This article reports a 39-year-old male ESRD patient with severe peritonitis, thoracic deformity, vascular calcification, and a history of multiple vascular access failures. Ultimately, a cuffed dialysis catheter was successfully placed via the left innominate vein under digital subtraction angiography (DSA) guidance. This case highlights the challenges of dialysis access reconstruction in the context of complex anatomy, severe infection, and coagulopathy, emphasizing the critical role of multidisciplinary collaboration and image-guided intervention.
