This necessitates robust evidence on the comparative safety of different regimens on birth outcomes. There is paucity of evidence on specific cART regimens and adverse birth outcomes in the Nigerian context. Methods A retrospective cohort study was conducted among 420 HIV-positive pregnant women who received antenatal and delivery care at the State Specialist Hospital in Gombe State, Nigeria, between 2018 and 2023. Participants were categorized based on their antenatal cART regimen: first-line (Dolutegravir-based regimen) or second-line (Atazanavir-based regimen). The primary outcomes were fetal death, low birth weight (LBW), preterm birth, and stillbirth. Risk differences were calculated, and multivariate logistic regression was used to identify predictors of adverse outcomes. Results The analysis revealed regimen-specific risks. Exposure to first-line cART was associated with a significantly higher risk of fetal death (Risk Difference [RD] = 0.48, 95% CI: 0.44–0.52) and preterm birth (RD = 0.48, 95% CI: 0.43–0.52). In contrast, exposure to second-line cART was associated with a higher risk of low birth weight (RD = 0.40, 95% CI: 0.39–0.52). The risk of stillbirth was marginally elevated with second-line cART (RD = 0.48, 95% CI: 0.43–0.54). Crucially, an unsuppressed maternal viral load (> 1000 copies/mL) was the strongest independent predictor of adverse outcomes (adjusted Odds Ratio [aOR] = 31.22; 95% CI: 10.29–94.72). Conclusion This real-world study found differential risks of adverse birth outcomes associated with first-line and second-line cART regimens among Nigerian women. However, the most critical modifiable risk factor was unsuppressed viral load during pregnancy. These findings underscore the paramount importance of achieving viral suppression for optimizing birth outcomes and suggest that regimen choice should be carefully considered alongside maternal virological status. HIV Pregnancy Dolutegravir Second-Line ART Nigeria Figures Figure 1 Figure 2 Figure 3 Figure 4 Introduction The advent of combined antiretroviral therapy (cART) has revolutionized the landscape of HIV treatment, markedly enhancing both the longevity and quality of life of individuals affected by the virus [ 1 , 2 ]. As the number of women of childbearing age living with HIV continues to increase, it has become imperative to scrutinize the effects of cART on maternal and fetal health during pregnancy [ 3 , 4 ]. Pregnant women with HIV navigate a complex set of considerations, including the critical need for effective viral suppression to prevent mother-to-child transmission, while simultaneously weighing the potential risks associated with antiretroviral medications [ 5 , 6 ]. The management of HIV infection in pregnant women is a significant public health concern, particularly in sub-Saharan Africa, where the prevalence of the virus is alarmingly high [ 7 ]. Nigeria, one of the largest populations of individuals with HIV, faces an urgent need for effective therapeutic strategies tailored to expectant mothers [ 8 , 9 ]. In this context, combined antiretroviral therapy (cART) has emerged as a transformative intervention that plays a crucial role in reducing the rate of mother-to-child transmission (MTCT) of HIV. Clinical studies have clearly demonstrated that when pregnant women adhere to cART, the risk of transmission of the virus to their infants can decrease to less than 1% [ 10 , 11 ]. Despite the well-established benefits of cART in preventing MTCT, the implications of different antiretroviral medications on pregnancy outcomes have not yet been thoroughly investigated. Women living with HIV experience unique physiological changes during pregnancy that can affect the metabolism and efficacy of antiretroviral drugs [ 12 , 13 ]. This raises critical questions regarding the potential risks associated with specific cART regimens, including their associations with adverse pregnancy outcomes, such as preterm labor, intrauterine growth restriction, and low birth weight [ 14 , 15 ]. Research findings have begun to elucidate how specific antiretroviral regimens might differentially impact these outcomes, particularly in the context of newer drug classes and treatment protocols continually evolving to enhance effectiveness and minimize side effects. Nonetheless, significant uncertainties remain regarding the long-term implications of cART during pregnancy, especially as treatment guidelines are adapted to incorporate advancements in pharmacological strategies. Other studies have assessed the pregnancy outcomes of neonates exposed to antiretroviral drugs. However, these studies were limited by their relatively small sample sizes and did not specifically focus on HIV-positive mothers [ 16 , 17 ]. Similarly, other studies did not compare the incidence of adverse birth outcomes across the combined antiretroviral medicines [ 18 ] but assessed the incidence of adverse maternal and fetal outcomes among Dolutegravir-based users [ 19 ]. Moreover, there is insufficient data on the incidence of adverse birth outcomes among neonates exposed to antiretroviral agents in Nigeria, making this study essential for understanding this phenomenon. Given these complexities, a retrospective cohort study offers a valuable lens through which to analyze the real-world implications of cART on pregnancy outcomes among Nigerian women. The primary objective of this study was to investigate the impact of various combined antiretroviral medications on pregnancy outcomes in HIV-positive women. METHOD Study Design A retrospective cohort study was employed in the study Study Area The research was conducted at the State Specialists’ Hospital Gombe (SSHG). The hospital has a total 340 beds, is the largest comprehensive health center owned by the state government, and is involved in the management of HIV. The state of Gombe is located in Northeast Nigeria, with a population of approximately 2,365,000 people. Study Population The study population included pregnant women with HIV who attended a clinic at the State Specialist Hospital Gombe between 2018 and 2023. Eligibilty Criteria Pregnant women with HIV Receiving PMTCT care at state specialist hospital Gombe (SSHG) Undergone delivery at SSHG Having a documented viral load result Ethical Considerations A written application for ethical clearance was sought from the Ethics and Research Committee of the Gombe State Ministry of Health (Ref; MOH/ADM/621/V.1/474). Sample Size Determination The sample was determined using an online (OpenEpi) software sample size calculator [20]. Using a power of 80%, margin of error of 5%, ratio of sample (exposed/unexposed) of 1.0, odds ratio of 2.8, and a confidence interval of 95%, the calculated sample size was 414. Therefore, the minimum sample size for this work is 414. Sampling Technique A systematic random sampling was employed to identify relevant patient records for the possible inclusion into the study while the sample interval was obtained by dividing the sample by the average patient seen in the prevention of mother-to-child transmission of HIV/AIDS (PMTCT) clinic in a week. Average number of patients seen in a week=95 Sample interval=sample size÷ average number of patients seen in a week Sample interval= 414/95 Approximately = 4 Research Instrument A proforma for this study was designed after an evaluation of relevant literature [21–24]. The first section obtained basic sociodemographic data of the population, the second section assessed the virological characteristics of the study population, and the third section assessed adverse fetal outcomes. Data Management Viral suppression was defined as a viral load greater than 1000 copies /ml of blood [25], while adverse birth outcomes were classified as premature birth, low birth weight, and stillbirth [21, 26]. The cascade of HIV treatment is classified as first-line (tenofovir disoproxil/lamivudine/dolutegravir) or second-line (abacavir/lamivudine/Atazanavir-ritonavir; zidovudine/lamivudine/Atazanavir-ritonavir) according to Nigerian HIV/AIDS care and treatment guidelines [27]. Data Analysis The data were analyzed using Statistical Products and Service Solutions, version 25.0 (SPSS Inc., Chicago, IL, USA) and NCSS (Version 2021). Categorical data were expressed as percentages of frequency, while univariate and multivariate logistic regression analyses were applied to identify predictors, if any, that may explain adverse fetal outcomes with maternal antiretroviral drugs. Risk difference was used to compare the incidence of adverse birth outcomes. Forest plots using risk difference was used to determine risk of adverse birth outcome across antiretroviral line of therapy. A p-value of < 0.05 was considered significant at a 95% confidence level. RESULTS Socio-demographic Data The study indicated that the majority of the population was aged between 25 and 35 years (n=289, 68.8%), had initiated antiretroviral therapy (ART) before the age of 25 years (n= 400, 95.5%), had more than three pregnancies (n=248, 59.0%), and had fewer than three living children (n=227, 54.0%). Additionally, nearly all participants were housewives (n=419, 99.8%) and over half of the population had attained secondary education and resided in urban areas (n=240, 57.1%). Only 9 women (2.1%) were diagnosed with HIV during their pregnancy. The majority of the women followed a TDF/3TC/DTG regimen (N = 266, 63.3%) and had no comorbidities (n=419, 99.8%). More than half of the study population belonged to the WHO Class 1 category (n=225, 53.3%). Additionally, 345 women (82.1%) initiated antiretroviral therapy (ART) before conception and 408 women (97.1%) began prenatal visits during the first trimester. In comparison, the risk associated with exposure to second-line antiretroviral drugs is 0.47 (95% CI, 0.43 - 0.52). Figure 2 shows the effect of maternal exposure to antiretroviral drugs on the incidence of low birth weight (LBW) in neonates. The data reveal an increased risk of LBW associated with exposure to second-line antiretroviral medications, with a risk difference of 0.3981 (95% CI 0.39 - 0.52), compared to exposure to first-line antiretroviral drugs 0.44 (95% CI 0.4031 - 0.4957). As depicted in Figure 3, neonates exposed to first-line maternal antiretroviral drugs exhibit a markedly elevated risk of preterm birth, with a risk difference of 0.48 (95% CI, 0.43 - 0.52). In contrast, the risk associated with exposure to second-line antiretroviral drugs is 0.46 (95% CI, 0.41 - 0.52). Figure 4 depicts the incidence of stillbirth among neonates exposed to antiretroviral drugs in utero. The risk of stillbirth was marginally elevated with exposure to second-line antiretroviral drugs (risk difference 0.484, 95% CI, 0.43 - 0.54), whereas neonates exposed to first-line antiretroviral drugs exhibited a reduced risk of this outcome (risk difference 0.483, 95% CI, 0.44 – 0.53). Factors Associated with Adverse Birth Outcomes The factors contributing to adverse birth outcomes are presented in Table 4. Statistical analysis indicated that the number of pregnancies (p = 0.02), delivery mode (p = 0.03), and virological status during pregnancy (p < 0.005) were significantly associated with adverse birth outcomes. Infants born to mothers with unsuppressed viral loads are at a significantly higher risk of adverse birth outcomes. In this cohort, maternal use of first-line antiretroviral therapy was associated with a higher risk of fetal death compared to second-line therapy. However, studies focusing on the WHO-recommended first-line regimen—tenofovir, emtricitabine, and efavirenz—found no significant increase in the risk of neonatal death compared to HIV-negative women or those on other regimens [ 28 – 30 ]. Nonetheless, among various ART regimens, tenofovir-emtricitabine-efavirenz was associated with a lower risk of adverse birth outcomes than other regimens first line treatments, such as those containing zidovudine or lopinavir-ritonavir, which were linked to higher risks of stillbirth, preterm birth, and neonatal death [ 28 ]. A study found no statistically significant differences in adverse fetal and infant outcomes between NNRTI-based and protease inhibitor (PI)-based regimens in 384 Kenyan women [ 31 ]. The mechanisms underlying these observed differences are not fully understood but may involve drug-specific toxicity, placental transport, or maternal immunological factors. For example, NNRTIs and some NRTIs have been associated with mitochondrial toxicity and potential effects on fetal development, while PIs may have different pharmacokinetic and safety profiles during pregnancy [ 32 , 33 ]. Our study revealed Infants born to women on second-line antiretroviral therapy had a higher risk of low birth weight than those exposed to first-line regimens. Research on second line antiretroviral therapy during pregnancy and birth outcomes shows mixed findings regarding low birth weight (LBW) risk. A related study reported no significantly increased LBW frequency among HIV-positive women, particularly those receiving protease inhibitors (PIs), though this association may relate to other obstetrical risk factors [ 34 ]. Similarly, a large meta-analysis among 45,427 pregnant women and found PI-based ART significantly associated with increased risk of very small for gestational age (VSGA) infants compared to NNRTI-based regimens, but no significant associations for other adverse outcomes including preterm birth and low birth weight [ 35 ]. Another meta-analysis found showing second-line ART particularly PI-based ART to be associated with increased risks of small for gestational age and VSGA compared to NNRTI-based regimens, with lopinavir/ritonavir specifically linked to higher SGA and VSGA risks compared to efavirenz-based regimens [ 36 ]. It is important to consider that women on second-line regimens may have additional risk factors such as advanced HIV disease, prior ART exposure, or coexisting infections, which could also contribute to adverse birth outcomes. Therefore, while the association between second-line ART and low birth weight is supported by a growing body of evidence, further research is needed to disentangle the effects of the medications themselves from those of underlying maternal health conditions. This work found neonates exposed to first-line maternal antiretroviral drugs exhibit a markedly elevated risk of preterm birth. A cohort study corroborates this finding in which HIV-infected women on WHO-recommended first-line ART regimens have higher rates of preterm delivery compared to uninfected women, but timing of ART initiation does not affect birth outcomes [ 37 ]. This finding is inconsistent with the reports from a randomized controlled trial and a cohort study, in which neonates exposed in utero to second line agents such a the PI-based regimes having a higher risk for preterm delivery [ 38 , 39 ]. Similarly, a large cohort study found neonates exposed to PI-based regimens had a much higher risk of preterm birth compared to their unexposed counterparts [ 40 ]. Another related study found a higher risk of preterm delivery by Women receiving zidovudine and tenofovir-based ART by ultrasound compared to zidovudine alone [ 41 ]. The biological mechanisms underlying this elevated risk are not fully understood but may involve immune activation, placental inflammation, or direct drug effects on placental development and function. Findings from this study also indicates the risk of stillbirth was marginally elevated with exposure to second-line antiretroviral drugs. A large comparative study found The risk of stillbirth to be marginally elevated with exposure to second-line antiretroviral drugs compared to the TDF-FTC-EFV regimen [ 28 ]. Other studies found inconsistent results- Cotter et al. (2006) found no differences in stillbirth rates regardless of therapy type [ 39 ]. Favarato et al. (2019) observed that conceiving on ART did not increase stillbirth risk, though women with HIV had higher overall stillbirth rates than the general population [ 42 ]. However, it is important to emphasize that while the risk appears marginally increased, the absolute risk may remain relatively low, and the benefits of effective ART in reducing maternal morbidity and the risk of mother-to-child HIV transmission are substantial. The findings highlight the need for close monitoring of pregnant women on second-line ART, careful selection of drug regimens, and further research to optimize maternal and fetal outcomes. Maternal virological status was a significant predictor of adverse birth outcomes. Infants born to mothers with unsuppressed viral loads are at a significantly higher risk of adverse birth outcomes. A Malawian study corroborates with this outcome in which HIV-positive mothers had increased risks of fetal/perinatal death, low birth weight, and preterm birth, with the strongest associations observed among women with detectable viral loads throughout pregnancy [ 43 ]. While another related study concluded that maintaining HIV viral load suppression between pregnancies is associated with improved maternal and infant outcomes in subsequent pregnancies [ 44 ]. A meta-analytic estimate found a high risk of adverse birth outcomes with HIV infection [ 45 ]. Unsuppressed maternal viral load reflects ongoing HIV replication, which can lead to heightened immune activation, systemic inflammation, and adverse effects on placental health. These factors are believed to contribute to placental insufficiency, impaired fetal growth, and preterm labor [ 46 ]. This study has several limitations inherent to its retrospective cohort design. First, data quality and completeness were dependent on clinical records at the time, which may have led to missing data on potential confounders like alcohol use. Furthermore, as our study was conducted within public health clinics, the findings may not be generalizable to populations outside of similar healthcare settings. Despite these limitations, the use of routinely collected data allowed for a large sample size and the examination of real-world effectiveness of these antiretroviral regimens. Conclusion The findings of this retrospective cohort study illuminate the complex relationship between antiretroviral therapy and birth outcomes among HIV-positive women in Nigeria. The analysis revealed a nuanced profile of risk, with first-line dolutegravir-based regimens associated with a significantly higher risk of fetal death and preterm birth, while second-line protease inhibitor-based regimens were associated with a greater risk of low birth weight. The substantial benefits of ART in preventing mother-to-child transmission of HIV continue to far outweigh the identified risks, but this research highlights the need for vigilant monitoring and support for pregnant women to achieve undetectable viral loads. This necessitates robust evidence on the comparative safety of different regimens on birth outcomes. There is paucity of evidence on specific cART regimens and adverse birth outcomes in the Nigerian context.\u003c/p\u003e\u003cp\u003e\u003cb\u003eMethods\u003c/b\u003e\u003c/p\u003e\u003cp\u003eA retrospective cohort study was conducted among 420 HIV-positive pregnant women who received antenatal and delivery care at the State Specialist Hospital in Gombe State, Nigeria, between 2018 and 2023. Participants were categorized based on their antenatal cART regimen: first-line (Dolutegravir-based regimen) or second-line (Atazanavir-based regimen). The primary outcomes were fetal death, low birth weight (LBW), preterm birth, and stillbirth. Risk differences were calculated, and multivariate logistic regression was used to identify predictors of adverse outcomes.\u003c/p\u003e\u003cp\u003e\u003cb\u003eResults\u003c/b\u003e\u003c/p\u003e\u003cp\u003eThe analysis revealed regimen-specific risks. Exposure to first-line cART was associated with a significantly higher risk of fetal death (Risk Difference [RD]\u0026thinsp;=\u0026thinsp;0.48, 95% CI: 0.44\u0026ndash;0.52) and preterm birth (RD\u0026thinsp;=\u0026thinsp;0.48, 95% CI: 0.43\u0026ndash;0.52). In contrast, exposure to second-line cART was associated with a higher risk of low birth weight (RD\u0026thinsp;=\u0026thinsp;0.40, 95% CI: 0.39\u0026ndash;0.52). The risk of stillbirth was marginally elevated with second-line cART (RD\u0026thinsp;=\u0026thinsp;0.48, 95% CI: 0.43\u0026ndash;0.54). Crucially, an unsuppressed maternal viral load (\u0026gt;\u0026thinsp;1000 copies/mL) was the strongest independent predictor of adverse outcomes (adjusted Odds Ratio [aOR]\u0026thinsp;=\u0026thinsp;31.22; 95% CI: 10.29\u0026ndash;94.72).\u003c/p\u003e\u003cp\u003e\u003cb\u003eConclusion\u003c/b\u003e\u003c/p\u003e\u003cp\u003eThis real-world study found differential risks of adverse birth outcomes associated with first-line and second-line cART regimens among Nigerian women. However, the most critical modifiable risk factor was unsuppressed viral load during pregnancy.
