Analysis of Pregnancy Rate with Expectant Management and Controlled Ovary Hyperstimulation Treatment in Endometriosis-associated Infertility Patients after Laparoscopic Treatment

Abstract
Objective:To compare pregnancy rate with expectant management or controlled ovary hyperstimulation(COH) treatment in endometriosis-associated infertility patients after laparoscopic treatment.Methods: Patients were divided into two groups,expectant management group had 231 patients and 2309 natural cycles,COH treatment group had 129 patients and 187 cycles.Cycle pregnancy rate and the accumulative pregnancy rate within 12 months were analyzed.Logistic regression was used to explore influential factors related to pregnancy rate.Results: Cycle pregnancy rate with expectant management and COH treatment was 3.29% and 21.39%,respectively.Cumulative pregnancy rate with expectant management that each patient has mean 10 expectant cycles is 32.9%.The cumulative pregnancy rate with COH that each patient has mean 1.5 COH cycles is 31.01%(P0.05).No association was observed between the pregnancy rate and age,GnRH-a use,or endometriosis stage.Conclusions: Patients with endometriosis-associated infertility after laparoscopic therapy can consider expectant management in short time.COH treatment can increase the cycle pregnancy rate.If ovary function is normal,the age dose not affect the pregnancy rate.Using GnRH-a after surgery can not improve the pregnancy rate,and the EM r-AFS staging has the limited value in predicting the fertility outcome.
