Clinical Characteristics and Outcomes of Pregnant Women with Omicron Infection in Jilin Province, China

Abstract
Background: To date, the clinical data of pregnant women infected with the Omicron variant is limited. This study aimed to describe clinical features and maternal-neonatal outcomes in pregnancy with the Omicron variant infection and assess the vertical transmission possibility in Jilin Province, China.Methods: In the retrospective study, medical records of pregnant patients with Omicron infection in Jilin Central Hospital and Jilin Maternity and Child Care Hospital were reviewed from March 14,2022 to April 06, 2022. Data were collected from electronic records including maternal age, gestational age on admission, epidemiologic history, history of SARS-CoV-2 vaccination, clinical manifestations, laboratory examinations, chest CT, disease severity, treatment measures, and maternal and neonatal outcomes. Amniotic fluid, umbilical cord blood (UCB), breastmilk samples along with nasopharyngeal and oropharynx swabs of neonates were collected and detected SARS-CoV-2 using RT-PCR.Findings: The twelve patients were all in the third trimester at onset of the disease. A total of eleven patients had COVID-19 inactivated vaccines. Cough were observed in nine patients and fever in five patients. Six patients had lymphopenia. Eleven patients had complications. One patient had vaginal delivery and eleven patients had caesarean section. The twelve newborns including one preterm and eleven full-term infants were all in good condition with Apgar score≥9. The birthweights were all >2500g. All patient were diagnosed with ‘mild type’ of COVID-19 pneumonia on admission. One patient (patient 6) developed ‘standard type’ with typical findings on her chest CT scan. Twelve patients had nasal oxygen support and antibiotics after delivery. Eleven patients were treated with Lianhua Qingwen capsules. None had corticosteroid treatment. All twelve patients had been discharged. Average hospitalization was 10·4 days (range 3-17 days). Nucleic acid tests for amniotic fluid, UCB, human milk, and neonatal nasopharyngeal and oropharynx swab were all negative.Interpretation: Pregnant women with Omicron infection had mild clinical manifestations, and both the maternal and neonatal outcomes were favorable.
