Abstract

症例は42歳，女性．40歳より腸管子宮内膜症の疑いにて内分泌治療が施行されており，また複数回の腸閉塞に対していずれも保存的加療を行われ軽快していたが，精査にて病変部位は同定されなかった．しかし，経過中に穿孔性腹膜炎を発症し緊急手術となり，切除標本から病理組織学的に小腸子宮内膜症穿孔と確定診断された．子宮内腔以外に子宮内膜や類似組織の異所性増殖をみる子宮内膜症は，全消化管にも発症しうる．しかし腸管子宮内膜症の多くは直腸・S状結腸に発症し，小腸発症例の頻度は低い．また小腸子宮内膜症の臨床症状は腸閉塞や下血であり，穿孔に至る症例は稀有である．今回この1例を経験し，文献的考察を加え報告する．
