The findings of this study will serve as a reference for the future construction of the committee. Methods: Using a questionnaire based on the Kano model to conduct the survey on 606 grassroots medical personnel in Shanghai to investigate their needs for the construction of RECs. Results: Among them, 77.23% reported having knowledge of the RECs, 68.15% of respondents believed that the regional health administrative department or a level 2 or above comprehensive medical hospital recognized by it after evaluation should be responsible for constructing the RECs. Out of the 38 construction demand survey projects, 4 are Must-be Quality, 19 are One-dimensional Quality, 7 are Attractive Quality, and 8 are Indifferent Quality. The quadrant matrix diagram shows that 19 requirements are in the advantage area, 8 requirements are in the area that needs improvement, 7 are in the secondary improvement area, and 4 are in the maintenance area. No reverse or problematic attributes were found. Conclusion: In the construction of RECs, relevant departments can use the attributes of the needs as benchmarks and promote construction in a hierarchical and phased manner. This will gradually achieve full coverage of the needs at all levels and track changes in the types of needs. The aim is to achieve a dynamic balance of the various types of needs at a higher stage. Regional ethics committee Demand Kano model Grassroots medical personnel Figures Figure 1 Background In 1987, the term 'Medical Ethics Committee' was first introduced in China[ 1 ], coinciding with the emergence of the 'Hospital Ethics Committee'. Currently, there are three main types of ethics committees: national or provincial, those in major hospitals and medical colleges, and those in regular hospitals[ 1 ]. Ethical reviews involving the first two types have been refined over the years. As grass-roots hospitals begin to conduct medical research, it is important that their ethics review process is given due consideration and improved. To achieve this goal, one effective method is to establish a regional ethics committee (REC). This simplifies the ethical review process and makes it more efficient. Over the past few decades, clinical research has become increasingly global and complex. Many clinical trials have been conducted to identify factors that can improve patients' health. Even in smaller hospitals, the complexity of research has led to ethical issues for researchers and institutional review boards (IRBs)[ 2 ]. These issues call for a regional ethical committee. Research indicates that numerous Chinese ethical committees continue to encounter significant challenges. Primary hospitals in China currently lack mature or complete ethics committees, which may have a negative impact on research at this stage[ 3 ]. RECs can cover multiple primary hospitals and provide unified ethical review standards and services. Constructing RECs can effectively meet the realistic needs of science, technology, and innovation in primary hospitals. The Kano model is a useful tool for categorising and prioritising user requirements based on survey data. It has proved to be widely applicable in the practice of user requirements analysis.[ 4 ]This study analyses the demand for REC construction among primary healthcare workers in a district of Shanghai, based on Kano's theoretical model. The aim is to provide a reference for carrying out the construction of RECs at a later stage. Participants and methods Participants From March to April 2023, the convenience sampling method was used to select the medical staff of primary hospitals in Shanghai for the questionnaire survey. Inclusion criteria: ① medical staff whose medical institutions are classified as level 1 and level 2 (including specialty and general hospitals); ② voluntary participation in this survey. Exclusion criteria: ① private and private medical institutions; ② advanced students, trainees; ③ not on duty during the survey period, including business trips, study and holidays. The study was reviewed by the Ethics Committee to ensure that the study strictly adhered to the principles of informed consent, confidentiality and no harm. The questionnaire contained 17 items for general information and 38 items for the construction needs Kano attribute questionnaire section, totally 55 items, and the sample size was calculated based on 5–10 times the number of questionnaire items, which was initially estimated to be 275–550. To account for the possible presence of invalid questionnaires, such as missing or incorrectly completed questionnaires, the sample size was increased by a further 10%, and the sample size was expected to be between 297 and 605. A total of 661 questionnaires were recovered, excluding 55 questionnaires that were completed in a short period of time, answered incompletely or appeared with a certain regularity, and 606 valid questionnaires, with an effective recovery rate of 91.68%, and the actual sample size of the survey was in line with the requirements. Methods: Kano model In 1984, Professor Noriaki Kano of Japan proposed the theoretical model of 'Must-be Quality - Charming Quality', also known as Kano's Needs Theory Model[ 5 ]. This model categorises demand attributes into six types: Must-be Quality (M), One-dimensional Quality (O), Attractive Quality (A), Indifferent Quality (I), Reverse Quality (R), and Questionable Quality (Q). Questionnaires designed based on the Kano model consist of pairs of questions. Each entry is paired with five answers in both forward and reverse directions, resulting in 5x5 possible combinations of responses. These responses are then classified according to the Kano model demand attributes (refer to Table. 1). The attribute with the highest frequency of occurrence is considered the Kano attribute of the demand. It consists of two parts: ① General information, which includes medical staff's age, gender, academic qualifications, degree, title, years of working experience, type of practice, type level of the hospital in which they work, ethical training and learning, conducting scientific research, and the medical staff's understanding of the REC and the construction of the hoped-for Main department. The Kano Attribute Questionnaire was designed to assess the construction needs of RECs. It consists of 9 dimensions and 38 items, each of which is scored positively or negatively. The response options are 'satisfied', 'deserved', 'indifferent', 'acceptable', and 'unsatisfied', with scores ranging from 1 to 5, as shown in Table. 2. The questionnaire achieved a Cronbach's alpha coefficient of 0.847 after testing. Investigation methods The group converted the questionnaire into an electronic format and obtained consent from the dean in charge of science and education or the head of the medical department of each primary hospital. They then distributed the QR code of the questionnaire via WeChat to the medical staff of the unit, who participated in the electronic survey by scanning the QR code. The questionnaire was designed with inter-question correlation logic and limitations on the number of responses. Respondents were given a time limit of 30 minutes and were not allowed to fill out the questionnaire more than once. The research group reviewed all the completed questionnaires and excluded those with incomplete answers or confusing logic. Statistical analysis The electronic questionnaire's raw data was imported into the SPSS database and analysed using SPSS 20.0 statistical software. The frequency and percentage were used to express the count data, while the measurement data were presented as mean ± standard deviation (± s). The Kano Attribute Questionnaire was used to determine the final attribute by counting the composition ratios of M, O, A, I, R, and Q for each item according to Table. 1. The Better-worse coefficient was then calculated, and the matrix quadrant diagram of demand was plotted. The Better-worse coefficient indicates the degree of impact on importance and satisfaction when an item is provided or not provided. The Better coefficient or degree of satisfaction is calculated as (A + O)/(A + O + M + I). The value is positive and represents the user's satisfaction with the provision of this feature. The Worse coefficient or degree of dissatisfaction is calculated as (M + O)/(A + O + M + I). The value is negative and represents the non-provision of this function, which decreases the user satisfaction rate. Results Basic information of the participants The primary health care workers' ages ranged from 22 to 68 years old, with an average age of 39.40 ± 8.71 years. Of the 606 workers, 487 (80.36%) were women, and 492 (81.19%) held a bachelor's degree or higher. Additional information is available in Table. 3. Additionally, Table. 5 indicate 68.15% (413) believed that the district healthcare committee should be the main body of RECs or that they should be established in second-level and higher general hospitals in the district after evaluation by the district healthcare committee. Importance-Satisfaction matrix The 38 entries related to construction demand were divided into four quadrants based on their respective satisfaction and importance levels. The resulting importance-satisfaction matrix was then plotted. There are 19 competitive advantage zones, numbered D1, E1, E2, E3, E4, F1, F2, G1, G2, G3, G4, G5, G6, G7, G8, G9, G10, H1, and H2. Additionally, there are 8 improvement zones, numbered C1, C2, C3, C4, G11, H3, I1, and I2, as well as 7 minor improvement zones, numbered D2, F3, F4, G12, I2, and I1. It is important to note that the improved text maintains the same content as the original text. Figure. 1 shows the maintenance zones (A1, A2, B1, and B2), the primary improvement zones (C1, C2, C3, C4, G11, H3, I1, and I2), and the secondary improvement zones (D2, F3, F4, G12, G13, G14, and G15), each numbered accordingly. Discussion Overall performance The study results indicate that out of the 38 construction contents, 30 of them are in demand by primary medical personnel, with most of the service demand attributes being higher. This reflects the universal demand of primary medical personnel for the construction of the REC. Currently, Chinese medical journals are placing greater emphasis on biomedical research ethics. Most journals require authors to provide proof of medical ethical review. According to this study, 61.88% of medical staff published at least one paper in the last five years, with 38.12% publishing more than two papers. The survey indicates the lack of ethical review in primary hospitals to meet the requirements of medical personnel.80.53% of the medical staff believed that ethics training was necessary. This highlights the high demand for platforms or institutions, such as RECs, that can provide ethical review, counselling, and training for clinical research in a specific region. Hierarchical Analysis of the needs of primary health care professionals for REC building Must-be requirements Must-be requirements are essential needs. Optimising these needs will not increase user satisfaction, but failing to meet them will significantly decrease it[ 6 ]. This study identifies four essential needs for building RECs by primary healthcare workers: the development of standard operating procedures for ethical review, the establishment of fees for ethical review, the hiring of independent consultants, and the establishment of an expert pool of independent consultants. The items' content strongly guarantees the effective operation of RECs and aligns with the basic normative requirements for constructing ethics committees. When building RECs, it is important to consider relevant practices both domestically and internationally. It is also crucial to take into account the specific situation of the region in order to formulate detailed standard operating procedures for RECs, establish appropriate fee standards, and appoint expert members. One-dimensional requirements One-dimensional requirements refer to the items that primary care staff expect from the REC or the level of service it can provide. This type of demand is positively related to user satisfaction. The more items provided or the better the service, the higher the user satisfaction. When the service is not provided or does not reach a certain level, the user satisfaction decreases[ 7 ]. The study results indicate that the construction of RECs requires 19 attributes across four dimensions of ethical review. These dimensions include providing ethical review, conducting scientific research projects, writing and publishing academic papers, conducting initial ethical reviews of new technologies and reviewing trial protocols, recruiting and protecting subjects, obtaining informed consent, and providing ethical consulting and argumentation before, during, and after project establishment. We offer ethical counselling and argumentation services before, during, and after project establishment. This is supported by the analysis of the importance-satisfaction matrix diagram, as depicted in Fig. 1 . All 19 items are located in the dominant zone, and satisfaction greatly increases once they are fulfilled. This is basically consistent with the functional positioning and review scope of RECs established in recent years in various provinces and cities in China[ 8 ]. Therefore, when building RECs, attention should be paid to the improvement of such needs. Attractive requirements Attractive requirements are those that go beyond expectations. If these demands are not met, user satisfaction will not decrease. However, if they are met, user satisfaction will increase significantly[ 9 ]. The study identifies seven areas of charismatic demand, including the construction of an information technology system (for reviewing applications, querying results, and communication), consultation on the design of medical and life sciences research protocols, and ethics training (through the establishment of a series of courses and popularisation training). These areas are located in the importance-satisfaction matrix and have little impact on the degree of importance but a significant impact on the degree of satisfaction. In recent years, institutions have developed ethical online review systems and information platforms for ethical management in hospitals. The use of information technology has proven to be powerful in ethical quality management, file traceability, information statistics, teleconferencing, ethical training, and consultation[ 10 , 11 ]. Research papers published in primary care institutions are increasing in number, but their quality needs improvement. Besides, the overall scientific research strength of these papers is weak. The experience of the REC of Beijing Municipal Hospital of Traditional Chinese Medicine demonstrates that effective communication and exchange mechanisms within the REC can gradually enhance the clinical scientific research capacity and ethical review level of medical institutions affiliated with the Traditional Chinese Medicine medical association [ 12 ]. Therefore, it is recommended to develop an electronic management system suitable for the operation of RECs, including online review functions and file management. This will enable the electronic operation of ethical review affairs and provide services such as consulting on research protocol design and ethical training. Such a system will help to fully utilise the role of RECs. Indifferent requirements Indifferent requirements are characterized by irrelevance, and the provision or non-provision of this attribute has little effect on increasing or decreasing satisfaction[ 13 ]. Only eight services in this study showed undifferentiated needs, which suggests that the overall need for REC building among primary healthcare workers in this study is universal. This attribute type is primarily located in the secondary improvement and maintenance zones of the importance-satisfaction matrix diagram. It has little effect on satisfaction but is more important to primary healthcare workers. According to Kano, users' attitudes towards the same attribute may change over time, and as a result, the Kano category to which the service content belongs may also change[ 14 ]. Therefore, when establishing RECs, it is important to consider projects with undifferentiated needs and optimize construction projects from a practical perspective to best meet the needs of primary medical personnel. Limitations The sample size of this survey may not be sufficient, which could result in a lack of representation of primary medical staff. Additionally, the comparison between primary and senior hospitals needs clarification, and 3rd level hospital staff should be included in this study to demonstrate the necessity of RECs. In summary, the number of participants should be increased, as well as their diversity. Conclusion The survey results indicate that 77.23% of primary medical staff in a district of Shanghai possess knowledge of RECs. Additionally, 68.15% of primary medical staff believe that the district health care committee should be responsible for constructing the RECs or that they should be established in second-level and above general hospitals in the district after evaluation by the district health care committee. Out of the 38 construction demands, 4 were mandatory attributes, 19 were preferred attributes, 7 were superficial attributes, and 8 were undifferentiated attributes. These demand attributes are significant in guiding the subsequent practice of REC construction. For demand items located in the advantageous and to-be-improved zones of the importance-satisfaction matrix, priority should be given to meeting and improving the two types of demands that can attract grass-roots medical personnel the most in future construction. The attributes of the demands in the construction of the RECs can be benchmarked by the relevant departments. The findings of this study will serve as a reference for the future construction of the committee.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eMethods: \u003c/strong\u003eUsing a questionnaire based on the Kano model to conduct the survey on 606 grassroots medical personnel in Shanghai to investigate their needs for the construction of RECs.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults: \u003c/strong\u003eAmong them, 77.23% reported having knowledge of the RECs, 68.15% of respondents believed that the regional health administrative department or a level 2 or above comprehensive medical hospital recognized by it after evaluation should be responsible for constructing the RECs. Out of the 38 construction demand survey projects, 4 are Must-be Quality, 19 are One-dimensional Quality, 7 are Attractive Quality, and 8 are Indifferent Quality. The quadrant matrix diagram shows that 19 requirements are in the advantage area, 8 requirements are in the area that needs improvement, 7 are in the secondary improvement area, and 4 are in the maintenance area. No reverse or problematic attributes were found.\u0026nbsp;\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eConclusion: \u003c/strong\u003eIn the construction of RECs, relevant departments can use the attributes of the needs as benchmarks and promote construction in a hierarchical and phased manner.
