Despite their clinical importance, training in these conditions remains poorly defined across Europe. We aimed to assess self-perceived knowledge in biliopancreatic diseases among Spanish gastroenterology trainees and early-career specialists, and to identify training-related factors associated with higher competence. Methods A nationwide cross-sectional online survey was conducted among final-year residents and specialists completing residency between 2018 and 2024 under the auspices of the Spanish Association of Gastroenterology(AEG) and the Spanish Association of Pancreatology(AESPANC). Participants reported demographics and training characteristics. General self-perceived knowledge was rated on a 0–10 scale, and knowledge of 45 pancreatic and biliary conditions was assessed using a 5-point Likert scale. Results Of 216 respondents (60.5% women; mean age 32.3 years), 28.8% were residents and 71.2% specialists. Median general knowledge scores were 7/10 for both pancreatic and biliary diseases. Highest competence was reported in common conditions (acute pancreatitis, biliary colic, acute cholecystitis, cholangitis), lowest scores were observed for chronic pancreatitis, autoimmune pancreatitis, cystic fibrosis, pancreatic cystic lesions, and benign biliary disorders. Factors associated with higher knowledge included presence of pancreatology specialists, larger case volumes, outpatient clinic exposure, participation in multidisciplinary committees, structured teaching, and formal competency assessment. Conslusions: Training in biliopancreatic diseases among Spanish gastroenterology trainees is heterogeneous, with significant gaps in clinically relevant domains. Structured clinical exposure, mentorship, formal evaluation, and targeted teaching are key factor. These findings highlight the need to standardise and strengthen pancreatology training pathways to improve patient care. In many countries, including Spain, gastroenterology training programmes follow structured, competency-based frameworks to ensure that trainees acquire the knowledge and skills necessary for independent practice ( 4 ). However, despite the clinical relevance and growing complexity of these conditions, training exposure in biliopancreatic diseases remains poorly characterised across Europe ( 5 , 6 ). Such gaps in training may lead to variability in patient management and potentially affect clinical outcomes. Currently, no specific European or national guidelines address training standards in pancreatology or biliary disorders. The Blue Book 2023, published by the European Section and Board of Gastroenterology and Hepatology, outlines general competencies for gastroenterology specialists, recommending a basic understanding of biliopancreatic diseases without specifying training depth or procedural requirements ( 7 ). Similarly, the Spanish national gastroenterology curriculum provides general guidance on patient volumes and endoscopic procedures but does not prioritise biliopancreatic diseases according to prevalence or clinical relevance ( 8 ). Overall, evidence on pancreatology-specific training standards is extremely limited, with no tailored competency frameworks or procedural benchmarks. Little is known about trainees’ perceived preparedness or their real clinical and endoscopic exposure to pancreatic and biliary diseases during residency ( 9 – 12 ), and no prior study has systematically evaluated these aspects in adult biliopancreatology. We hypothesised that, in the absence of specific recommendations, gastroenterologists trained in Spain may receive heterogeneous education in biliopancreatic diseases, resulting in gaps in knowledge and confidence. The aims of this study were to assess self-perceived knowledge acquired during residency and to identify training-related factors associated with higher confidence in managing biliopancreatic diseases. Methods Participants We conducted a nationwide cross-sectional survey targeting final-year gastroenterology residents and early-career specialists who completed their residency between 2018 and 2024. The study and survey were conducted under the auspices of the Spanish Association of Gastroenterology (AEG) and the Spanish Association of Pancreatology (AESPANC). An anonymous, voluntary online questionnaire was administered through Google Forms, with data collection taking place between April and June 2025. Before accessing the survey, all participants were presented with an introductory information page describing the study objectives, voluntary nature of participation, confidentiality procedures, and estimated completion time. To minimise selection bias, all eligible members of AEG and AESPANC were invited via institutional mailing lists. Single-entry participation was enforced through Google Forms settings, and suspected duplicate submissions were excluded to ensure data integrity. The survey The survey questionnaire was specifically developed for this study by the authors. The questionnaire collected demographic information, including age, sex, professional category, and the region where residency training was being completed or had been completed. The full English version of the questionnaire is provided as Supplementary Material 1. Participants were asked about their personal interest in biliopancreatic diseases during residency and key training-related characteristics, including hospital type and the availability of pancreatology or biliary specialists at their centre. Additional items assessed clinical exposure (e.g., number of biliopancreatic patients evaluated, responsibility for common conditions), participation in structured educational activities (e.g., multidisciplinary committees, teaching sessions), and engagement in research or advanced training opportunities (e.g., external rotations or awareness of national/European courses). Finally, the survey included a dual self-assessment approach. Participants rated their perceived general knowledge of pancreatic and biliary diseases on a 0–10 scale. In addition, they evaluated their knowledge of 45 specific conditions (pancreatic (31) and biliary ( 14 )) using a 5-point Likert-type scale categorised as: none ( 1 ), low ( 2 ), moderate ( 3 ), high ( 4 ), and very high ( 5 ). These items were selected by domain experts and approved by the Pancreatology Working Group of the AEG. All items were presented on a single page, with no skip logic or adaptive questioning, and in the same fixed order for all participants. A pilot test involving 6 gastroenterologists was conducted to assess clarity, usability, and technical functionality of the questionnaire. Statistical analysis Categorical variables were summarised as frequencies and percentages, and continuous variables as medians and interquartile ranges (IQR) or means and standard deviation (SD) depending on distribution. Normality was assessed using the Kolmogorov–Smirnov and Shapiro–Wilk tests. For comparative purposes, respondents were stratified according to their self-perceived general knowledge of pancreatic or biliary diseases into a lower (scores < 7) and a higher (scores ≥ 7) knowledge groups. Group differences were assessed using the T Student test for parametric variables and chi-square or Fisher’s exact test for categorical variables, as appropriate. All analyses were conducted using R (version 4.4.2; R Core Team) via the RStudio interface. Reporting guidelines compliance This study adheres to the STROBE (Strengthening the Reporting of Observational Studies in Epidemiology) guidelines ( 13 ) for cross-sectional studies and the CHERRIES (Checklist for Reporting Results of Internet E-Surveys) recommendations ( 14 ), to ensure the methodological quality and transparency of the survey-based research. Results Demographic and training-related characteristics Demographic characteristics are summarised in Supplementary Table 1. A total of 216 respondents completed the survey, most of whom were women (60.5%), with a mean age of 32.3 years (SD 2.7). Overall, 28.8% were final-year gastroenterology residents, whereas 71.2% were certified specialists. Responses were obtained from all regions of Spain. A total of 63% of respondents reported a particular interest in pancreatology, 68.2% in biliary diseases, and 75.3% in advanced pancreatobiliary endoscopy. Most participants trained in tertiary hospitals and had access to pancreatology or biliary specialists. Although exposure to a substantial case volume was common, attendance at multidisciplinary biliopancreatic committees and outpatient clinics was limited. Formal evaluations of pancreatobiliary competencies were infrequent, participation in research projects was low, and awareness of national/European training courses remained generally limited. Self-perceived knowledge General self-perceived knowledge was high, with a median score of 7 (IQR 6–8) for pancreatic and 7 (IQR 7–8) for biliary diseases. Detailed assessments of the 45 items (Table 2 ; Figs. 1 – 2 ) showed the highest perceived competence (median 4 [IQR 3–4]) in common conditions such as acute pancreatitis (AP), biliary colic (BC), acute cholecystitis and cholangitis. Conversely, the lowest scores (median 1 [IQR 1–2]) were reported for chronic pancreatitis–related domains (management of diabetes, nutritional deficiencies, ductal stones/strictures), autoimmune pancreatitis, cystic fibrosis and other rare pancreatic disorders, as well as choledochal cysts and benign biliary lesions. Factors associated with higher knowledge Several training-related factors were significantly associated with higher general self-perceived knowledge (see Table 1 ). These included having a particular interest in pancreatology during residency (p < 0.001), having pancreatology specialists within the gastroenterology department (p = 0.001), managing a larger number of pancreatobiliary cases (p = 0.003), gastroenterology-led responsibility for conditions such as BC (p = 0.030), regular attendance at multidisciplinary pancreatobiliary committees (p = 0.007), more intensive exposure to pancreatobiliary outpatient clinics (p = 0.002), receiving a higher number of dedicated teaching sessions (p = 0.003), undergoing structured competency evaluation (p < 0.001), participating in a greater number of research projects (p = 0.008), and being aware of the AESPANC Residents’ Course (p = 0.046) and United European Gastroenterology Masterclasses (p = 0.017). Discussion This national survey provides the first comprehensive assessment of how Spanish gastroenterology trainees and early-career specialists perceive their knowledge and training in biliopancreatic diseases, identifying major gaps as well as training-related factors associated with stronger competence. Training heterogeneity—closely linked to the absence of precise recommendations—is a recognised global challenge ( 5 – 9 ). Previous evidence highlights the substantial variability in European gastroenterology curricula, noting that pancreatology is among the subspecialties with fewer structured learning opportunities and with unclear mentorship and evaluation frameworks ( 5 – 9 ). Our findings mirror this variability within Spain: responsibility for common conditions such as BC differs widely between hospitals; attendance at multidisciplinary biliopancreatic committees is limited; exposure to specialised outpatient clinics is scarce; participation in research projects is low; and formal competency assessments are infrequent—even in high-volume tertiary centres. Given the increasing incidence of biliopancreatic diseases, harmonising and strengthening competency-based training standards is becoming increasingly urgent. Only a minority of respondents were aware of national or European training initiatives, echoing previous observations of low attendance at specialty meetings( 10 ). This highlights a missed opportunity for scientific societies to improve dissemination and facilitate access to high-quality educational programmes. Self-perceived knowledge of general pancreatology and biliary disease was overall high. As in the study by Hopson et al. on paediatric pancreatology ( 10 ), performance across the 45 items aligned closely with disease prevalence, with the highest scores in common conditions such as AP, acute cholecystitis, cholangitis, and BC. In contrast, substantial gaps emerged in less frequent conditions—including cystic fibrosis, autoimmune pancreatitis, and benign biliary disorders as well as in several highly prevalent and clinically relevant domains, particularly chronic pancreatitis, recurrent-AP, pancreatic cystic lesions, and screening of high-risk individuals for pancreatic cancer. These deficits likely reflect limited exposure to outpatient-based conditions, rather than true rarity. These findings underscore the need for residency programmes and scientific societies to prioritise these domains in structured educational initiatives. Several training-related characteristics were consistently associated with higher self-perceived knowledge. Greater clinical exposure—defined by patient volume and outpatient clinic experience—was strongly linked to increased confidence, consistent with Hopson et al.( 10 ), where limited patient exposure was identified as a major barrier to competent learning. Participation in multidisciplinary biliopancreatic committees also proved beneficial by providing exposure to complex case discussions and cross-specialty decision-making ( 15 ). The presence of a dedicated pancreatology mentor further supported academic development, consistent with evidence showing higher research productivity among trainees with local mentorship ( 12 ). Limited research opportunities, frequently reported in the literature ( 10 , 11 ), mirrored the low participation in pancreatology-focused projects observed in our cohort. Finally, targeted teaching sessions and formal competency assessments—often, though not necessarily, linked to the presence of a pancreatologist—were associated with higher knowledge. This aligns with previous evidence identifying the lack of dedicated teaching as a major training barrier ( 10 ). These findings have clear implications for practice, reinforcing the need for structured, competency-based training that provides adequate inpatient and outpatient exposure, fosters participation in multidisciplinary decision-making, and ensures mentorship by dedicated specialists. Residency programmes should incorporate clear learning objectives, standardised evaluation tools, and targeted teaching sessions to address the main areas of deficit identified in this survey. This study has limitations inherent to survey-based designs, including potential selection bias. Some factors associated with higher knowledge—such as research involvement or awareness of specialised courses—may therefore reflect underlying interest rather than causal effects; for this reason, these elements were not emphasised in our analysis. Nevertheless, its robust methodology, sizeable national sample, and adherence to CHERRIES standards provide a comprehensive and reliable assessment of current training gaps. Conclusions In summary, this study reveals substantial variability in training experiences and significant gaps in self-perceived competence in key biliopancreatic domains among Spanish gastroenterology trainees and early-career specialists. Several modifiable factors—including clinical exposure, participation in multidisciplinary settings, availability of pancreatology mentors, and structured teaching with formal assessment—appear to play a crucial role in strengthening knowledge. These findings highlight an urgent need to standardise and enhance training pathways in biliopancreatic diseases, with coordinated efforts from training institutions and scientific societies to ensure that future gastroenterologists are adequately equipped to manage these increasingly prevalent and clinically complex conditions. We aimed to assess self-perceived knowledge in biliopancreatic diseases among Spanish gastroenterology trainees and early-career specialists, and to identify training-related factors associated with higher competence.\u003c/p\u003e\u003ch2\u003eMethods\u003c/h2\u003e \u003cp\u003eA nationwide cross-sectional online survey was conducted among final-year residents and specialists completing residency between 2018 and 2024 under the auspices of the Spanish Association of Gastroenterology(AEG) and the Spanish Association of Pancreatology(AESPANC). Participants reported demographics and training characteristics. General self-perceived knowledge was rated on a 0\u0026ndash;10 scale, and knowledge of 45 pancreatic and biliary conditions was assessed using a 5-point Likert scale.\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e \u003cp\u003eOf 216 respondents (60.5% women; mean age 32.3 years), 28.8% were residents and 71.2% specialists. Median general knowledge scores were 7/10 for both pancreatic and biliary diseases. Highest competence was reported in common conditions (acute pancreatitis, biliary colic, acute cholecystitis, cholangitis), lowest scores were observed for chronic pancreatitis, autoimmune pancreatitis, cystic fibrosis, pancreatic cystic lesions, and benign biliary disorders. Factors associated with higher knowledge included presence of pancreatology specialists, larger case volumes, outpatient clinic exposure, participation in multidisciplinary committees, structured teaching, and formal competency assessment.\u003c/p\u003e\u003ch2\u003eConslusions:\u003c/h2\u003e \u003cp\u003eTraining in biliopancreatic diseases among Spanish gastroenterology trainees is heterogeneous, with significant gaps in clinically relevant domains. Structured clinical exposure, mentorship, formal evaluation, and targeted teaching are key factor.
