This paper reports a case of a 21-year-old woman with low abdominal discomfort, dysmenorrhoea, and a pelvic mass, whose symptoms and history led to an initial misdiagnosis of endometriosis. Using laparotomy, the authors identified pelvic splenosis—peritoneal splenic implants following prior traumatic splenic rupture and surgery. The pelvic mass was removed, while other implants were left in place. This paper is centrally about endometriosis — it describes splenosis that mimicked endometriosis, including misdiagnosis and symptom overlap (dysmenorrhoea and pelvic mass).
