Structured vs. Unstructured Encounter Forms: A Comparison
OA: closed
Abstract
The “ENCOUNTER FORM” is an instrument used to record information regarding a patient visit. This is used for permanent storage in the medical record, for data entry, or, at times, for both purposes. The design of the encounter form is a function of both the needs of the medical information system and the ability of that system to interact with the providers who document the care. Experience with two types of forms, structured and unstructured, and provider acceptance of the forms, is the topic of this paper.
