VP63.04: Non‐expert interobserver agreement for sonographic diagnosis of adenomyosis

Abstract
To assess non-expert interobserver agreement for sonographic diagnosis of adenomyosis. 47 videoclips and 3D volumes (25 cases with adenomyosis and 22 cases without adenomyosis according to expert subjective impression) were evaluated by one expert observer and five less-experienced researchers. All four non-expert examiner underwent a one-day theoretical course and example cases about criteria for suspecting adenomyosis according to MUSA consensus. In addition, all trainees were trained on the use of a dedicated software for manipulating 3D volumes. Each sonographic adenomyosis criterion (globular shaped uterus, myometrial asymmetry, myometrial cysts, fan-shape shadows, echogenic islands, interrupted junctional zone, heterogeneous myometrium) was evaluated separately. If two or more criteria were found, adenomyosis was diagnosed. Interobserver agreement was analysed by the kappa index with 95% confidence interval (CI). Out of the 47 analysed videoclips and volumes, trainees 1, 2, 3, 4 and 5 identified adenomyosis in 24, 27, 24, 28 and 24 cases, respectively. We observed a good degree of agreement among all the trainees with kappa values that ranged from 0.66 (good) to 0.83 (very good) (table 1). The degree of agreement between each trainee and the expert was good in all the cases. The sonographic criteria used in diagnosing adenomyosis were reproducible parameters in adenomyosis diagnosis, even by less experienced observers.
