Effect of a Multimodal Supervised Therapeutic Exercise Program on Quality of Life, Pain, and Lumbopelvic Impairments in Women With Endometriosis Unresponsive to Conventional Therapy: A Randomized Controlled Trial

This parallel-group randomized controlled trial evaluated whether a nine-week multimodal supervised therapeutic exercise program (“Physio-EndEA”) improves quality of life, pain outcomes, and lumbopelvic impairments in 31 women with endometriosis unresponsive to conventional therapy, recruited from two public university hospitals and measured at baseline, post-intervention, and at one year. Women were randomized to Physio-EndEA (a one-week lumbopelvic stabilization learning phase followed by eight weeks of stretching, aerobic, and resistance exercises tailored and supervised by a physiotherapist with progression) versus a control group receiving usual gynecologist treatment; four participants dropped out for reasons unrelated to the intervention. Compared with controls, the exercise group showed improved quality of life with large effect sizes and reductions in dyspareunia intensity and pain-catastrophic thoughts, along with higher pressure pain thresholds, improved abdominal/back strength and lumbopelvic stability, and changes in transversus abdominis and lumbar multifidus muscle architecture. The main caveat explicitly noted is the small sample size (31 total with attrition), which may limit generalizability.
