We conducted a systematic follow-up of children with mBWT from a previous retrospective study. The imaging criteria for diagnosing mBWT is the presence of more than two nodules with a diameter of >1cm in one or both kidneys. During the follow-up period, we paid particular attention to whether the patients developed end-stage renal disease (ESRD). A total of 56 patients with mBWT were included in this study. Ten patients (17.8%) underwent one-step bilateral surgery. Twelve children underwent total nephrectomy plus nephron-sparing surgery (TN + NSS), and 44 children underwent bilateral nephron-sparing surgery (NSS). After a median follow-up of 6 years (range 0-15 years), the 5-year OS and EFS rates were 86.8% and 77.1%, respectively. The patients who underwent TN + NSS and those who underwent bilateral NSS showed no statistically significant differences in EFS and OS. During the follow-up, 17 patients experienced impaired renal function or a progressive deterioration. The multivariate analysis results showed that one-stage bilateral surgery was an important factor for long-term renal impairment. In patients with mBWT, NSS was appropriate after 6 weeks of neoadjuvant chemotherapy (NAC). One-step bilateral NSS was a risk factor for long-term renal function impairment in patients with mBWT.
