Профілактика олігоменореї та вторинної аменореї в дівчат-підлітків

Objective — to develop expectancy tables providing physicians with an opportunity to raise, as early as initial examination of a girl, the question of possible emergence of hypomenstrual syndrome (HMS).Materials and methods. We examined 151 female patients with primary oligomenorrhea (I OM), 85 patients with secondary oligomenorrhea (II OM) and 305 girls with secondary amenorrhea (SA) aged 13—18 years, as well as 102 girls of the same age with regular menstrual cycle. Data about the prevalence of hirsutism and the onsets of menarche among the population were received in the course of preventive examination of 1650 adolescents in Kharkiv and Kharkiv Oblast. Statutory values of body mass index for every age were determined using the percentile method. State of hemodynamics of internal genitalia was determined using common methods. We were estimating prognostic significance and informative value of every risk factor investigated.Results and discussion. It was determined that prognostics for the development of I OM are: early reproductive age of a mother at the time of birth of a girl, disorders of mother’s menstrual and reproductive functions, delayed menarche, reverse puberty, threat of termination of pregnancy in the prenatal anamnesis of a girl. Prognostics for the development of II OM are: untimely menarche, obesity, acute hirsutism. Prognostics for the development of SA are: delayed menarche and disorders of menstrual function in a mother, her early age at the time of childbirth, complicated perinatal anamnesis, body mass deficiency, acute hirsutism and delayed menarche in a girl. Decrease of systolic blood velocity in uterine artery was identified as a risk factor to all disorders of menstrual function.Conclusions. Identified prognostics are basis for selection of girls to risk groups for emergence of HMS. Doctors of different specialties, primarily pediatricians and family physicians, should take part in preventive activities against emergence of HMS.
