Early identification of reversible frailty factors in elderly AIDS patients and active intervention can delay the frailty and disability of elderly AIDS patients. This study investigated the status and influencing factors of frailty in 219 elderly AIDS hospitalized patients, of whom 161 (73.52%) developed frailty. The results of univariate analysis showed that nutritional status, cognitive status, anxiety, and depression were all related to frailty in elderly AIDS patients, and the differences were statistically significant ( P < 0.05). Binary Logistic regression analysis showed that elderly AIDS patients with malnutrition ( OR = 0.172, 95% CI : 0.054–0.550, P = 0.003) and moderate anxiety ( OR = 6.540, 95% CI : 1.776–24.079, P = 0.005) were more likely to develop frailty. The survey results show that the prevalence of frailty is high among elderly AIDS hospitalized patients in China. We should pay attention to the current situation of frailty among AIDS hospitalized patients and take effective intervention measures as early as possible to delay the occurrence of frailty. elderly AIDS patients frailty influencing factors Introduction UNAIDS estimates that the number of people aged 50 years or older with HIV infection globally increased from 5·4 million in 2015 to 8·1 million in 2020. In China, the number of individuals aged 50 and above living with HIV has increased from 22% in 2011 to 44% in 2022[1] . Furthermore, the elderly population has become the demographic group with the highest annual growth rate of reported HIV infections in China. The combined effects of ageing and HIV infection result in elderly AIDS patients facing a greater number of complex health challenges, with frailty being a particularly prominent issue. The prevalence of frailty among elderly individuals living with HIV has been documented to range from 6.3% to 22.6% [2-5] . Frailty not only increases the risk of falls[6] , readmissions[7] and mortality[8] in elderly patients, but also increases the consumption of medical resources and the burden on families[9] . In recent years, there has been an increase in studies of frailty in elderly HIV-infected patients, both nationally and internationally, but there has been insufficient research into its determinants, particularly in elderly HIV-infected patients in hospitals. Studies have shown that hospitalisation, as a source of stress, can lead to further deterioration and loss of independence in older people, especially those who are already frail[10] . In addition, AIDS is a chronic infectious disease. As the disease progresses and worsens, hospitalised patients often experience a state of chronic fatigue, with reduced nutrient intake and utilisation, and often suffer from malnutrition, which in turn affects their health[11] . In addition, during hospitalisation, older people with AIDS often experience psychological problems such as depression and anxiety due to financial burdens, the stigma associated with HIV infection, feelings of isolation and concerns about disease progression[12] , which can seriously affect the quality of life of older people and, to some extent, increase the incidence of frailty. Therefore, it is necessary to understand the incidence and determinants of frailty in elderly patients with AIDS in order to provide a reference for establishing an early warning mechanism for frailty in elderly patients and for implementing frailty interventions. Although there is a growing global awareness of frailty in older people living with HIV/AIDS, there is limited research in China on the specific factors that contribute to frailty in older people living with HIV who are hospitalised. Studies in Western countries have shown that HIV-specific factors (e.g. duration of infection, viral load) and non-HIV-related factors (e.g. diet, physical activity level) may influence frailty in older people living with HIV [13, 14] . However, due to different social, cultural and health care contexts, the results of these studies may not be fully applicable to older people living with HIV in China. Methods 1. 1 Study participants A total of 219 elderly AIDS patients treated in an infectious disease hospital in Chengdu were selected by convenience sampling from March 2022 to August 2022. Inclusion criteria: (1) age ≥ 50 years old[ 15 ] ; (2) patients diagnosed with AIDS; (3) informed consent and voluntary participation in this researcher. Exclusion criteria: (1) patients with mental illness or dementia; (2) patients with severe heart, brain, kidney and other organ dysfunction or in the acute attack of the disease; (3) language communication disorder or hearing impairment. Remove the shedding criteria: those who fail to complete the questionnaire as required. 1.2 Sample size The sample size of elderly AIDS patients was estimated according to the hypothesis test of the sample size required by the overall rate. The calculation formula is as follows: after consulting the literature, it is known that the incidence of frailty in AIDS patients is about 45%[ 16 ], δ = 0.1,0.45, α = 0.05, Z α = 1.96, 95 cases are substituted into the formula, and the lost follow-up rate is increased by 20%. General information questionnaire On the basis of consulting the relevant literature, the general data questionnaire was designed by the research team, which included gender, age, education, marriage, occupation, personal income, health insurance, time of diagnosis of HIV, antiviral therapy, drinking, smoking and the number of CD4 + cells. Vulnerable Elders Survey The Vulnerable Elders Survey was used to assess patients' frailty, including age, self-rated health, activity status, and functional status, with a total of 13 items[ 17 ]. Among them, age: <74 is scored as 0 points; 75–84 is scored as 1 point; ≥85 is scored as 3 points. For self-rated health, if "poor" or "average" is selected, it is scored as 1 point; if "good", "very good" or "excellent" is selected, it is scored as 0 points. For physical function, if the three items "no difficulty, some difficulty, some difficulty" are selected, it is scored as 0 points; "great difficulty" or "unable to do" is scored as 1 point, and the total score is 2 points. If one or more physical functions are difficult or impossible to perform, 4 points are awarded. The total score ranges from 0 to 10 points, and the score ≥ 3 points is considered frail, and 0 to 2 points is considered non-frail. Hospital Anxiety And Depression Scale The Hospital Anxiety And Depression Scale is used to assess anxiety and depression in patients[ 18 ]. HADS is a self-rating scale with two subscales, HADS-A and HADS-D, with a total of 14 items, of which 7 items (1, 3, 5, 7, 9, 11, 13) assess anxiety and 7 items (2, 4, 6, 8, 10, 12, 14) assess depression. The 3-point Likert scale is used. The higher the total score on the scale, the higher the level of anxiety and depression. The scores are divided into: 0–7 points for no symptoms; 8–10 for mild; 11–14 for moderate; 15–21 for severe. Mini Nutritional Assessment The Mini-Nutritional Assessment is used to assess the nutritional status of elderly patients[ 19 ]. The scale consists of 18 indicators in 4 parts with a total score of 30 points. A score of ≥ 24 points indicates normal nutrition, a score of 17–24 points indicates potential malnutrition, and a score of < 17 points indicates malnutrition. The higher the score, the better the nutritional status. Rapid Cognitive Screen Rapid Cognitive Screen is used to assess the cognitive function of elderly patients[ 20 ]. RCS consists of 3 items: delayed recall of 5 words, clock drawing test, and story recall. In the delayed recall of words, one word is scored for each correct answer; number marking and hand marking in the clock drawing test are scored as 2 points each; the ability to remember stories is scored as 1 point. The RCS score range is 0–10 points, 0–5 points for dementia, 6–7 points for MCI, and 8–10 points for normal. Data collection Unify the training of investigators before the survey to clarify the research background, survey content and questionnaire filling standards. Qualified investigators and researchers themselves use unified guidelines to conduct a face-to-face survey of the subjects who meet the inclusion exclusion criteria, and the questionnaire is collected and checked on the spot to ensure the integrity of the data. Statistical analysis The database was established by Excel software, and the data was processed by SPSS26.0 statistical software. The statistical description of normal distribution data is based on the mean ± standard deviation, and the counting data is expressed by the number of cases (constituent ratio). Chi-square test was used in univariate analysis, nonparametric test was used in rank data and non-normal distribution data, and logistic regression analysis was used in multivariate analysis. Results Characteristics of participants There were 219 elderly AIDS patients in this study, aged 50–82 years (62.21 ± 8.244), approximately 79% (n = 173) of the participants were male, 72% (n = 156) were married, 56% (n = 123) had a personal monthly income of ≤ 3,000 dollars, 76% (n = 167) were on antiretroviral therapy and 56% (n = 124) of participants had a CD4 + cell count ≤ 200. see Table 1 for details. Univariate analysis was used to test the difference between frail and non-frail patients. The results showed that nutritional status, cognitive status, anxiety, and depression were associated with frailty in elderly AIDS patients, and the differences were statistically significant (P < 0.05). See Table 1 for details. Regression Analysis The factors with statistical significance (P < 0.05) in single-factor analysis were analyzed by multiple factors. Binary logistic regression model showed that malnutrition and moderate anxiety were independent factors influencing frailty. That is, elderly AIDS patients with malnutrition (OR = 0.172,95%CI:0.054–0.550,P = 0.003) and moderate anxiety (OR = 6.540,95%CI:1.776–24.079,P = 0.005) were more likely to develop frailty.. Discussion The results of this study showed that 161 elderly patients with AIDS developed frailty with an incidence of 73.52%, while the incidence of frailty in elderly HIV-infected patients in the outpatient setting ranged from 6.1–20.47%. First, as AIDS progresses, neutrophils become more active, which in turn activates the body's T cells, which can lead to problems associated with HIV infection such as inflammation and rapid aging[ 21 ]. Second, In some hospitalised older people with HIV, prolonged bed rest and limited physical activity impair physical function and nutritional status, [ 22 ] resulting in reduced muscle protein synthesis, increased muscle catabolism, muscle wasting and an increased risk of muscle frailty[ 23 , 24 ]. In addition, the hospital environment can limit the social activities of older people with AIDS, increase their psychological distress, and lead to emotional problems and cognitive decline, all of which are risk factors for frailty[ 25 , 26 ]. Therefore, medical personnel should pay attention to the frailty of elderly HIV/AIDS patients and inform them about the importance of adherence to antiretroviral therapy, regular exercise, and nutrition to strengthen the body's immunity, prevent and control frailty in elderly HIV/AIDS patients, and improve their quality of life. Nutrition is considered to be one of the most important pathophysiological factors in frailty syndrome[ 27 ] .. The results of this study show that malnutrition is one of the factors leading to frailty. Studies have shown that throughout the course of HIV infection, the patient's nutritional status is closely related to their immune system[ 28 – 30 ]. Elderly AIDS patients are prone to malnutrition due to immune function damage[ 31 ], and malnutrition in turn accelerates the loss of muscle mass and muscle strength[ 32 ], weakens the body's immune function, and leads to rapid progression of AIDS, thus affecting the occurrence of frailty in elderly AIDS patients[ 33 ]. Nutrition is a modifiable risk factor for frailty. It can be improved through intervention[ 34 ]. Nurses should assess the nutritional status of elderly patients with AIDS, find the causes of malnutrition in elderly patients with AIDS, educate them on nutrition and healthy eating, and for patients with malnutrition, formulate an individualized nutrition support plan for elderly patients with AIDS through a multidisciplinary team to delay the onset of frailty. The results of this study show that anxiety in older people with AIDS is associated with frailty. The elderly people with AIDS in this study were mostly in a state of mild to moderate anxiety. Elderly AIDS patients often experience anxiety due to a lack of correct understanding of the disease, social discrimination, side effects of treatment and opportunistic infections caused by the virus[ 35 ]. Studies have shown that long-term exposure to anxiety-inducing stressors can overstimulate and disrupt the neuroimmune axis, leading to neuroendocrine and immune dysfunction, increased vulnerability, and decreased stress resistance, thereby increasing the risk of frailty in older people with AIDS[ 36 , 37 ]. It is suggested that health professionals should pay attention not only to the physical health of older people with AIDS, but also to their mental health, and promptly assess and intervene in those at high risk of anxiety. Conclusion This study found that the incidence of frailty in older people with AIDS was high, and that the main influencing factors were nutritional status, cognitive status, anxiety and depression. Healthcare professionals should pay close attention to the frailty status of older people with AIDS and carry out regular comprehensive assessments and preventive interventions, such as changing poor lifestyle habits, personalised nutritional interventions, multi-exercise, cognitive training, and identifying and intervening in psychological problems. The subjects in this study came from only one top-level hospital in Chengdu, so there are certain limitations in the selection of the sample. In the future, longitudinal studies with a large sample size and multiple centres can be used to explore the predictors of frailty in elderly inpatients with AIDS, so as to delay the progression of frailty in elderly patients with AIDS and improve their quality of life and health status. Early identification of reversible frailty factors in elderly AIDS patients and active intervention can delay the frailty and disability of elderly AIDS patients. This study investigated the status and influencing factors of frailty in 219 elderly AIDS hospitalized patients, of whom 161 (73.52%) developed frailty. The results of univariate analysis showed that nutritional status, cognitive status, anxiety, and depression were all related to frailty in elderly AIDS patients, and the differences were statistically significant (\u003cem\u003eP\u003c/em\u003e\u0026thinsp;\u0026lt;\u0026thinsp;0.05). The survey results show that the prevalence of frailty is high among elderly AIDS hospitalized patients in China.
