Abstract

患者女,31岁.周期性腰痛3年,加重伴左下肢疼痛1年.以周期性腰痛为首发症状,月经期出现,后逐渐缓解.症状逐渐加重,后出现左下肢疼痛,于2010年11月21日收入院.影像学检查提示腰椎管内占位.无高血压、心脏病、糖尿病、风湿、类风湿病、肝炎、结核病史,无药物过敏史.2003年行剖宫产术,2008年行痔疮切除术.体检:脊柱生理弯曲存在,活动可,左大腿前内侧下部、左膝关节内侧上部感觉减退.左足拇指背伸力弱,肌力Ⅳ+,双下肢肌力、感觉未见明显异常.辅助检查:腰椎CT示腰3椎体后部中间及左侧区域、左侧椎弓根可见不规则骨质破坏,周围可见骨质硬化(图1).腰椎MRI示腰椎曲度存在,序列整齐,腰3椎体后部中间及左侧区域见异常信号影,T2WI上表现为高、等、低混杂信号,T1WI表现为等、低混杂信号,肿物周围骨质信号异常,T1WI、T2WI上均表现为低信号,左侧神经根通道狭窄.L4～5、L5 ～S1椎间盘信号改变,略向后膨出,硬膜囊未见明显受压(图2).请妇产科医师会诊,高度怀疑子宫内膜异位侵及椎管.考虑经保守治疗无效,遂于全麻下行腰椎后路病灶清除植骨融合内固定术,术后给予消炎、止痛、脱水药物治疗。
