Clinical and Psychological Status Analysis of 455 Children and Parents Infected with Familial Aggregation Omicron Variant in Shanghai in Parent-Child Ward

Abstract

Objective: The clinical characteristics, treatment outcomes and sleep psychological problems of children and parents infected with familial aggregation Omicron variants under a parent-child ward treatment mode were comprehensively analyzed to determine the effects of local and regional familial aggregation infections on children and their families and accumulate experience in the diagnosis and comprehensive treatment of Omicron variant strains.Methods: The clinical data of 225 children and 230 adult family members admitted at the Lingang District of Shanghai’s Sixth People’s Hospital from April 8, 2022, to May 31, 2022, were retrospectively collected and analyzed to investigate their clinical characteristics and response to treatments. The factors of interest were integrated Traditional Chinese Medicine (TCM) and western medicine treatment, respiratory viral load dynamics, change of detoxification status and different members of the disease turnaround time. Their sleeping habits and psychological dynamics during hospitalization were also comprehensively analyzed via a WeChat mobile questionnaire.Results: (1) The clinical symptoms of fever, nausea, vomiting and wheezing in the children were significantly higher than in adults (P < 0.05). In addition, nasal congestion, runny nose, cough, dry pharynx, pharynx itching and pharyngeal pain were lower than in adults (P < 0.05). The number of adults who underwent TCM, antiviral and symptomatic treatments was significantly higher than children (P < 0.05). (2) The time of turning negative in the moderate group was higher than in the mild group, and the time of turning negative in the unvaccinated group was higher than in the vaccinated group (P < 0.05). (3) The Cycle Threshold Value (Ct value) of ORF1ab and N gene of the children group were higher than the adult group. The time of turning negative in the adult group treated with TCM and antiviral treatment was faster than that in the symptomatic treatment group (P < 0.01) within 11 days after infection (P < 0.05). The increase rate of Ct value of ORF1ab and N gene in adults treated with TCM was significantly higher than in those who underwent symptomatic treatment (P < 0.01). (4) By scoring the children’s CSHQ, we found varying levels of sleep problems at all ages, sleeping habits, latency and anxiety, night awakenings and abnormal sleep (P < 0.05). In the adult group, SSRS ≥3 scores accounted for more than 50% of adults with insomnia, sleep deprivation, sleep instability and early awakening. The distribution of males and females was consistent with the overall distribution.The proportion of adults with anxiety and depression was 21.3% and 16.4%, respectively, and the associated symptoms were related to the COVID ,work-related factors, time(end of the pandemic), safety and family factors (P < 0.05).Conclusion: Infections in children in this epidemic were mainly family cluster infections. The clinical symptoms of children and adults were mainly upper respiratory tract related. With comprehensive treatment, children converted to negative faster, vaccination was conducive to recovery, and all patients had good prognoses.
