Determinants of Oral Pathogen Prevalence in North-East Nigeria: A Cross-Sectional Study

Abstract

Abstract Background Oral diseases remain a major public health problem globally, with limited data available from North-East Nigeria. This study assessed the prevalence of oral pathogens and their risk factors among patients in North-East Nigeria. Methods A cross-sectional study was conducted among 303 patients with oral health complaints in selected health facilities. Data were collected using structured questionnaires. Oral swabs were processed for pathogens using standard microbiological techniques. Statistical analyses were performed using chi-square tests and logistic regression to identify predictors of infection, with p < 0.05 considered significant. Results Overall, 37.29% of participants were positive for at least one oral pathogen. The most prevalent organisms were Staphylococcus aureus (16.17%) and Streptococcus mutans (12.21%). Females (21.8%) and urban residents (23.1%) exhibited significantly higher prevalence than males (15.5%) and rural residents (14.2%). Brushing frequency and flossing were significantly associated with lower infection risk. Conclusion The study revealed a high prevalence of oral pathogens among patients in North-East Nigeria, with S. aureus and S. mutans as the predominant isolates. Poor oral hygiene practices, female gender, and residence were key determinants. Strengthening oral health education, promoting regular tooth brushing and flossing, and improving access to preventive dental services could help in reducing the burden of oral infections in the region. Caries Flossing Oral hygiene Dental Gum bleeding Figures Figure 1 INTRODUCTION The human oral cavity harbors one of the most diverse microbial ecosystems in the body, with more than 700 species of bacteria, fungi, and viruses identified to date [ 1 ]. Under healthy conditions, these microorganisms exist in balance with the host, contributing to oral and systemic health [ 2 ]. However, disturbances in this balance driven by poor hygiene, dietary practices, or host susceptibility can lead to dysbiosis, allowing pathogenic organisms to dominate [ 2 ]. Such microbial imbalances are implicated in common oral conditions such as dental caries, gingivitis, and periodontitis, and have also been linked to systemic diseases including diabetes, cardiovascular disease, and respiratory infections [ 1 , 3 ]. Globally, oral diseases affect an estimated 3.5 billion people, making them among the most prevalent non-communicable diseases [ 4 ]. In Nigeria, the burden is considerable. Dental caries prevalence among adults ranges from 4% to 40%, with untreated lesions accounting for nearly 90% of cases [ 5 ]. Among children, early childhood caries is reported in about 17% (95% CI: 11–24%), highlighting that the problem cuts across age groups [ 6 ]. The dietary and life style lead increase in dental problems [ 7 ]. Also, poor oral hygiene practices are a key driver of this burden. In rural Edo and Delta States of Nigeria, over one-third of residents demonstrated poor oral hygiene, and nearly three-quarters brushed only once daily, while 94% had never visited a dentist [ 8 ]. National surveys further show that only 8.9% of adolescents practice good oral hygiene, with irregular tooth brushing, low flossing rates, and high consumption of sugary snacks being common [ 9 ]. Among pregnant women oral care practices are inconsistent: while toothbrush and toothpaste use is common, twice-daily brushing remains low, and traditional cleaning methods such as chewing sticks persist in up to 27.7% of cases [ 10 ]. Specific pathogens are strongly influenced by hygiene behaviors and host factors. For instance, Streptococcus mutans is closely associated with caries in individuals with high sugar intake and poor hygiene, while Candida albicans thrives in immunocompromised hosts and those with inadequate oral care [ 1 ]. These patterns highlight the dual role of hygiene practices and underlying risk factors in shaping disease outcomes. Beyond biological determinants, cultural and socioeconomic contexts shape oral health in Nigeria. Cultural beliefs and norms around oral care continue to influence hygiene behaviors, sometimes undermining modern preventive practices [ 11 ]. Moreover, structural barriers including; limited access to dental services, poverty, and low awareness contribute to sustained high prevalence rates in underserved regions such as North-East Nigeria. Despite the growing recognition of oral health as a public health priority, there is limited data on the prevalence of oral pathogens, their risk factors, and the role of hygiene practices in North-East Nigeria. This study therefore aimed to fill this gap by determining the prevalence of oral pathogens in the region, identifying associated risk factors, and evaluating the impact of oral hygiene practices. The findings provided locally relevant evidence to guide preventive strategies, strengthen oral health policies, and support culturally grounded interventions to improve oral health outcomes. METHODOLOGY Study Area and Design This cross-sectional study was conducted in two selected states of North-Eastern Nigeria (Fig. 1 ): Bauchi and Gombe. These states were chosen to provide a representative overview of oral health challenges in the region, which is characterized by limited dental health infrastructure, diverse sociocultural practices, and high burden of communicable diseases (Fig. 1 ). Study Population The study population comprised patients of all ages and genders who presented at selected health facilities with oral issues in the study states during the study period (March to August, 2025). Inclusion criteria were individuals who consented to participate and provided oral samples for microbiological analysis. Patients who were critically ill, not willing to participate or unable to provide samples were excluded. Sample Size Determination The sample size was calculated using the standard single-proportion sample size formula: n = \(\:\frac{{(\text{Z}\:{\alpha\:}/2)}^{2}\:\text{X}\:\text{P}(1-\:\text{P})}{{\text{d}}^{2}}\) Where: p = 0.23 (23%) [ 5 ] d = 0.05 (5% precision) \(\:\text{Z}\:{\alpha\:}/2\) = 1.96 Therefore, n = \(\:\frac{{\left(1.96\right)}^{2}\:\text{X}\:0.23(1-\:0.23)}{{0.05}^{2}}\) The required sample size is therefore 272. The sample size was adjusted by 10% to achieve the required precision, power and confidence level. Therefore, 303 patients with oral issues at the selected centers were enrolled in the study. Ethical Considerations Ethical approval was obtained from the Research and Ethics Committees of the Federal Medical Centers Gombe with reference numbers NHREC/25/10/2013 dated 3rd March, 2025, and Extreme Hospital Azare Bauchi State (reference numbers; EXHA/02/25/033). Written informed consent was obtained from adult participants, and from the parents or guardians of children below 16 years of age, before enrolment. Assent was also obtained from children aged seven years and above. Confidentiality of participants’ information was ensured, and all procedures were conducted in accordance with the Declaration of Helsinki (2013). Data Collection Socio-demographic information and oral hygiene practices were collected using a structured questionnaire developed by the research team (Supplementary File 1), administered through face-to-face interviews. The questionnaire captured variables such as age, gender, residence, oral hygiene habits (frequency of brushing, use of toothpaste, flossing, and traditional methods), dietary habits, and dental care utilization. Sample Collection and Laboratory Analysis Oral swab samples were aseptically collected from participants using sterile swab sticks. Samples were transported in appropriate transport media to the microbiology laboratories of the respective Medical Centers within each state for microbiological analysis. For bacterial pathogens, samples were inoculated on selective and differential media, including blood agar, MacConkey agar, and chocolate agar, and incubated at 37°C for 18–24 hours as described by Cheesbrough [ 12 ]. Colonies were identified based on morphological characteristics, Gram staining, and standard biochemical tests, as demonstrated by Cheesbrough [ 12 ]. For fungal pathogens, samples were cultured on Sabouraud dextrose agar (SDA) supplemented with chloramphenicol and incubated at 30°C for up to 7 days. Identification of fungal isolates was carried out using colony morphology, lactophenol cotton blue staining, and, where applicable, germ tube testing and sub-culturing on Chromagar Candida for Candida albicans and others [ 13 ]. Data Analysis Data were entered into SPSS version 23.0 (IBM Corp., Armonk, NY, USA) for statistical analysis. Descriptive statistics were used to summarize demographic characteristics, oral hygiene practices, and prevalence of pathogens. Associations between risk factors and occurrence of oral pathogens were assessed using chi-square tests, and multivariable logistic regression was applied to identify independent predictors. A p -value < 0.05 was considered statistically significant. RESULTS Prevalence of Oral Pathogens Out of the 303 oral samples analyzed, 113 (37.29%; 95% CI: 30.6–41.4) were positive for at least one pathogen (Table 1 ). Staphylococcus aureus was the most frequently isolated organism (16.17%), followed by Streptococcus mutans (12.21%), Candida albicans (4.62%), and Staphylococcus epidermidis (4.29%). Demographic Characteristics and Prevalence of Oral Pathogens Table 2 presents prevalence stratified by demographics. Infection rates were significantly higher among adolescents aged 11–19 years (17.2%) and adults aged 25–64 years (17.2%) compared with younger children (χ² = 13.882, p = 0.003). Females exhibited a significantly higher prevalence (21.8%) than males (15.5%) (χ² = 46.367, p < 0.001). Similarly, individuals residing in urban areas (23.1%) had a significantly higher prevalence than rural dwellers (14.2%) (χ² = 46.367, p < 0.001). Oral Hygiene Practices and occurrence of oral pathogens As shown in Table 3 , brushing frequency was significantly associated with infection status (χ² = 11.744, p = 0.003). Those who brushed more than twice daily had the lowest proportion of infections. Flossing practice also showed significant association (χ² = 8.307, p = 0.016), with daily flossers recording least positive cases. In contrast, type of cleaning material, toothbrush replacement, and mouth rinsing after meals did not show statistically significant associations ( p > 0.05). Lifestyle, Risk Factors and Occurrence of Oral Pathogens Lifestyle habits such as consumption of sugary snacks/drinks, smoking, alcohol intake, sharing toothbrushes, and wearing dental appliances were not significantly associated with occurrence of oral pathogens (Table 4 ). Oral Health History and Occurrence of Oral Pathogens Table 5 indicates that persistent bad breath (χ² = 18.263, p < 0.001) and presence of mouth sores/ulcers (χ² = 7.511, p = 0.006) were significantly associated with occurrence of oral pathogens. Other conditions, including gum bleeding, showed no statistically significant associations. Access to Care, Health Status and occurrence of oral pathogens As presented in Table 6 , recent antibiotic use was strongly associated with low prevalence of oral pathogens (χ² = 10.932, p = 0.001). Infections were also more prevalent among individuals with a history of chronic illness (χ² = 4.996, p = 0.025). Frequency of dental visits and use of mouthwash showed no significant associations. Predictors of Oral Pathogens Bivariate and multivariate analysis identified several independent predictors (Table 7 ). Female gender was a strong predictor (OR = 5.62; 95% CI: 3.35–9.41; p < 0.001). Rural residence was associated with a two-fold higher risk (OR = 2.10; 95% CI: 1.26–3.50; p = 0.004). Other significant predictors included recent antibiotic use (OR = 0.44; 95% CI: 0.27–0.72; p = 0.001), chronic illness (OR = 0.19; 95% CI: 0.04–0.96; p = 0.044), poor flossing practice (OR = 0.44; 95% CI: 0.25–0.77; p = 0.004), persistent bad breath (OR = 0.27; 95% CI: 0.14–0.50; p < 0.001), and mouth sores/ulcers (OR = 0.46; 95% CI: 0.26–0.81; p = 0.007). DISCUSSION This study looked at 303 patients with oral infections, visiting the selected health centers in two states in the North-East of Nigeria. The study found that 37.29% (113 out of 303) had at least one type of pathogenic bacteria or fungi in their mouths. The most common bacteria found were Staphylococcus aureus (16.17%) and Streptococcus mutans (12.21%), while Candida albicans was found in 4.62% of the cases. The rate of 37.29% seen in this group matches the high number of oral health issues reported worldwide. It is estimated that around 3.5 billion people are affected by oral diseases, which are among the most common non-communicable diseases [ 14 ]. The World Health Organization's Global Oral Health Status Report highlights how widespread these preventable oral health issues are and stresses the need for better monitoring and prevention efforts [ 14 , 15 ]. The findings of our study also supported the report of Obayemi et al. [ 5 ] in which a prevalence range of 4% to 40% was seen among adult Nigerians. Our study found that S. aureus and S. mutans were the most common bacteria associated with oral infections, which is somewhat different from older research that focuses mainly on caries and identifies S. mutans and other related bacteria as the main ones causing tooth decay. However, this finding matches recent studies from Nigeria and other nearby areas, which have shown that S. aureus is often found in the mouths of people in both communities and clinics [ 16 , 5 ]. Local factors like how people take care of their teeth, use of antibiotics, and the chance of certain bacteria growing in people with other health problems might explain why S. aureus is more common in some groups. There are several possible reasons why Staphylococcus aureus was found so often: it can come from the skin or nose and end up in the mouth, or previous use of antibiotics may have allowed resistant strains of S. aureus to grow. Other studies have shown that the mouth can be a place where S. aureus lives, including strains that are resistant to some antibiotics. A detailed analysis showed that being female (prevalence = 21.78%, χ² = 46.367, p = 0.000), aged 10–19 or 25–64 years (prevalence = 17.16%, χ² = 13.882, p = 0.003), living in a urban area (prevalence = 23.10%, χ² = 46.367, p = 0.000), were key factors linked to high prevalence of these harmful bacteria and fungi. This pattern is similar to reviews from Nigeria that show more tooth decay and related bacteria as people get older, likely because of long-term exposure to foods that cause tooth decay and less effective oral hygiene habits over time [ 6 ]. Also, in our study, women and people living in cities had higher rates of these bacteria, which is different from some national reports that have found either no difference or higher risks for people in rural areas. Folayan et al . [ 10 ] reported inconsistent oral care practices among pregnant women in Nigeria. This might be the reason for the high prevalence of the pathogens observed among women in this study. Also, the high prevalence observed among adolescents and adults might be due poor oral hygiene or dietary habit of consuming sugary foods. Frequent tooth brushing (χ² = 11.744, p = 0.003), and regular flossing (χ² = 8.307, p = 0.016) seem to help protect against dental problems, as shown by other studies that look at many factors. This supports the idea that keeping mouth clean reduces the amount of harmful bacteria and lowers the risk of diseases. The findings of Folayan et al. , [ 9 ] corroborate with our findings as they reported irregular tooth brushing, low flossing rates, and high consumption of sugary snacks among adolescent Nigerians with oral issues. Although some research says that cleaning between teeth only gives small benefits for gum health, doing both brushing and flossing together likely removes places where harmful bacteria can grow, which matches what we found [ 17 ]. Things like bad breath and mouth sores can also be signs of poor oral hygiene. Conclusion This study highlights a substantial burden of oral pathogens among patients in North-east Nigeria, with over one-third of participants harboring at least one pathogenic organism. Staphylococcus aureus and Streptococcus mutans were the predominant isolates, underscoring the dual importance of both opportunistic and cariogenic pathogens in oral infections. Significant associations with gender, residence, oral hygiene practices, and indicators such as persistent bad breath and mouth ulcers emphasize the multifactorial nature of oral health risks in the region. Importantly, frequent brushing and regular flossing were protective, supporting the need for intensified oral health promotion campaigns. The findings call for community-based interventions, improved access to dental care, and integration of oral health into broader public health strategies. Females (21.8%) and urban residents (23.1%) exhibited significantly higher prevalence than males (15.5%) and rural residents (14.2%).
