This prospective study evaluated retinoic acid receptor alpha (RARα) expression in endometrial tissue and endometrioid heterotopias from patients with endometriosis (n=28) compared with a control group (n=10), using the Revised American Society for Reproductive Medicine classification by disease stage. The authors found RARα expression in endometrioid heterotopias was lower than in eutopic endometrium from patients with endometriosis and lower than in control endometrium, by 3.4-fold and two-fold respectively (p<0.001). They interpret this reduced receptor expression as consistent with impaired vitamin A binding to the receptor and improper retinoic acid metabolism, while noting that their prior work reported increased retinol levels in peripheral blood and peritoneal fluid in endometriosis.
