Figures Figure 1 Figure 2 Figure 3 Figure 4 Figure 5 Figure 6 Figure 7 Figure 8 Figure 9 Figure 10 INTRODUCTION AlloHSCT (allogeneic hematopoietic stem cell transplantation) is a proven therapeutic modality for the treatment of malignant and benign hematological diseases but is still associated with significant morbidity and mortality [ 1 , 2 , 3 ]. The main causes of failure remain disease relapse and treatment-related mortality. The antileukemic effect of AlloHSCT is a result of two key biological processes: the elimination of residual, chemosensitive tumor cells from the conditioning chemotherapy and the antileukemic effect of the graft (graft versus leukemia, GVL). The importance of the GVL effect described and highlighted by the Seattle group in 1970 and highlighted by the success of non-myeloablative allogeneic transplantation [ 4 , 5 ]. The effector mechanism of GVL and GvHD is complex and not yet fully understood [ 6 ]. Medicine continues to search for risk factors directly related to improving the outcome of AlloHSCT. Crucial to improving overall survival and reducing relapse-free mortality after AlloHSCT is timely and functionally adequate immune reconstitution. A surrogate marker for it is the recovery of the donor lymphoid system represented by lymphocytes circulating in the peripheral blood. The role of ELR as a biologically relevant prognostic marker for transplant outcome has been investigated in a number of studies [ 7 , 8 , 9 , 10 , 11 ]. AIM OF THE STUDY The aim of our study was to identify the prognostic significance of the early lymphocyte recovery (ELR), presented as absolute lymphocyte count (ALC) for the outcome of AlloSCT. MATERIALS AND METHODS 1. Patients The medical records of 96 patients with acute myeloid leukemia (AML), myelodysplastic syndrome (MDS) and acute lymphoblastic leukemia (ALL) who received AlloHSCT between 2017–2021 years. Demographic, clinical, and laboratory data were obtained from the hospital information system (HIS). Patients were selected based on the following criteria. Inclusion criteria were: 1. Patients aged ≥ 18 years. 2. Diagnosis: AML, MDS, ALL. 3. Indicated to proceed to AlloHSCT. 4. Signed informed consent. 5. Documented engraftment. 6. Survival 30 days after AlloHSCT. Exclusion criteria used: 1. Patients with acute promyelocytic leukemia. 2. Patients with relapsed or de novo acute leukemia after first AlloHSCT. 4. Patients with primary graft failure, graft rejection, poor graft function, according to generally accepted definitions [ 12 ]. 2. Methods The choice of regimen intensity was at the discretion of the treating team based on the patient's age and comorbidity index. MAC-eligible patients with AML and MDS were preferentially conditioned with a busulfan-based regimen (Bu 3.6 mg/kg for 4 days), and those with ALL with a total body irradiation (TBI)-based regimen (10–12 Gy). Patients transplanted from a match unrelated donor (MUD) undergo “in vivo” T-cell depletion with antithymocyte globulin (ATG). All patients received standard anti-infective prophylaxis (Ciprofloxacin, Fluconazole, Acyclovir), prophylaxis for veno-occlusive disease (VOD) with Ursodeoxycolic acid and immunosuppression with cyclosporin A (CycA) + methotrexate (MTX) / mycophenolic acid (MMF) + tacrolimus (TAC) / post-transplant cyclophosphamide (PTCy) + MMF + TAC/CycA +ММF. 3. Statistical analysis Statistical analysis includes assessment of quantitative and categorical variables with absolute and relative frequencies. The type of the distribution was established with a Kolmogorov–Smirnov test. A chi (χ²) square analysis (Fisher's exact test) was applied to test the relationship between categorical variables. A non-parametric Mann-Whitney test was used for testing the significance of the difference between two average values from two independent samples. Cox regression analysis as well as log-rank test (Mantel-Cox) were applied to search for significant factors related to survival and other time-evolving events. Mean and median time-to-event were assessed by Kaplan-Meier’s method. Values of p < 0.05 were considered significant. Analysis was performed by IBM SPSS v. 22. RESULTS 1. Characteristics of the patients and transplantation program The baseline characteristics of all 96 patients included in the analysis are presented in Tables 1 . The median follow-up was 54.7 months (95% CI 45.86–63.6). The median age was 43 years (32–52), ranging from 19 to 68 years. The leading indication for AlloHSCT was AML, in 63.5%. Patients' physical status and comorbidity related to AlloHSCT were assessed with ECOG-PS [ 13 ] and HSCT-CI [ 14 ]. More than half of the patients were transplanted in first remission CR1 -53.1% and calculated DRI [ 15 ]- low/intermediate − 54.2%. Regarding the profile of the transplant program, 55.2% of patients were transplanted from a MUD. 55.2% underwent myeloablative conditioning. GvHD prophylaxis with antithymocyte/antilymphocyte globulin (ATG) was used in 46.9% of patients included in the analysis. ATG was the choice of GvHD prophylaxis in unrelated donor transplants. For 8.3% of MUD patients ATG was not available at time of transplantation. All patients received G-CSF mobilized hematopoietic stem cells from peripheral blood. 2. Early lymphocyte recovery and absolute lymphocyte count Assessment of ELR as ALC was performed on D + 21 and D + 30. Considering that the lowest normal reference value for ALC (0.5 x 10 9 /l) is not clinically relevant for this patient population, we used Cox regression analysis to test the significance of ALC factors – D + 21 and D + 30 for assessment of OS. Only D + 21 proved to be a significant factor for OS (p = 0.004, HR = 0.998, 95% CI 0.996–0.999). The function thus obtained from the Cox analysis was transferred into a ROC curve to determine the best cut-off value of D + 21 in terms of OS (Figure №1). The area under the curve is 0.797 (at a population value of 0.5) and is statistically significant (p < 0.001). The limit value of the function was determined to be 0.5376, which corresponds to ALC 0.220× 10 9 /l of D + 21. Based on the created new variable, the patients were grouped and evaluated in two groups – DLR with ALC < 0.220 x 10 9 /l and ELR with ALC ≥ 0.220 x 10 9 /l. 3. The role of early lymphocyte recovery for transplant outcome Stratified by lymphocyte recovery, the median OS for ELR was not reached compared with 225 days for DLR (p < 0.001), Fig. 2 . In terms of PFS, it was also demonstrated a significant difference in the two cohorts, the median PFS for ELR was not reached compared to 196 days for the DLR group (p < 0.000), Fig. 3 . The median CIR was not reached for patients with ELR compared to those with DLR, which was 393 days (p = 0.022), Fig. 4 . Median NRM was not reached in both groups, but time to event was significantly longer for patients with ELR, 2261 days compared to 1889 days for the DLR group, (p = 0.003), Fig. 5 . The results at 1st and 3rd year of AlloSCT stratified by lymphocyte recovery are presented in Tables 2 . Significantly better results were reported both at 1st and 3rd year in patients who achieved ELR at D + 21 after stem cell infusion. Factors associated with early lymphocyte recovery A statistically significant relationship was proved between ALC D + 21 and DRI, response rate before AlloHSCT and the choice of a haploidentical donor – Table 3 . We performed a univariable and multivariable analysis to estimate the relationship between ELR and these factors in the terms of OS. The univariable model identifies as a significant – ECOG PS, HCT CI, DRI and response rate before AlloHSCT, of them only DRI was confirmed by the multivariable analysis (p = 0.04, HR 1.926, 95.0% CI 1.030–3.603). We studied the degree of cytokine response in the preengrafment period, assessed by the maximum value of serum CRP (max CRP) during the first seven days after AlloHSCT and maximum body temperature (max BT) - Fig. 6 . So far it is known that CRP may be a net surrogate marker for early disease/syndrome leading to the development of a/cGVHD and eventual NRM [ 17 ]. Our results show that high values of max CRP (normal values up to 5 mg/L for our laboratory) in this period lead to delayed early lymphocytic recovery. The median max CRP in the group of patients with ALC D + 21 below 0.220 x 10 9 /L is 139.5 mg/L, in contrast of only 27.5 mg/L for patients with ALC D + 21 above 0.220 x 10 9 /L (p = 0.001). 4. Analysis of post transplantation complications and mortality according to lymphocyte reconstitution. The frequency of aGvHD was statistically significantly higher in patients with DLR, Table.8. To assess the relationship between ALC and aGvHD in the context of OS, we divided the sample into two groups: with and without manifestations of aGvHD. In both groups, the higher median survival of patients with ELR was seen. In the aGvHD group, significance was borderline (p = 0.055), while in the non-aGvHD group, the difference was significant (p = 0.001), Figs. 7 and 8 . Median OS for patients with ELR was not reached in both groups. For ELR patients with registered aGvHD, it was 208 days, compared to 320 days for those without. We can conclude that patients with ELR have a lower incidence of aGvHD and better survival, regardless of the development or not of an acute graft reaction. The frequency of cGvHD was significantly higher in the ELR group 79.2% vs. 20.8% for the DLR (p = 0.035), respectively. It’s seen that all patients with mild chronic graft reaction have ELR, without this being confirmed as a statistically significant fact, Table 4 . To evaluate the relationship between ALC and cGvHD in terms of OS, we divided the sample into two groups: with and without cGvHD. In both groups, the higher median survival of patients with ELR was seen, Figs. 9 and 10 , (p = 0.040 in the group with cGvHD, p = 0.003 in the group without). Median OS for patients with GvHD was not reached in both groups. For patients with GvHD with recorded cGvHD, it was 208 days, compared to 417 days for those without. We can conclude that patients with ELR have a better survival, despite the higher frequency of cGvHD registered in them. One of the explanations of this is the fact that ALC could’n represent the speed of immune, recovery of CMV-specific cytotoxic T-lymphocytes. We couldn’t estimate the significance of ERL/DLR for other types of viral infection such as adenovirus and BKV, we have only 2 patients with proven adenovirus infection and 6 cases with BKV cystitis. The immunohematology recovery, followed dynamically in both groups, are presented in Table 5 . A significant delay in neutrophil and platelet recovery was observed in the group of DLR. As well as lower values of WBC, Hb, PLT, NK-cells, Neu at the end of the first month of AlloSCT, without statistical significance for NK-cells and Neu. Most of them include a small number of patients are limited to a specific disease type, donor type, conditioning regimen, and graft source. Le Blank [ 16 ] and Michaelis [ 17 ] analyzed only patients with AML, Kumar S et al. [ 18 ] considered only cases of bone-marrow transplantation, and Amandine Le Bourgeois [ 19 ] - umbilical cord transplantation. They covered a wide range of time points for post-transplant assessment (days 21 to 100 after AlloHSCT) and offered different threshold values for defining low ALC (0.175 x 10 9 /l cells to 0.5 x 109/l). Our initial analysis on the role of ELR for the outcome of AlloHSCT, in assessing OS, defined a “cut off” value for ALC of 0.220× 10 9 /L and time point D + 21 from transplantation as an independent prognostic factor for prolonged OS. Rigoni et al [ 20 ] demonstrated the association of ELR on D + 21 with higher OS in a patient population with a profile similar to ours, choosing an arbitrary cut-off value for analysis of ALC of 0.300× 10 9 /L. Kim et al [ 21 ] identified as a cutoff value with prognostic significance for OS - ALC − 0.350 x 10 9 /L on D + 21. Kumar et al. concluded that ALC below 0.175 x 10 9 /L on D + 21 was a predictor of relapse risk in transplanted ALL patients. The study of the relationship between ELR and other clinical endpoints assessing the effectiveness and safety of AlloHSCT – PFS, CIR, NRM proved that patients with ELR on D + 21 had longer relapse-free survival and time-delayed NRM-related event. Regarding NRM, the medians were not reached in both groups, but the time to event was statistically significantly longer for patients with ELR. In contrast to Rigoni et al., after median follow-up of 20 months, their study demonstrated a significantly longer disease-free survival, but not NRM, for patients with ALC > 0.300× 10 9 /L on D + 21. Kim et al. demonstrated a significantly longer risk-free survival and a lower relapse rate in patients with ALC > 0.350× 10 9 /L on D + 21, but a non-significant difference for NRM. Kumar et al. reported a significantly longer risk-free survival and lower relapse rate in patients with ALC > 0.175× 10 9 /L on D + 21 but did not assess NRM. This is the first study to identify risk factors for delayed lymphocyte recovery - DRI, the response to AlloHSCT and the severity of the cytokine response in the pre-engraftment period. Known factors leading to DLR are HLA-incompatible donor, dose of infused CD 34 + cells, administration of ATG, administration of G-CSF to stimulate engraftment, aGvHD [ 22 , 23 , 18 ]. Our results confirm that the choice of a haploidentical donor is an unfavorable factor for ELR − 8 (72.7%) of patients with a haploidentical donor had DLR (p = 0.029). The influence of the haploidentical donor on lymphocyte recovery is well known [ 24 ]. Delayed immune reconstitution in haploidentical transplantation is a distinctive feature of the immunopathogenesis of this type of procedure. Unlike Kim et al. [ 25 ] and Damlaj et al. [ 26 ], in our study the relationship between the amount of infused CD34 + cells and DLV was not confirmed. In terms of OS only DRI was confirmed by the multivariable analysis as a significant risk factor for DLR (p = 0.04, HR 1.926, 95.0% CI 1.030–3.603). Analysis of data regarding the significance of ELR in terms of complications in the post-transplant period reached key conclusions regarding aGvHD, cGvHD and their association with survival. ELR correlates with the clinical manifestation of an acute graft reaction, as also established by Rigoni et al. Analysis stratifying patients based on median D + 21 lymphocyte recovery also showed a statistically significant difference in median D + 21 ALC in patients with and without aGvHD manifestations. These results translated into improved survival for patients with ELR both with overt aGvHD and without documented reactivity. This raises the question of conducting additional research aimed at clarifying the dynamics of immune reconstitution and identifying the lymphocyte subpopulations involved in the immunopathogenesis of aGvHD and the role of immunoregulatory cells. We also found a higher frequency of cGvHD in patients with ELR, in contrast to Rigoni et al., they only proved a correlation between ALC and the incidence of aGvHD. Despite the higher frequency of cGvHD for ELR, their median OS was not reached. The improved OS in the ELR group at a higher incidence of cGvHD confirms the protective role of cGvHD and supports the hypothesis of Damlaj et al that ELR may be a surrogate marker for GVL. A significant association between early lymphocyte recovery, presented in the context of LD-index (Lymphopenia duration), and cGvHD was also observed by Yanagi and colleagues [ 27 ]. Further studies of lymphocyte subpopulation composition in the setting of cGvHD are needed to elucidate the immunopathogenesis of these results. Our study confirms that ELR can be a statistically proven surrogate marker for stable graft engraftment, tracking the time to hematologic recovery for neutrophils and platelets (and D + 30 hematological recovery from AlloSCT). A drawback of our study is that the role of the graft source for immunohematologic after AlloSCT could not be evaluated because patients who received bone marrow stem cells were excluded from the study due to their extremely small number. CONCLUSION We can conclude that ELR presented as ALB of D+21≥ 0.220 x 10 9 /L is a clinically significant prognostic factor for the outcome of transplantation - universal, accessible, easily measurable and cheap. It could serve as a starting point already in the early post-transplant period to initiate risk-adapted immunomodulatory strategies in patients with high-very high DRI and those not transplanted in CR1. Considering that the lowest normal reference value for ALC (0.5 x 10\u003csup\u003e9\u003c/sup\u003e/l) is not clinically relevant for this patient population, we used Cox regression analysis to test the significance of ALC factors \u0026ndash; D\u0026thinsp;+\u0026thinsp;21 and D\u0026thinsp;+\u0026thinsp;30 for assessment of OS. Only D\u0026thinsp;+\u0026thinsp;21 proved to be a significant factor for OS (p\u0026thinsp;=\u0026thinsp;0.004, HR\u0026thinsp;=\u0026thinsp;0.998, 95% CI 0.996\u0026ndash;0.999).
