Methods We conducted a cohort study, retrospectively reviewing records of 1,291 pregnant women on ART. Descriptive statistics summarised demographics and clinical characteristics. Chi-square, Fisher’s exact, Generalised Estimating Equations and logistic regression assessed variations in viral-suppression across the maternal timeline (p < 0.05). Results ART regimen distribution was: DTG (62.5%), EFV (28.8%), NVP (4.5%), and PI (4.2%). Regimens varied in effectiveness before and after (p = 0.000, p = 0.018) but not during pregnancy. Longitudinal risk of non-suppression was noted for; poor adherence (aIRR = 8.48, CI: 1.82–39.43, p = 0.000) and third line (aIRR = 8.48, CI: 1.82–39.43, p = 0.006). Increased non-suppression odds were observed; before pregnancy, PI regimens (OR: 6.49, CI: 2.55–16.53, p = 0.000), third line (OR: 26.83, CI: 1.64–439.36, p = 0.021), poor adherence (OR: 19.64, CI: 2.70–142.68, p = 0.003). During pregnancy, third line (aOR: 51.75, CI: 1.62–1651.93, p = 0.026), and poor adherence (aOR: 26.84, CI: 1.56–461.08, p = 0.023). After pregnancy, PI regimens (OR: 3.78, CI: 1.35–10.59, p = 0.011), and third line (OR: 21.57, CI: 1.32–351.61, p = 0.03, aOR: 51.75, CI:1.62-1651.93, p = 0.026). Additional predictors included BMI, ART Duration, second line regimen and age. Conclusion ART effectiveness varied before and after but not during pregnancy. PI regimens were linked to non-suppression, while EFV, NVP, and DTG showed similar suppression across maternal timelines. Non-suppression was driven by poor adherence, advanced regimens, ART duration, nutrition, and age, highlighting the need for targeted support. Introduction Viral-suppression is a key indicator of antiretroviral therapy (ART) effectiveness and is particularly crucial in the prevention of mother-to-child transmission (PMTCT) of HIV[ 1 – 3 ]. While ART has significantly improved maternal and infant health outcomes, pregnancy-related physiological changes can alter ART pharmacokinetics[ 4 , 5 ], potentially leading to suboptimal viral suppression[ 6 ]. This, in turn, increases the risk of virologic failure, which may be influenced by factors such as ART regimen choice, adherence, drug resistance, and the complexities of managing HIV during pregnancy and postpartum[ 7 – 9 ]. Uganda's ART regimens have evolved to improve treatment outcomes. Prior to 2010, the standard first-line regimen included zidovudine (AZT), lamivudine (3TC), and nevirapine (NVP). Due to concerns about toxicity and resistance, it was replaced with tenofovir (TDF), 3TC, and NVP. By 2010, the preferred regimen shifted to TDF/3TC/efavirenz (EFV), and with the introduction of dolutegravir (DTG) in 2013, TDF/3TC/DTG was adopted as the WHO-recommended first-line regimen in 2016. This was on the basis that this regimen had shown superior viral suppression and a high genetic barrier to resistance. However, in instances of DTG unavailability or drug interactions, EFV 400 mg/day with TDF and 3TC is an alternative[ 3 , 10 , 11 ]. While both TDF/3TC/DTG and TDF/3TC/EFV are effective in viral suppression, data on their longitudinal performance during the maternal timeline in resource-constrained settings like Uganda remain limited. Understanding their impact on maternal viral load suppression is vital for improving maternal and child health outcomes[ 10 ]. Given that ART prescription follows a public health approach, utilizing real-world data from HIV program implementation enhances translational research to refine HIV treatment protocols. This study assessed viral load suppression throughout the maternal timeline among women on different ART regimens at Mildmay Uganda PMTCT clinic. The findings aim to inform healthcare providers and policymakers on optimizing maternal HIV treatment strategies. This research supports efforts to reduce mother-to-child transmission, improve maternal and infant health outcomes, and contributes to global targets for achieving HIV epidemic control by 2030. Methods and materials Study Design This cohort study retrospectively reviewed medical records of patients enrolled in the PMTCT clinic at the Mildmay Uganda Hospital (MUgH) from 2018 to 2023. Study site and population This study was conducted at MUgH, a peri-urban facility in Wakiso District with 50-bed capacity, providing integrated TB-HIV care and ART to nearly 15,000 active HIV patients[ 12 ]. The study population included pregnant and breastfeeding mothers registered at the PMTCT clinic between 2018 and 2023. Data Collection and Management Data collection and participant recruitment were conducted over a three-month period (April to June 2024). Data on participants' socio-demographic characteristics and medical history were extracted from PMTCT registers, patient cards, and Electronic Medical Records (EMR). Data entry was performed using Microsoft Excel, with regular accuracy checks against hard copy records to ensure data integrity. A flowchart illustrates the data collection process. Statistical Analysis Statistical analyses were performed using Stata 16. Descriptive statistics summarized demographic and clinical characteristics as frequencies and percentages. Categorical variables were compared using the Chi-square or Fisher’s exact test as appropriate. Viral load suppression in copies/mL was categorized into three groups: suppressed (≤ 200), low viremia (201–999), and high viremia (≥ 1000)[ 3 ] and later reclassified as suppressed (≤ 200) and unsuppressed (≥ 201) while assessing associational factors. In the descriptive analysis, 1,291 participants were included. To compare viral suppression before and during pregnancy, 1,248 participants were analyzed. While 941 participants were considered for longitudinal analysis using Generalized estimating equations (GEE). GEE assessed ART regimens' effect on viral suppression over time and logistic regression examined the impact of selected variables on viral suppression across the maternal timeline. Overall statistical significance was set at p < 0.05. Ethical Considerations Ethical approval was obtained from the Mildmay Uganda Research Ethics Committee (#REC REF 0201–2024) and the Uganda National Council for Science and Technology (HS3873ES). A waiver of consent was granted for using de-identified retrospective data. Results Table 1 summarises the sociodemographic characteristics of 1,291participants. Majority were on DTG (807, 62.5%), followed by EFV (372, 28.8%), NVP (58, 4.5%), and PI (54, 4.2%). DTG was predominant in ages 25–34 (466, 63.9%), while EFV was common in > 35 years (125, 32.2%). Normal BMI participants mostly used DTG (338, 63.3%) and EFV (149, 28.6%) (p = 0.449). WHO Stage 1 participants mainly used DTG (761, 62.4%), while stage 3 had more on EFV (5, 55.6%). First-line ART was most common (1,175, 91.1%), mainly DTG (743, 63.2%). All other characteristics were significant (p < 0.05) except age, BMI, and ART experience. The EFV-based regimen had suppression rates of 94.89%, 94.24%, and 93.59%, respectively. NVP-based showed 93.10%, 94.64%, and 93.48%, while PI-based had the lowest suppression at 79.63%, 88.00%, and 85.71%. Statistical significance was observed before (p = 0.000) and after pregnancy (p = 0.018) time points, not during pregnancy (p = 0.678). Longitudinal analysis of viral suppression across maternal timelines In the unadjusted model, PI-based regimens were significantly associated with higher risk of viral non-suppression (IRR = 3.20, 95% CI: 1.63–6.30, p = 0.001), but this lost significance in the adjusted model. In adjusted models, poor adherence (aIRR = 8.48, 95% CI: 1.82–39.43, p = 0.000), fair adherence (aIRR = 5.03, 95% CI: 1.11–22.86, p = 0.036), second-line ART (aIRR = 3.14, 95% CI: 1.75–5.62, p = 0.000), and third-line ART (aIRR = 8.48, 95% CI: 1.82–39.43, p = 0.006) were significant predictors of non-suppression. During Pregnancy : In unadjusted models, third-line ART (OR: 18.88, 95% CI: 1.16–307.13, p = 0.039), second-line ART (OR: 3.72, 95% CI: 1.90–7.27, p = 0.000), poor adherence (OR: 49.38, 95% CI: 5.05–483.02, p < 0.05), and ART duration of 1–2 years (OR: 4.53, 95% CI: 1.80–11.43, p = 0.001) were linked to non-suppression. Obesity (OR: 0.08, 95% CI: 0.53–0.12, p = 0.000) was protective. In adjusted models, third-line ART (aOR: 51.75, 95% CI: 1.62–1651.93, p = 0.026), second-line ART (aOR: 4.45, 95% CI: 1.65–12.13, p = 0.003), and poor adherence (aOR: 26.84, 95% CI: 1.56–461.08, p = 0.023) remained significant. After Pregnancy : In unadjusted models, PI-based regimens (OR: 3.78, 95% CI: 1.35–10.59, p = 0.011), third-line ART (OR: 21.57, 95% CI: 1.32–351.61, p = 0.031), second-line ART (OR: 21.57, 95% CI: 1.32–351.61, p = 0.031), and underweight status factor (OR: 3.38, 95% CI: 1.33–8.60, p = 0.011) were associated with non-suppression. In adjusted models, only second- and third-line ART remained significant, while women aged 25–34 years showed a protective effect (aOR: 0.18, 95% CI: 0.04–0.84, p = 0.029). Our study provides real-world evidence on the longitudinal patterns of viral-suppression among 1291 pregnant women living with HIV in Uganda and the impact of different ART regimens. The findings highlight key determinants of viral-suppression across maternal timelines emphasizing the role of ART regimen choice, adherence, and patient characteristics in influencing treatment success. There was variation in the effectiveness of ART regimens, before and after but not during pregnancy. This could be attributed to perceived risk of non-suppression to the unborn child that potentially increases adherence irrespective of regimen type[ 13 ]. This highlights the need for regimen consistency during pregnancy. PI-based regimens were associated with higher odds of non-suppression in unadjusted models, though these associations lost significance in multivariate analysis. This may reflect confounding by adherence due to pill burden or prior treatment failure, as PI regimens are often prescribed in complex treatment scenarios. Programmatically, PI-based regimens are primarily prescribed as second-line therapy, which may explain their association with poorer treatment outcomes. Patients on these regimens are likely to have a history of virologic failure or drug resistance, necessitating closer clinical monitoring and strengthened adherence support. Consistently, second- and third-line ART regimens were associated with higher odds and increased risk of viral non-suppression, particularly before and during pregnancy, even after adjusting for potential confounders. Contrary to WHO recommendations promoting DTG-based regimens for their superior efficacy [ 3 , 14 – 20 ], our analysis did not find significant differences in suppression between DTG and other regimens except for PI based regimens across the maternal timeline. This suggests that treatment success depends on more than regimen choice. Factors such as treatment fatigue, long-term toxicity from older regimens[ 21 , 22 ], and emerging resistance may reduce efficacy over time, highlighting the need for enhanced adherence support, resistance monitoring, and strategies to address ART-related toxicities. Adherence emerged as a strong predictor of suppression. Poor adherence significantly increased non-suppression odds before and during pregnancy, even in adjusted models. Longitudinally, adherence had the strongest impact on incidence rates. Pregnancy-related challenges, such as nausea, stigma, and increased healthcare demands, may hinder adherence[ 23 , 24 ], emphasizing the need for targeted counselling and support programs tailored to pregnant women. Young women (15–24 years) had higher odds of non-suppression before pregnancy in unadjusted models, possibly due to lower ART literacy, stigma, or competing priorities[ 25 – 27 ], though this effect was not significant after adjustment. Women aged 25–34 years showed a protective effect postpartum, likely due to greater maturity and improved healthcare engagement[ 28 ]. However, age was not significant in longitudinal analysis, suggesting that adherence and psychosocial support may be more critical than age alone. A history of ART regimen change was protective during pregnancy, likely due to improved suppression after switching to more effective regimens[ 14 ], aligning with global recommendations promoting regimen optimization. However, this effect was not sustained in adjusted or longitudinal analyses, suggesting that prior treatment failure, or resistance may impact long-term suppression[ 29 ]. Additionally, regimen changes often follow treatment failure, potentially introducing residual effects that weaken observed benefits over time[ 30 ]. BMI significantly influenced viral-suppression, with underweight women (BMI < 18.5) showing higher odds of non-suppression before and after pregnancy, possibly due to poor nutritional status or advanced HIV disease, which can impair treatment response [ 31 ]. Conversely, obesity (BMI > 30) appeared protective during pregnancy. However, these associations did not persist in multivariate or longitudinal analyses, suggesting confounding by factors such as immune function, nutritional reserves, or disease severity. Participants in WHO stage 3 had higher odds of non-suppression before pregnancy, but this was not significant after adjustment, suggesting that while advanced disease may predict worse outcomes, factors like regimen potency, and immune recovery play a more significant role in long-term viral-suppression[ 32 ]. However, the small sample size in this category warrants cautious interpretation. Similarly, longer ART duration (> 5 years) was linked to non-suppression before pregnancy, possibly due to fatigue, resistance, or outdated regimens [ 14 , 21 , 22 ]. Conversely, shorter ART duration (1–2 years) increased non-suppression during pregnancy, reflecting early treatment challenges [ 33 ]. However, these associations also lost significance in multivariable and longitudinal analyses. Strengths and Limitations This study benefits from a large sample size, real-world clinical data, and robust methods, including GEE for repeated measures. Despite these strengths, limitations include missing data, self-reported adherence subject to bias, and residual confounding. Findings best apply to similar ART program settings. Programmatic Implications, Conclusion, and Recommendations Our study highlights that ART effectiveness varied before and after but not during pregnancy. PI-based regimens were associated with non-suppression while EFV-based, NVP-based, and DTG-based regimens exhibited comparable viral-suppression across all time points. Adherence, ART drug line regimes and drug resistance may play a more significant role in long-term outcomes than regimen choice alone. Viral non-suppression varied across the maternal timeline. Before pregnancy, risk factors included PI regimen, second- and third-line regimens, poor adherence, and age (15–24 years). During pregnancy, risk factors included poor adherence, ART duration of 1–2 years, and advanced regimens, while obesity was protective. Postpartum, second- and third-line regimens and underweight status increased odds, whereas age 25–34 was protective. These findings emphasize the need for timeline-specific interventions, optimizing ART, improving adherence, and addressing nutrition. Targeted support for younger women and those on advanced regimens is vital. Future research should investigate pharmacokinetics, pharmacogenetics, adherence strategies, and socio-behavioural factors to enhance maternal and neonatal outcomes.
