Low-Grade Endometrial Stromal Sarcoma: Clinicopathologic and Prognostic Features in a Cohort of 102 Tumors

This retrospective clinicopathologic study analyzed 102 patients with low-grade endometrial stromal sarcoma treated with hysterectomy and staged using FIGO 2018 and FIGO 1988 criteria, incorporating detailed microscopic assessments (including depth/pattern of myometrial permeation, spread to cervix/serosa/parametrium, and specific morphologic variants). The authors found 34 recurrences and 9 disease-specific deaths among those with follow-up (median 79 months), with 5-year recurrence-free survival of 80% and disease-specific survival of 94%, decreasing to 51% and 87% at 10 years. On univariate and multivariate analyses, cervical stromal involvement was associated with shorter recurrence-free survival, and lack of a conventional component and myxoid stroma were also linked to worse outcomes in univariate testing, with a major limitation that prognostic prediction remains difficult given the rarity of aggressive behavior and the heterogeneity of adjuvant treatments.
