Zusammenfassung Neuentwicklungen auf dem Gebiet der operativen Endoskopie führten zu einer wesentlichen Erweiterung der Behandlungsmöglichkeiten bei der Endometriose. Bei entsprechender Ausrüstung und Ausbildung der Operateure können Endometriosebefunde bis Stadium IV nach AFS laparoskopisch behandelt werden. Grundprinzip der Therapie ist die radikale Entfernung aller sichtbaren bzw. tastbaren Endometrioseherde. Eine exakte präoperative Diagnostik ist bei ausgedehnten Befunden unerläßlich. Die Vorteile der Laparoskopie liegen in der geringeren postoperativen Morbidität und besseren Lebensqualität. Hinsichtlich der Radikalität und postoperativen Fertilität ist die Laparoskopie gegenüber der Laparotomie als gleichwertiges Verfahren anzusehen.
