Abstract

Abstract Purpose This study aims to evaluate whether diode laser-assisted deroofing, integrated into Minimally Invasive Techniques for Hidradenitis Suppurativa (MITHS), can accelerate wound healing, reduce recurrence, and improve symptom resolution compared to conventional surgical approaches. Does diode laser therapy as part of MITHS enhance clinical outcomes in HS patients versus traditional surgical methods? Methods We conducted a retrospective single center, single surgeon study between January 2022 and January 2024 involving 55 HS patients (Hurley stages 2–3) who underwent MITHS with a 1470 nm diode laser ablation after excisional tunneling debridement with an ultrasonic instrument. Hidradenitis tracts were ablated intraoperatively using the lasers. Outcomes were assessed based on symptom resolution: full, partial, or unresolved which was self reported. Patient demographics, prior treatment history, lesion location, and adverse effects were also recorded. Results Among 55 patients, 62 MITHS procedures were performed. Initially, 60% (n = 27) achieved complete resolution and 40% (n = 18) partial resolution. After subsequent treatments, 58% (n = 36) were complete and 29% (n = 18) partial; 13% (n = 8) were lost to follow-up. No symptom worsening occurred. Adverse effects were minimal and transient, including two reversible upper extremity nerve palsies. All procedures were well-tolerated under local anesthesia. Conclusion Diode laser therapy as an adjunct to MITHS shows high efficacy, minimal morbidity, and rapid recovery, offering a less invasive alternative for HS management. It may achieve remission or cure in moderate-to-severe cases and addresses unmet therapeutic needs. Further prospective studies are needed to validate and optimize its role in multidisciplinary care.

Hidradenitis Suppurativa Diode laser Minimally Invasive Inflammation Figures Figure 1 Figure 2 Figure 3 Background Hidradenitis suppurativa (HS) is a chronic inflammatory skin condition that primarily affects apocrine glands. Also known as acne inversa, HS is a relatively common condition that affects nearly 1% of the global population 1 , an estimated 81 million people. HS typically occurs after puberty with the average age of onset in the 2nd or 3rd decades of life with a female predominance 2 . While the exact etiology of HS is unclear, the pathophysiology involves follicular occlusion of the follicular pilosebaceous unit followed by follicular rupture and a cyclical pro-inflammatory response. This inflammatory vicious circle ultimately results in local pain, purulence, tissue destruction and scarring in the apocrine- rich areas of the body, most commonly the groin, axilla, inner thighs, buttocks and breasts 2 – 3 . It is a multifactorial disease in which genetic predisposition and environmental factors play a key role in disease severity and progression. Due to lack of understanding of HS pathogenesis, a valid animal model and the multifactorial nature of the disease there have been challenges in the development of therapeutic approaches 3 . Conservative treatment includes patient education, pain management, adhering to strict hygiene standards, cessation of smoking, losing weight, amongst other strategies 1 , 3 . Systemic antibiotics are considered first line therapy according to guidelines, though none are officially FDA approved 1 , 3 . Use of immunosuppressants or immunomodulatory drugs has little or conflicting evidence for their efficacy 4 . Adalimumab, an anti-TNF-alpha-antibody and secukinumab, an interleukin-17A inhibitor are currently the only two FDA approved therapies for the treatment of moderate-to-severe HS 5–6 . Surgical intervention remains the only potentially curative therapy to a local area with current options limited to incision and drainage, radical excision, or deroofing. However in recent years, several minimally invasive treatments for HS (MITHS) have been developed for hard-to-heal wounds of various dermatologic diseases. These techniques include hydrosurgical and ultrasonic debridement/excision of abscess/ hidradenitis tracts. MITHS has been proposed as a treatment strategy for acute flares that require prompt, effective intervention to prevent escalation into a full body flare and staging to radical excision where remission cannot be achieved. This method can also achieve full remission and can be used as a maintenance therapy with the aim of minimizing recurrence or maintaining remission of HS symptoms. It has been well established that laser therapy enhances and accelerates the process of wound healing 7 , 8 . Recently, intralesional laser and light-based therapies have become more commonly used in the management of HS. Laser therapy can act in two different ways, selective and ablative. Diode lasers act selectively and are considered the most useful in Hurley stage 1 and 2, because of their ability to reduce the number of hair follicles, sebaceous glands and consequently to reduce the bacterial load 9 – 11 . Although these lasers have been used topically, we adopted the method of using a diode laser that enters the tract and ablates it 12 .By combining the limitations of traditional surgical intervention with our emerging understanding of MITHS, we are given an exciting and novel opportunity to enhance care for future HS patients. In addition to the MITHS procedures, we propose diode laser therapy as a promising adjunct to improve wound healing and speed recovery after these procedures. We report 55 HS patients treated intraoperatively during a MITHS procedure with a 1470 nm wavelength, emitted from a diode laser into debrided abscess/hidradenitis tracts. Methods A retrospective study was conducted using the EPIC electronic health record (EHR) for patients diagnosed with Hidradenitis Suppurativa (HS) who underwent MITHS with Diode laser ablation of tracts from January 2022 to January 2024. This study was carried out at a single center and included patients seen by a sole surgeon, resulting in 55 individual records and a total of 62 instances of laser therapy performed. Data collected for each patient included patient-specific demographics, prior medical history, prior HS treatment history, location of lesion(s), date of laser therapy, and date of last follow-up. The primary outcome measured was the resolution status, categorized as 'Full Resolution' if no further purulent drainage was observed from the regions that underwent laser ablation and 'Not Resolved' if continued significant drainage was reported by the patient. Additionally, the designation of 'Partial Resolution' was given when drainage was observed in some but not all tracts, or when the drainage was significantly reduced following laser therapy. Analysis consisted of calculating the percentage of self-reported resolution status. Results Patient outcomes were collected in 55 patients, 15 males and 40 females aged between 20 and 70 years old (mean age 39) suffering from HS. Self-reported racial demographics were noted as predominantly Black ( n = 30,54.5%) followed by White (n = 16, 29%) with 3.6% (n = 2) reporting as unspecified. 34.5% (n = 19) of patients had a history of tobacco use with 10.9% (n = 6) reporting to be current users, 23.6% (n = 13) reporting previously having used and 65.4% (n = 36) reporting no history of tobacco use. The most reported comorbid conditions being obesity (n = 36, 65.4%), hypertension (n = 18, 32.7%) and Type 2 Diabetes Mellitus (n = 16, 29%). We also report 6 (10%) patients with inflammatory bowel disease (IBD) and 7 (12.7%) patients with PCOS. Nearly all patients were considered to be at an HS Hurley stage 3 (n = 47), with 3 HS Hurley stage 2 patients and remaining patients were unspecified (n = 5). Anatomical sites affected by HS were the axilla, groin, buttocks, labia, perineum, thigh, mons pubis, breast, lower abdomen, back, perianal, face, neck, chest and scrotum. Lesions of the groin (26.6%), axilla (26%) and buttocks (17.1%) were the most common locations compared to the overall number of lesions. The demographics of the patients have been summarized in Table 1 . Out of the 55 patients, 6 patients had multiple procedures with a total of 62 MITHS procedures and 4 radical excisions performed. Diode laser therapy was only utilized during the MITHS procedures. After the initial MITHS procedure, 27 patients reported full resolution (60%) and 18 patients reported partial resolution (40%) ( Fig. 1 ) . 4 patients with partial resolution reported a period of symptom remission for a range of 2–6 months. After all the MITHS procedures, 36 patients reported full resolution (58%) and 18 reported partial resolution (29%) with 8 patients (13%) lost to follow up ( Fig. 2 ) .Of those that reported partial resolution, 10 patients (58.8%) reported 76%-99% perceived symptom relief, 2 (11.8%) reported 51%-75% symptom relief, 2 (11.8%) reported 26%-50% symptom relief and 3 (17.6%) reported 1%-25% symptom relief ( Fig. 3 ) . All patients reported at least some symptom relief and denied any worsening of symptoms. All patients tolerated the MITHS procedure and laser therapy well. The only adverse effects noted were transient upper extremity nerve palsies in 2 patients after procedures involving the axillary region, both attributed to stretch, not thermal injury. One patient did receive therapy and the other had spontaneous resolution of symptoms with a normal electromyography. Serious adverse side effects, suppuration or infections did not occur. Future treatment should be underpinned by personalized therapy and surgical treatments that reflect the goals of the individual patient. HS can have a great impact on a patient’s quality of life, social and work activities due to frequent relapses and new flare locations. In an uncertain field of various surgical techniques, it is difficult to identify the best way to stop progression of the disease. Historically, surgical intervention has been limited to when disease severity has progressed to the point of maximal medical therapy. It is our opinion that surgical procedures like MITHS should become a regular part of maintenance therapy. In our patients, most patients required repeat MITHS every 6 months to 2 years if they fell into the Stage III category. The use of diode laser therapy in conjunction with minimally invasive surgical techniques could be considered as an alternative approach to the traditional, more extensive surgical options. Diode laser therapy has been associated with good clinical outcomes in several studies 9 , 13 – 15 . It is postulated that laser therapy may be useful in the treatment of HS through the induction of selective, cytotoxic effects and immunomodulatory factors 15 . Additionally, photodynamic therapy has been used for its ability to reduce biofilms, which is a common finding in hidradenitis lesions 14 . The main limitations that other researchers have found with the diode laser being poorly absorbed with low penetration can be mitigated by use intralesionally after minimally invasive debridement of hidradenitis tracts. In our study, diode laser therapy was utilized as an adjunct to surgical treatment of targeted hidradenitis lesions. The full procedure entails: we enter the tract through the existing opening then use the surgical debridement device with hypochlorous acid to excise the infected tract. The debridement device is ergonomically suited to fitting within the tract because it is > 10 cm in length and < 5 mm in diameter allowing for excision of the entire tract. This effectively creates a sterile tunnel that is amenable to laser destruction instead of traditional deroofing. We laser the tract, starting either at the blind end of the tract or 0.5 cm from the other opening to avoid burning the skin. We then pull the laser filament back at a rate of 1 mm per millisecond, stopping 0.5 cm from the skin opening. This leaves a minimal open wound that heals within weeks instead of months. The full benefits of the laser are seen at one month post operatively. Patient recovery is 3–5 days with minimal pain and all immunomodulators can be continued. Sixty percent of patients reported full resolution after the initial surgery. After subsequent procedures, complete resolution was reported in 64% of patients, potentially indicating a dose-dependent additive benefit. Even in patients that did not experience full resolution of their symptoms, more than half (66.7%) of these patients reported markedly improved symptom relief (76%-99% symptom relief). This substantial improvement with MITHs should be combined with ongoing maximal medical (via dermatology and infectious disease) and lifestyle interventions to achieve and maintain full remission. Every patient reported some level of improved symptom relief after the use of the diode laser during a MITHS procedure. Moreover, minimally invasive procedures, such as MITHS with diode laser therapy, can be performed under local anesthesia and therefore do not require hospitalization. The short recovery period for this procedure adds to its accessibility for patients that may not have the time for a prolonged recovery course, as is the case with traditional surgical interventions. In our study, the treatment was well tolerated with very few adverse side effects that were transient and resolved without treatment. Patients returned to work within 5 days in most cases. This treatment approach could be a good alternative for those patients in which conservative or medical treatment is not efficacious and large radical excision is not conducive to their life. Laser therapy can be performed in patients utilizing concurrent biologic treatment because it doesn’t increase the risk of immunosuppression and does not induce auto-antibody formation, therefore can be repeatedly used without complications 11 . The main limitations of our study are our small sample size and the retrospective acquisition of data collection. Bias may have been introduced in the patient's subjective perspective of their perceived symptom relief. However, there is very little data in the literature focused on the use of diode laser therapy to improve symptoms of HS. Future research would be insufficient if patient perspectives in improvement of their symptoms were not collated alongside more robust data sources, especially considering the composite effect of HS on a patient’s quality of life. Conclusion Diode laser therapy used in adjunct with MITHS could represent a less invasive, less inflammatory and more accessible alternative approach compared to more extensive surgical approaches, such as I&D, deroofing and radical excision. Future studies investigating the clinical applications of diode laser therapy in HS will likely need to adopt a multidimensional approach to understanding and intervening in this patient population to yield meaningful benefits. Does diode laser therapy as part of MITHS enhance clinical outcomes in HS patients versus traditional surgical methods?\u003c/p\u003e\u003ch2\u003eMethods\u003c/h2\u003e \u003cp\u003eWe conducted a retrospective single center, single surgeon study between January 2022 and January 2024 involving 55 HS patients (Hurley stages 2\u0026ndash;3) who underwent MITHS with a 1470 nm diode laser ablation after excisional tunneling debridement with an ultrasonic instrument. Hidradenitis tracts were ablated intraoperatively using the lasers. Outcomes were assessed based on symptom resolution: full, partial, or unresolved which was self reported. Patient demographics, prior treatment history, lesion location, and adverse effects were also recorded.\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e \u003cp\u003eAmong 55 patients, 62 MITHS procedures were performed. Initially, 60% (n\u0026thinsp;=\u0026thinsp;27) achieved complete resolution and 40% (n\u0026thinsp;=\u0026thinsp;18) partial resolution. After subsequent treatments, 58% (n\u0026thinsp;=\u0026thinsp;36) were complete and 29% (n\u0026thinsp;=\u0026thinsp;18) partial; 13% (n\u0026thinsp;=\u0026thinsp;8) were lost to follow-up. No symptom worsening occurred. Adverse effects were minimal and transient, including two reversible upper extremity nerve palsies. All procedures were well-tolerated under local anesthesia.\u003c/p\u003e\u003ch2\u003eConclusion\u003c/h2\u003e \u003cp\u003eDiode laser therapy as an adjunct to MITHS shows high efficacy, minimal morbidity, and rapid recovery, offering a less invasive alternative for HS management. It may achieve remission or cure in moderate-to-severe cases and addresses unmet therapeutic needs.
