According to the World Health Organization, about 804,000 fatal suicides and several times this number of suicide attempts occurred worldwide in 2017. Evidence suggests that the incidence of suicide varies among Iran provinces. It is essential to know these differences to allocate resources and adopt the right policies. Methods We used data recorded by the Ministry of Health and the National Forensic Medicine Organization during the study years to provide information about the rate of suicide attempts in the provinces of Iran. To accurately calculate the under-reporting of suicide attempts according to the frequency of successful suicide, we used TSsmooth methods in STATA 14 software. We ran Exponential, Moving Average, and H Winter models and the best model was chosen according to the lowest root mean square error. Results There were 30,137 successful suicide attempts and 349342 unsuccessful suicide attempts during the eight-year study period. Around 18,684 suicides resulted in the death of males, and 11,453 were females. Among the unsuccessful suicide attempts, 237,552 cases were males, and 111,790 were females. The incidence rate of eight years of successful suicide based on age in the age group (15–25 years) was higher than in other groups. Conclusion Considering the current situation in Iran, it is suggested that interventional measures be designed to reduce the incidence of suicide attempts, especially in the age group of 15–49 years, and to conduct pilot studies in provinces with relatively higher incidence rates than other provinces of the country. Suicide is among the most critical health challenges globally, ranking 13th among the causes of death worldwide and third in the age group of 15–34 years [1]. Around 1,000 suicides happen every day worldwide, and as many as 10 to 20 times, suicide attempts occur. Every year in the United States, 35,000 people die from suicide. Between 2000 and 2007, 256,085 people died from suicide in the United States. Most suicide attempts occur in Asia, which accounts for more than 60% of all deaths worldwide. India, China, and Japan account for 40% of the world's suicides. The suicide rate leading to death in Iran is 6 per 100,000 population annually [2]. In Islamic countries, suicide is less studied; furthermore, in many Islamic countries, national suicide statistics are not collected or, if collected, are not reported to the World Health Organization. Epidemiological studies in the United States show that about 4.6% of adults attempt suicide, and 14% have severe suicidal ideation [3]. The suicide attempt is quantitatively and qualitatively different from suicide. But suicide attempt is closely related to successful suicide: for example, 20–70% of those who have committed suicide have attempted suicide at least once in the past, and 3–10% of those who attempt suicide eventually die as a result, but suicide attempts are 8–25 times more common than successful suicide. According to the World Health Organization, a suicide attempt is a non-lethal action in which a person intentionally engages in unusual behavior (such as self-injury or eating a substance over the amount prescribed for treatment) without the intervention of others. And his purpose is to achieve his expected changes [4]. The causes of suicide attempts differ in different people. Factors such as frustration or bad mental condition, addiction, drug abuse, financial problems, family challenges, cultural issues, etc., can be involved in its formation. In general, factors such as age, gender, race, psychological factors, socioeconomic situation, family condition, level of access to counseling services, etc., can play a substantial role in the formation of suicide attempts and their recurrence in individuals. Methods of suicide attempts are different in different societies and can be referred to as methods such as self-immolation, hanging, intentional poisoning, etc. [5]. Suicide attempt rates in Iran are lower than most countries, particularly in Western societies, but higher than the Middle East. According to the investigations, Iran is the third country in the world after China and India, where female suicide attempts have increased. However, the highest rate of suicide attempts is related to the youth age group [6]. In a cross-sectional study to investigate lifelong suicide attempts, the results showed that 3.2% of people had attempted suicide in their lifetime, of which 63% were women. The most common age was 15–24 years old, and the most common causes of these actions were marital and family problems. The suicide attempt occurred during the lifetime of 3.9% of Christians and 2.9% Muslims. It was more common in people under permanent stress who drank alcohol, and addicts were at higher risk for a suicide attempt than the general population. Prescriptions of some drugs, especially antidepressants and methadone, were more likely to increase this risk [7]. Given the importance of the subject as a health problem, especially in adults, Moghaddam Nia confirms this issue: "Since epidemiological findings show that suicide attempt has multiple and continuous causes and risk factors, that often take place simultaneously, such as mental disorders, personality disorders, family disputes, etc., the best model for suicide prevention requires a long-term approach in which risk factors are identified and prevented [8]. According to the sensitivity and importance of the phenomenon of suicide attempts and the factors that cause the suicide attempt, analysis of data available in responsible organizations can help us extend our knowledge of the epidemiology of suicide attempts and plan effective health programs to diminish the burden caused by this event on the country's health care system. This study aimed to correct the estimation of the number of cases of suicide attempts (unsuccessful suicide attempts) in the whole country from 2009 to 2016 first to predict the rate of suicide attempts in the country and its provinces and also provide a description of the rate of successful suicide rates in the country in the same period. Materials and Methods Study setting: The study was performed as a cross-sectional design. Data about suicidal attempts were gathered from the information registered in the Ministry of Health from 2009 to 2016. Data from successful suicide was collected from the suicide registry of the legal medicine organization responsible for registering successful suicides in the country. Sampling: We analyzed all the registered data regarding suicide in the country and its provinces from 2009 to 2016. Data collection method: According to Iranian law, the death certificate for all deaths due to suicide has to be issued by the legal medicine organization. Hence the successful suicide statistics are more accurate than the suicide attempts statistics. Suicide attempts are reported to the Ministry of Health, but there is underreporting in some provinces and some years. We checked the ratio of successful suicide to suicide attempts in the country's provinces in each year of the study and calculated the confidence interval of these ratios. The ratios outside the 95% confidence interval were omitted, and then we tried to impute these missing values with TSsmooth methods in STATA14 software. Exponential, single, double, H-winter, and three types of moving average models were implemented. The best model in our data with the lowest root mean square error rate was chosen. Smoothing is an exploratory data analysis technique to reveal the overall shape of a series. TSsmooth models use the moving average process to predict a variable in the model, the first difference of which starts from the lower order, which changes serially compared to the variable's previous value. Moving average models are typically used to extract a trend or signal from a series while eliminating high-frequency components or noise. In exponential and de-exponential models, an adaptive prediction algorithm is implemented, which, using a moving average filter with geometric weight, predicts variable values that are missed at defined intervals. These models automatically detect panel data, placing series and predictions in a long variable. In the exponential smoothing method, closer data weigh more than farther data. This method is used to predict a short time. In the single exponential smoothing method, a weighted moving average is used with weights exponentially geometrically. (Weighted moving average with exponentially decreasing weights). The double exponential smoothing method models the two components' trends and levels. (9). Demographic data were also received from the National Statistics Center to calculate the incidence rates by provinces and the whole country from 2016 to 2019. Using SPSS 24 and Excel 2019 software, incidence rates in different years and other descriptive statistics were calculated, reviewed, and eventually reported. Statistical analyzing : First, to find the fewer reported cases per year for each province, we calculated the ratio of successful to unsuccessful suicides; then, for the calculated ratios during the eight years of study for each province, we calculated the average and 95% confidence interval. The ratios outside the 95% confidence interval were omitted, and then we tried to impute these missing values with TSsmooth methods in STATA14 software. TSsmooth methods, dexponential and exponential models, as well as three models of moving average (1-1-2, 1-1-1, and 2-1-2), and finally, the H Winters model were tested on the data. After comparing RMSE in 6 models, the 2-1-1 moving average model was chosen. The missed values were predicted using this model, and then the number of underreported cases for each year and the province was estimated. Results Descriptive analysis of suicide incidence leading to death from 2009 to 2016 in the country In this study, 30,137 suicides leading to death during eight years between 2009 and 2016 were investigated. Around 62 percent of suicide deaths were men, and 38 percent were women. The average age of the study population in cases of fatal suicides was 31.7 ± 18.8 years. The highest incidence by gender in 2016 for men was 7.8 per 100,000 people. The rate of successful suicides was twice as high in men as in women during the eight years of study. The age group in which the incidence of successful suicide was higher than other groups was 15–24 years old. In 2016, the incidence of suicide was higher than in other years. Most of the suicides leading to death occurred during the eight years of study in urban areas. Suicide rates in the country's provinces during the eight years of study are shown in Table 1 . The overall average incidence during eight years is 5.62 per 100,000 people. The highest incidence rates during these years are related to Ilam province in 2010, equal to 24.7 per 100,000 people. The highest average eight years incidence among the country's provinces is Ilam province, with 18.6 per 100,000 people. After Ilam province, Kermanshah, Lorestan, Hamedan, and Gilan provinces have the highest incidence rates in the whole country with the average eight years incidence rates per 100,000 people (13.6–12.4–8.9 - and 8.5), respectively. The lowest incidence rates are related to Yazd and Sistan and Baluchistan provinces. Results of descriptive analysis of the suicide attempt data (unsuccessful suicide) in the whole country from 2009 to 2016 based on demographic variables In this study, 349,342 cases of suicide attempts (unsuccessful suicide) during eight years from 2009 to 2016 were investigated. Of the unsuccessful suicide attempts, 237552 were men, and 111790 were women. The age average of those who attempted suicide that did not result in death was 35.4 ± 24.5 years. The highest incidence by gender in 2014 was for women with123 per 100,000 people. Unsuccessful suicides, in contrast to successful cases, were more common in women than men during the eight years of study. Women attempt suicide more than men, but men die more due to suicide. The age group in which the incidence of unsuccessful suicide was higher than other groups is 15–24 years old. Most suicide attempts occurred in urban areas during the eight years of study. Analysis of the incidence of suicide attempt (unsuccessful suicide) from 2009 to 2016 by province ،he unsuccessful suicide incidence rates for each province from 2009 to 2016 are shown in Table 1 ,. The average incidence during the eight years of study is 89.4 per 100000 people. The highest incidence rates during these years are related to West Azerbaijan province in 2009, equal to 277 in 100000 people. The highest average of 8 years among the country's provinces is the West Azerbaijan province, with 237 per 100000 people. After West Azerbaijan province, Ilam, Ardabil, Mazandaran, and North Khorasan provinces, with average eight years incidence rates per 100000 people (189–132.5–122.9 - and 120), have the highest incidence rates in the country. The lowest incidence is related to Tehran province. Table 2 shows the amounts of RMSE reported by the six implemented models. Because the RMSEs reported by STATA 14 software for the moving average 112 models was less than other models, all the missing values were imputed according to this model. The data for Tehran province is provided in Table 3 and Fig. 1 and for the other provinces is given in table number one in the appendix file. The blue line in the figure indicates the incidence of unsuccessful suicide attempts, and the orange line shows the incidence of successful suicide atte. The chart of Tehran province is presented in Fig. 1 . (The rest of the provinces are in the appendix). Although the incidence of suicide attempts in the country has changed significantly during the years under study, the results show that the incidence of suicide leading to death in the last three years of the study was almost constant. Still, compared to the beginning of the study, it enhanced by 17%. In cases of unsuccessful suicide attempts, the highest incidence rate in 2014 was 98.4 per 100000 people, and the lowest incidence rate in 2009 was 74.6 per 100000 people. Alvaromka and co-workers' study shows a declining trend in Spanish suicide [10]. Zinki et al. also exhibited a steady trend of suicide in Australia [11]. Maybe one of the reasons for the increasing trend is related to improving the registration system, increasing drug abuse, unemployment in high-incidence provinces, increasing depression, political issues such as sanctions, and economic problems such as fluctuating inflation in the country [12, 13]. As shown in the tables, the incidence of successful and unsuccessful suicide attempts in the country's provinces shows a different trend. Some provinces have shown an increasing trend, some a decreasing trend, and others a variable trend. The trend obsrved in the provinces can be attributed to factors such as economic, social, migration, unemployment, increase or decrease in population, improving the registration system, Improving the diagnosis of people with varying degrees of mental disorders, educating and counseling individuals and families of people who have attempted suicide, restricting access to some methods of suicide and the use of new forms of suicide by individuals and other preventive or risk factor measures. For example, Ilam province had a relatively declining trend in incidence since 2010, which can be attributed to ongoing training of families in identifying signs and symptoms of suicide, life skills training and dealing with problems, holding cheerful meetings for families, and other intervention and preventive measures in this province. However, this province still has the highest rate of suicide leading to death compared to other provinces in the country; that can be attributed to the lack of welfare facilities, the extended families followed by family disputes and divorce, lack of jobs, and lack of factories and industrial opportunities compared to other provinces. Ilam, Kermanshah, Lorestan, Hamedan, and Gilan provinces had the highest incidence (average 8-year incidence per 100,000 people, respectively: 18.6–13.6–12.4–8.9–8.5) of suicide leading to death. The provinces of West Azerbaijan, Ilam, Ardabil, Mazandaran, and North Khorasan also had the highest incidence of suicide attempts that did not result in death. (Average incidence of 8 years per 100,000 people, respectively: 237 − 189–132.5–122.9–120). The highest incidence of suicides leading to death during the eight years of study is related to Ilam province in 2010, which is equal to 24.7 per 100,000 people. The highest incidence rate of unsuccessful suicide attempts during the eight years of study also happened in West Azerbaijan province in 2009, with an incidence of 277 cases per 100,000 people. Conclusion The results of this study indicate the lack of an integrated registration system and monitoring system for suicide attempts in the country. It is recommended that an active surveillance system be implemented in the Ministry of Health for valid registration and monitoring of suicide attempt rates in the country. There is also a need for intervention measures to reduce the incidence of suicide attempts, especially in the age group of 15–49 years in the country in the provinces with high incidence (Ilam - Kermanshah - Lorestan - Gilan - West Azerbaijan, etc.) and then other provinces according to priority. The results of this study demonstrated the need for further investigation to identify high-risk populations and organize intervention programs to control and limit the incidence of suicide attempts. suicide attempt is a conscious and deadly action committed by an individual with the knowledge of its outcome. Suicide is among the most critical health challenges globally, ranking 13th among the causes of death worldwide and third in the age group of 15\u0026ndash;34 years [1]. Around 1,000 suicides happen every day worldwide, and as many as 10 to 20 times, suicide attempts occur. Every year in the United States, 35,000 people die from suicide. Between 2000 and 2007, 256,085 people died from suicide in the United States. Most suicide attempts occur in Asia, which accounts for more than 60% of all deaths worldwide. India, China, and Japan account for 40% of the world's suicides. The suicide rate leading to death in Iran is 6 per 100,000 population annually [2].\u003c/p\u003e \u003cp\u003eIn Islamic countries, suicide is less studied; furthermore, in many Islamic countries, national suicide statistics are not collected or, if collected, are not reported to the World Health Organization. Epidemiological studies in the United States show that about 4.6% of adults attempt suicide, and 14% have severe suicidal ideation [3].\u003c/p\u003e \u003cp\u003eThe suicide attempt is quantitatively and qualitatively different from suicide. But suicide attempt is closely related to successful suicide: for example, 20\u0026ndash;70% of those who have committed suicide have attempted suicide at least once in the past, and 3\u0026ndash;10% of those who attempt suicide eventually die as a result, but suicide attempts are 8\u0026ndash;25 times more common than successful suicide. According to the World Health Organization, a suicide attempt is a non-lethal action in which a person intentionally engages in unusual behavior (such as self-injury or eating a substance over the amount prescribed for treatment) without the intervention of others. According to the investigations, Iran is the third country in the world after China and India, where female suicide attempts have increased. However, the highest rate of suicide attempts is related to the youth age group [6].\u003c/p\u003e \u003cp\u003eIn a cross-sectional study to investigate lifelong suicide attempts, the results showed that 3.2% of people had attempted suicide in their lifetime, of which 63% were women. The most common age was 15\u0026ndash;24 years old, and the most common causes of these actions were marital and family problems. The suicide attempt occurred during the lifetime of 3.9% of Christians and 2.9% Muslims. It was more common in people under permanent stress who drank alcohol, and addicts were at higher risk for a suicide attempt than the general population. This method is used to predict a short time. In the single exponential smoothing method, a weighted moving average is used with weights exponentially geometrically. analysis of 8 years of data on successful and unsuccessful suicide attempts in Iran indicated that during the study period, 30,137 suicides led to death, and 349,342 suicide attempts that did not result in death took place. Although the incidence of suicide attempts in the country has changed significantly during the years under study, the results show that the incidence of suicide leading to death in the last three years of the study was almost constant. Still, compared to the beginning of the study, it enhanced by 17%. In cases of unsuccessful suicide attempts, the highest incidence rate in 2014 was 98.4 per 100000 people, and the lowest incidence rate in 2009 was 74.6 per 100000 people. According to the World Health Organization, about 804,000 fatal suicides and several times this number of suicide attempts occurred worldwide in 2017. Evidence suggests that the incidence of suicide varies among Iran provinces. It is essential to know these differences to allocate resources and adopt the right policies.\u003c/p\u003e\u003cp\u003e\u003cb\u003eMethods\u003c/b\u003e\u003c/p\u003e \u003cp\u003eWe used data recorded by the Ministry of Health and the National Forensic Medicine Organization during the study years to provide information about the rate of suicide attempts in the provinces of Iran. To accurately calculate the under-reporting of suicide attempts according to the frequency of successful suicide, we used TSsmooth methods in STATA 14 software. We ran Exponential, Moving Average, and H Winter models and the best model was chosen according to the lowest root mean square error.\u003c/p\u003e\u003cp\u003e\u003cb\u003eResults\u003c/b\u003e\u003c/p\u003e \u003cp\u003eThere were 30,137 successful suicide attempts and 349342 unsuccessful suicide attempts during the eight-year study period. Around 18,684 suicides resulted in the death of males, and 11,453 were females. Among the unsuccessful suicide attempts, 237,552 cases were males, and 111,790 were females.
