Management of vitamin D deficiency and hypothyroidism in pre-pregnancy preparation of women with sexual dysfunction associated with endometriosis

Abstract

In modern conditions, infertile women have a high percentage of so‑called «diseases of civilization»: autoimmune thyroid disorders, dysmetabolic processes, endometriosis, and female sexual dysfunction. Researchers note the role of vitamin D deficiency or insufficiency in the manifestations of these pathological conditions. The lack of an integrated assessment of health and a clear personalized approach when prescribing treatment gives the pathological process a chronic nature and sometimes leads to a persistent loss of fertility. Objective — to determine the relationship between the features of FSD in women with and without endometriosis and changes in hormonal support of reproductive function during the control periods of therapy. Materials and methods. Study design — prospective cohort study, which included 130 women of reproductive age. Patients were stratified into 3 groups according to diagnosed female sexual dysfunction and external genital endometriosis: Group IA (n=70) — main group — women with a combination of female sexual dysfunction and external genital endometriosis; Group IB (n=30) — comparison group — women with female sexual dysfunction without external genital endometriosis; Group II (n=30) — control group — healthy reproductively active women. Management of low vitamin D levels and thyroid hypofunction was carried out as part of complex therapy for hormonal homeostasis disorders. The study endpoints were: 1. Vitamin D levels, Thyroid peroxidase (TPO) antibody (TPO‑Ab) levels, Thyroid stimulating hormone (TSH) levels, FSD. The indicated indicators were studied before treatment and at the control periods of therapy — 3, 6, 9 months. Biochemical studies were conducted in the clinical laboratory of biochemical, hormonal and immunological studies of the Ukrainian Scientific and Practical Center for Endocrine Surgery, Transplantation of Endocrine Organs and Tissues of the Ministry of Health of Ukraine. Female sexual dysfunction was assessed using a validated questionnaire with scoring conducted via an online calculator. The presence of FSD was determined when the value of the female sexual function index (FSFI) was less than 26.55 points. Statistical analysis was performed using the specialized statistical software package SPSS 25.0 (StatSoft Inc., USA). Results. In women with FSD, regardless of the presence of endometriosis, a statistically significant increase in vitamin D was achieved 3 months after the start of therapy, with positive dynamics maintained until the 9th month, p<0.001. In the comparison groups, changes in the thyroid hormone profile were noted at these times: a statistically significant decrease in the average values of TPO‑Ab after 3 months (p<0.001) and TSH after 6 months from the start of treatment (p<0.001). Regarding TSH values, a statistically significant difference was observed between patients in groups IA and IB at the 6‑month and 9‑month marks, with levels being higher in IA (p=0.006 and p=0.018, respectively). These laboratory parameters were statistically significantly higher in women with FSD compared to the control group throughout the entire observation period, p<0.001. Analysis of the frequency of diagnosing FSD in patients with endometriosis depending on changes in laboratory parameters levels in the control period «6 months» showed that FSD persisted in 97.9% (n=47) and 81.3% (n=39) of patients with reduced vitamin D levels and functionally increased TSH. In most patients of group IA, a clinically significant increase in TPO‑Ab was noted regardless of the presence of FSD. In patients of groups IB 6 months after the start of treatment, the frequency of detecting FSD was statistically independent of the levels of vitamin D, TSH and TPO‑Ab, p>0.05. However, in women who persisted FSD, vitamin D deficiency was significantly more often detected (p<0.05). Using binary logistic regression, it was found that only the maintenance of functionally elevated TSH levels was a predictive factor for the maintenance of FSD in this cohort of patients. Linear regression analysis showed that in patients with endometriosis, only increased vitamin D levels significantly improved scores in all categories of the FSD questionnaire. Conclusions. Combined correction of vitamin D and TSH levels allows to achieve a positive therapeutic effect in women with both endometriosis and monoform. Reducing TSH levels in women with and without endometriosis significantly affects the frequency of detection of VSD against the background of treatment and improves its manifestations in the categories of «Desire», «Lubrication», «Orgasm» and «Pain» in patients without endometriosis. Therefore, the preservation of functionally elevated TSH levels is a prognostic factor for the preservation of FSD in women. Endometriosis worsens the response of the woman’s body to the prescribed treatment, which requires individualized medical tactics at the pre‑gravid stage.

Установлено роль дефіциту чи недостатності вітаміну D у маніфестації цих патологічних станів. Відсутність інтегральної оцінки здоров’я та чіткого персоніфікованого підходу при призначенні лікування надає патологічному процесу хронічного характеру та іноді призводить до стійкої втрати фертильності. Мета роботи — визначити зв’язок між особливостями гормонального забезпечення репродуктивної функції в контрольні терміни терапії в жінок із сексуальною дисфункцією (ЖСД) з ендометріозом і без нього. Матеріали та методи. Проведено проспективне когортне дослідження із залученням 130 жінок репродуктивного віку. Стратифікацію пацієнток на три групи виконали відповідно до діагностованої ЖСД і зовнішнього генітального ендометріозу: група ІА (основна) — 70 жінок із поєднанням ЖСД та зовнішнього генітального ендометріозу, група ІБ (порівняння) — 30 жінок із ЖСД без зовнішнього генітального ендометріозу, група ІІ (контрольна) — 30 здорових, репродуктивно активних жінок. Менеджмент знижених рівнів вітаміну D та гіпофункції ЩЗ здійснювали в складі комплексної терапії порушення гормонального гомеостазу. Кінцевими точками дослідження були рівень вітаміну D, антитіл до тиреоїдної пероксидази (АТ‑ТПО), тиреотропного гормону (ТТГ), ЖСД. Ці показники досліджували до лікування та в контрольні терміни терапії (через 3, 6 та 9 міс). Біохімічні дослідження проведено в клінічній лабораторії біохімічних, гормональних та імунологічних досліджень ДНП «Український науково‑практичний центр ендокринної хірургії, трансплантації ендокринних органів і тканин МОЗ України». Наявність ЖСД визначали за допомогою опитувальника з підрахунком балів на онлайн‑калькуляторі. Наявність ЖСД визначали при величині індексу жіночої статевої функції (FSFI) <26,55 балів. Статистичний аналіз виконували за допомогою спеціалізованого пакета статистичних програм SPSS 25.0 (StatSoft Inc., США). Результати. У жінок із ЖСД незалежно від наявності ендометріозу статистично значущого (p<0,001) підвищення рівня вітаміну D вдалося досягти через 3 міс від початку терапії зі збереженням позитивної динаміки до 9‑го місяця. У групах порівняння в ці терміни зареєстрували зміни гормонального профілю ЩЗ: статистично значуще зменшення середніх значень АТ‑ТПО через 3 міс (p<0,001) і ТТГ через 6 міс (p<0,001) від початку лікування. У пацієнток групи ІА концентрація ТТГ була статистично значущо більшою, ніж у групі ІБ, через 6 міс (р=0,006) і 9 міс (р=0,018). Зазначені лабораторні показники були статистично значущо (p<0,001) вищими в жінок із ЖСД порівняно з контрольною групою впродовж усього періоду спостереження. Аналіз частоти діагностування ЖСД у пацієнток з ендометріозом залежно від зміни рівня лабораторних показників у контрольний термін 6 міс показав, що ЖСД зберігалась у 97,9% (n=47) та 81,3% (n=39) пацієнток зі зниженим вмістом вітаміну D і функціонально підвищеним рівнем ТТГ. У більшості пацієнток групи ІА відзначено клінічно значуще збільшення концентрації АТ‑TПО незалежно від наявності ЖСД. У пацієнток групи ІБ через 6 міс від початку лікування частота виявлення ЖСД статистично значущо не залежала від рівнів вітаміну D, ТТГ і АТ‑TПО (p>0,05), але в жінок, у яких зберігалася ЖСД, статистично значущо частіше виявляли дефіцит вітаміну D (p<0,05). Використання бінарної логістичної регресії дало змогу виявити, що лише збереження функціонально підвищеного рівня ТТГ є прогностичним чинником збереження ЖСД у цієї когорти хворих. Лінійний регресійний аналіз показав, що у пацієнток з ендометріозом лише підвищення рівня вітаміну D статистично значущо впливало на поліпшення оцінки за всіма категоріями опитувальника щодо наявності ЖСД. Висновки. Комбінована корекція рівнів вітаміну D і ТТГ дає змогу досягти позитивного терапевтичного ефекту в жінок як із ендометріозом, так і в моноформі. Зниження концентрації ТТГ у жінок з/без ендометріозу статистично значущо впливає на частоту виявлення ЖСД на тлі лікування та поліпшує її вияви за категоріями «Бажання», «Змащення», «Оргазм» та «Біль» у пацієнток без ендометріозу. Збереження функціонально підвищеного рівня ТТГ є прогностичним чинником збереження ЖСД у жінок. Ендометріоз погіршує відповідь організму жінки на лікування, що потребує індивідуалізованої лікарської тактики на прегравідарному етапі. Посилання Kryzhanovska O.
