Abstract

目的:他科精査で原因が特定できなかった慢性骨盤痛に加え,CTで卵巣静脈の拡張や骨盤内静脈瘤を認め,骨盤内鬱滞症候群(pelvic congestion syndrome;PCS)が疑われた症例について,静脈造影による診断および血管内治療効果を検討した.対象と方法:2014年1月~2016年12月の間に他科にて原因が特定できず当科を受診した慢性骨盤痛に加え,CTで卵巣静脈の拡張や骨盤内静脈瘤を認めた12例を対象に,両側卵巣静脈・両側内腸骨静脈を選択的に造影.PCSの診断基準を満たす症例には責任静脈本幹の血管内治療を施行した.結果:静脈造影所見から10例をPCSと診断し,血管内治療(コイル塞栓術)を施行.全例で術前症状が消失し再増悪を認めなかった.結論:慢性骨盤痛に加え,CTで卵巣静脈の拡張や骨盤内静脈瘤を認める場合はPCSを疑い,静脈造影による診断および血管内治療を積極的に行うべきである.
