Comparing the Administration of Letrozole and Megestrol Acetate in the Treatment of Women with Simple Endometrial Hyperplasia without Atypia: A Randomized Clinical Trial

This pilot randomized clinical trial compared oral letrozole versus megestrol acetate in 50 perimenopausal women aged 44–50 with abnormal uterine bleeding and biopsy-confirmed simple endometrial hyperplasia without cytologic atypia, using transvaginal ultrasound plus histologic assessment. Participants received either megestrol acetate 40 mg for 2 weeks per month over 2 months or daily letrozole 2.5 mg for 2 months, and outcomes were analyzed with t tests and chi-square/Fisher’s exact tests (P<0.05). Both treatments led to significant improvements, but resolution rates did not differ between groups (P=0.74), while side effects were significantly less frequent with letrozole (2 vs 9; P=0.02). As a pilot with a small sample and short 2-month treatment window, the study’s limitation is the limited ability to assess longer-term outcomes.
