This case report describes the laparoscopic management of a 33-year-old woman presenting with primary infertility, hypermenorrhoea, and severe dysmenorrhoea. The patient had a history of unsuccessful IVF attempts due to external endometriosis and was found to have isolated adenomyosis of the posterior uterine fundus alongside a subserous myom. Surgeons performed a uterus-sparing resection using a laparoscopically modified Osada technique, removing the adenomyotic tissue in two blocks after opening the myometrium and repairing the defect with a double-layer suture reinforced by an anti-adhesion barrier. Postoperative imaging revealed small intramural hematomas, but the patient’s recovery was uneventful with patent fallopian tubes confirmed by chromopertubation. This paper is centrally about adenomyosis — specifically detailing a surgical approach for deep infiltrating adenomyosis of the uterine fundus while also addressing concurrent endometriosis.
