Abdominal wall endometriosis, presenting as a painful mass in reproductive-aged women, is confirmed by histopathology showing endometrial glands and stroma in muscle, regardless of pelvic organ involvement.

This case report describes a 35-year-old woman with a history of hysterectomy who presented with a painful anterior abdominal wall mass and gastrointestinal symptoms. Ultrasound imaging identified a hypoechoic lesion, which was surgically excised and subsequently confirmed via histopathology to contain endometrial glands and stroma infiltrating skeletal muscle fibers. Crucially, the ovarian tissue examined during surgery showed no evidence of endometriosis, establishing the diagnosis as isolated extra-pelvic disease rather than a manifestation of pelvic involvement. This paper is centrally about endometriosis — specifically the rare presentation of isolated abdominal wall endometriosis in a patient without concurrent pelvic or ovarian disease.

Histopathology confirming endometrial glands and stroma within muscle is diagnostic, even when pelvic organs are spared. Clinical Image A 35-year-old woman presented with a gradually enlarging, painful swelling over the anterior abdominal wall, associated with nausea and vomiting. She had a history of hysterectomy 5 years ago. On examination a firm localized mass was palpated in the right anterior abdominal wall, just below the umbilicus. Ultrasound revealed a well-defined hypoechoic lesion beneath the umbilicus, measuring approximately 26 × 14 mm. Surgical excision revealed an abdominal wall mass ( Figure 1A), while the ovaries appeared grossly unremarkable (Figure 1C). Histopathology of the excised mass demonstrated endometrial glands and stroma infiltrating skeletal muscle fibers, confirming endometriosis (Figure 1B). In contrast, ovarian histology showed normal cystic follicles and stroma without endometriotic deposits (Figure 1D). Abdominal wall endometriosis is a rare condition and may clinically mimic abscess, suture granuloma, hematoma, desmoid tumor, sarcoma and metastatic malignancy. While imaging such as ultrasound or MRI is helpful, histopathology remains the gold standard. Importantly, isolated abdominal wall disease without pelvic or ovarian involvement, as in this case, highlights the need for a high index of suspicion in women with a history of prior obstetric or gynecological surgery. Surgical excision with clear margins remains the treatment of choice, offering both definitive diagnosis and symptomatic relief. [1-3].
