conclusion. The use of surgical treatment of DIE in order to increase fertility has not been confirmed by qualitative controlled studies, but it may be applicable to reduce the symptoms of endometriosis in women before IVF. Evaluation of the fertility index allows you to choose the optimal management strategy for the patient. According to the ASRM, laparoscopic surgery for DIE is the best approach to increase fertility.
