Abstract

Abstract Background Identifying disparities in poverty characteristics across different family structures is crucial for understanding the multidimensional nature of poverty and developing targeted intervention strategies. Methods This study was conducted in 2023 socioeconomic deprived rural areas of Shandong Province, China. The multidimensional poverty index system includes six dimensions, with higher scores reflecting greater deprivation. The deprivation scores were calculated according to the weights of multidimensional poverty index system to explore the poverty characteristics of different family structures. Ordinary least squares (OLS) regression analysis was used to assess the impact of family structure on various poverty characteristics. Results The results showed that compared to single-person households, young couple households, elderly couple households, two-generation families, and multigenerational families exhibited significant negative correlations with economic security, education, social capital, and living conditions ( P  < 0.05). Additionally, elderly couple households, two-generation families, and multigenerational families were significantly negatively correlated with health status compared to single-person households ( P  < 0.1). Specifically, elderly couple households showed a significant negative correlation with healthcare utilization compared to single-person households ( P  < 0.1). Conclusion These findings suggest that decision-makers should pay more attention to the distinct poverty characteristics of various family structures, which will enable the implementation of tailored and comprehensive support measures that effectively address the unique challenges faced by different household types.

In 2015, the United Nations adopted the Sustainable Development Goals (SDGs), with “End poverty in all its forms everywhere” as the first goal [ 1 ]. Thanks to global efforts, the number of people living in poverty decreased from 1.96 billion in 1981 to 632 million in 2019. However, factors like the COVID-19 pandemic and geopolitical conflicts have caused the global poverty population to rise to 1.1 billion by 2023[ 2 , 3 ]. Poverty is inherently dynamic, and the definition of the impoverished population should align with the socio-economic development[ 4 ]. For instance, low- and middle-income countries such as China and India typically employ unidimensional indicators like household income or consumption levels to measure absolute poverty[ 5 , 6 ]. Countries in the Organization for Economic Co-operation and Development (OECD) use metrics based on a percentage of median income or a proportion of national average income to assess relative poverty[ 7 , 8 ]. Since Sen introduced the concept of “capability poverty”, academic research on poverty has increasingly adopted a multidimensional perspective[ 9 ]. The adoption of a multidimensional poverty indicator system allows for monitoring various dimensions of poverty, facilitating the identification of impoverished individuals and the formulation of more precise anti-poverty strategies[ 10 ]. This study is based on the multidimensional poverty index system developed by Hao et al., using the Delphi method and Analytic Hierarchy Process (AHP) to reflect China’s characteristics [ 11 ]. The family, as the fundamental unit of society, holds a pivotal role in determining the incidence of familial poverty. Family structure refers to the composition of family members, including both demographic characteristics and intergenerational patterns[ 12 ]. Countries like the United Kingdom and the United States have refined their poverty thresholds based on household structure. These nations typically provide greater attention and higher poverty standards for single-person households, families with children, and households with elderly members[ 13 , 14 ]. By identifying specific household characteristics, we aim to better capture the real-life experiences of impoverished populations, rather than merely focusing on income poverty[ 15 , 16 ]. Previous studies have explored the correlation between family structure and poverty[ 17 ], focusing on the number of family members, the number of labor, the number of children, and the number of elderly. Family structure has a generally positive influence on households, primarily by affecting family size[ 18 ]. Reciprocal support among family members can alleviate pressures during challenging times, further helping to reduce familial poverty[ 19 ]. The composition of family structures significantly influences the labor availability. Consequently, households with greater labor force participation tend to have diversified income sources, leading to higher overall income levels[ 20 ]. The employment status and skill levels of family members significantly influence the extent of household poverty[ 21 ]. In addition to the quantity of labor, other factors, such as the proportion of children and elderly members in the family, also impact poverty levels. This is because the proportion of children is closely linked to educational expenditures, while the proportion of elderly members is closely related to healthcare costs[ 22 , 23 ]. Low-, middle-, and high-income countries have undertaken explorations to measuring poverty[ 24 ]. However, research on the deprivation characteristics of these structures from a multidimensional poverty perspective remains limited. Our goal is to identify the distinct poverty characteristics across various family structures. This study, by focusing on multidimensional poverty, aims to provide policymakers with a more comprehensive understanding of post-poverty alleviation challenges. It emphasizes the importance of considering family structural disparities when devising poverty reduction strategies[ 25 ]. 2 Methods 2.1 Data source This study was conducted in 2023 in Shandong Province, China, where all poverty-stricken counties had been eliminated by 2020. The study focused on counties that were previously impoverished, primarily located in three cities within Shandong Province. A multistage sampling method was employed: two counties selected from each city, three townships selected from each county, four villages from each township, with face-to-face interviews conducted with 35 households per village. The survey initially included 2,527 households, however, due to the missing value of key variables related to the household head’s information, leaving 2,399 households included in the final analysis. 2.2 Measurement of family structure types In accordance with the classifications proposed by Krueger et al.’s and Dong et al.’s, the definition and categorization of family structures in this study were detailed in Table 1 [ 17 , 26 ]. The economic dimension encompassed four key indicators: employment, basic pension and medical insurance, current assets, and fixed assets. The health status dimension included nutrition, physical health, mental health, and illness. The education dimension encompassed both education attainment and vocational training. The social capital dimension included social participation and access to information. The healthcare utilization dimension covered healthcare accessibility, chronic disease treatment, inpatient care, National Basic Public Health Services (NBPHS), and health checkups. Finally, the living conditions dimension encompassed the availability of clean fuels, sanitary toilets, safe water, electrification, safe housing structures, and high-value durable goods. Each indicator was categorized into two types based on whether the family was deprived (0 = not deprived, 1 = deprived). We then multiplied each indicator by its corresponding weight according to different family structures and calculated the total score by summing all indicators within each dimension. Figure 1 shows the weights of the multidimensional poverty index system achieved by our team in 2024[ 27 ]. 2.4 Control variables The control variables included the household consumption expenditures (0 = low, 1 = lower, 2 = average, 3 = higher, 4 = high), payment for government subsidies (0 = yes, 1 = no), payment for medical assistance (0 = yes, 1 = no), payment for inclusive commercial health insurance (0 = yes, 1 = no), incurrence of high-cost medical expenses in the past five years (0 = no, 1 = yes), identification as an impoverished household (0 = no, 1 = yes), assumption of debts (0 = no, 1 = yes), subjective economic status (0 = good 1 = bad), residence of friends or relative in county or city (0 = yes, 1 = no), and whether friends or relative held official positions at or above the county level (0 = yes, 1 = no). 2.5 Data analysis One-way ANOVA was used to determine if there are significant differences in means among different groups. The Kruskal-Wallis H test was then performed to further explore whether the medians of multiple independent samples were identical. Finally, ordinary least squares regression analysis was conducted to examine the correlations between family structures and the six dimensions of multidimensional poverty index system. All analyses, including the one-way ANOVA, Kruskal-Wallis H test, and OLS regression, were performed using Stata 14.0. 3 Results 3.1 Descriptive analysis Table 2 presents the descriptive statistical analysis results for 2399 households. Among various family structures, single-person households, elderly couple households, and multi-generational households exhibited the highest levels of economic disadvantage, with deprivation scores of 0.1264, 0.0762, and 0.0390, respectively. Young couples and two-generation households faced the greatest health deprivations, with scores of 0.0413 and 0.0343, respectively. All control variables exhibited a significant correlation with family structures ( P < 0.05). The poverty deprivation scores description of different family structures are shown in Fig. 2 . This study primarily examines the disparities in poverty characteristics across various family structures. Similarly, elderly couple households, two-generation families, and multigenerational families showed a significant negative correlation with health status compared to single-person households ( P < 0.05). Compared to single-person households, elderly couple households, two-generation families, and multigenerational families showed a significant negative correlation with health status ( P < 0.1). Compared to single-person households, only elderly couple households showed a significant negative correlation with healthcare utilization ( P < 0.1). The relationships between the five types of family structures and the six dimensions are shown in Fig. 3 . This study reveals that different family structures exhibit distinct characteristics in terms of poverty disparities. Specifically, single-person households experience the most severe deprivation among family structures, which is consistent with Choi S et al.’s research[ 28 ]. Two-generation families are the least deprived family structure, which is also consistent with the research conducted by Gao M[ 29 ]. However, this contradicts Deng Th et al.’s finding[ 30 ], which suggests that the incidence of poverty is higher in two-generation cohabiting families, as the functional impairments of elderly individuals within these families have a more pronounced impact on poverty. Additionally, different family structures exhibit diverse poverty characteristics. Single-person households, elderly couple households, and multigenerational families experience the most severe deprivation in terms of economic security, while young couple households and two-generation families face the greatest challenges in health. Firstly, the economic deprivation experienced by single-person households is the most severe across all dimensions. Single-person households are not only constrained by single income source but also bear all living expenses alone, resulting in very limited capacity to withstand financial risks[ 31 ]. Despite growing attention from the country and society towards single-person households, gaps in the social security system make it difficult for them to obtain sufficient support[ 32 ]. The deprivation rate for medical assistance among single-person households is the highest among all family structures, at 17.1%. Secondly, single-person households, elderly couple households, two-generation families and multigenerational families experience the least deprivation in living conditions, while young couple families have the lowest level of educational deprivation. This phenomenon can be attributed to China’s poverty alleviation initiatives, which have largely secured housing security in impoverished rural regions[ 33 ]. China’s implementation of nine-year compulsory education has provided young couple households with greater educational opportunities[ 34 ]. Additionally, the country’s emphasis on employment has provided many young people numerous chances for vocational training[ 35 ]. Thirdly, elderly couple households exhibit the highest deprivation among all dimensions. The deprivation rate of elderly couple households in subjective economic status was 12.4% higher than that of young couple households. A plausible explanation is that, in relatively impoverished rural areas, elderly couple households had limited access to education during their youth and primarily relied on odd jobs or farming for livelihood. As a result, they often lack stable income in their later years[ 36 , 37 ]. Fourth, two-generation families experience the most severe deprivation in the health dimension. In relatively impoverished rural areas of China, many two-generation families follow a skipped-generation household structure, typically comprising grandparents and their grandchildren. Families with a generational gap often consist of elderly individuals living alone and children left behind in rural areas, making them particularly vulnerable, facing heightened psychological and physical challenges[ 38 ]. Fifth, multigenerational families face the most severe economic deprivation. Multigenerational families typically consist of young, middle-aged, and elderly generations, with the middle-aged bridging the gap between caring for aging parents and raising their own children[ 39 ]. We found that the debt ratio of multigenerational families is 34.2%, 18.2% higher than that of young couple families, which have the lowest debt ratio. This suggests a relatively severe economic burden for multigenerational families. Elderly couple households were significantly associated with all dimensions of poverty. Several potential reasons may explain this phenomenon. Firstly, family structure can influence the household’s financial situation. Only elderly couple households demonstrated a significant relationship with the utilization of healthcare services ( P < 0.1). Elderly individuals typically have greater healthcare needs, but due to economic constraints, their routine healthcare consumption remains limited. This can lead to hospitalization for minor ailments and result in high-cost medical expenses[ 40 ]. Despite the important role of medical insurance in safeguarding public health, disparities persist due to uneven healthcare development, leading to health inequalities[ 41 ]. Regarding household consumption expenditures, 52.59% of elderly couple households fall into the lower expenditure categories, representing more than half of this group. This can be attributed to the decline in labor capacity among the elderly and their limited income sources, which in turn affects their consumption expenditures[ 31 ]. Secondly, family structure influences income redistribution. Among elderly couple households, the government subsidy deprivation rate is the highest, accounting for 34.42% of all households without government subsidies. In China, government subsidies primarily encompass includes old-age allowances and minimum subsistence allowances. A significant portion of the elderly population in Shandong Province has not yet reached the age of 80, the eligibility threshold for receiving senior citizen allowances[ 42 ]. Despite their poverty, many elderly individuals fail to qualify for the minimum subsistence allowance because their income does not meet the required criteria[ 43 ]. The percentage of elderly couple households that were formerly impoverished is 18.2%, a 6.2% increase compared to the deprivation rate of young couple households. For elderly couple households who were once impoverished, the government faces challenges in implementing sustainable poverty alleviation measures such as employment support[ 44 ]. Consequently, these families exhibit a heightened reliance on income redistribution mechanisms[ 45 ]. Ultimately, family structure significantly influences social connections. In households with elderly couples, 79.8% reported having no relatives or friends residing in urban areas or counties, while 96.7% indicated no connections with individuals holding official positions in cities or counties. This indicates that the social networks of the elderly have become more fragile. There is a noticeable decline in their engagement in social activities, with most maintaining connections primarily with neighbors or children[ 46 , 47 ]. As a result, it is crucial to emphasize the importance of poverty alleviation policies focusing on elderly households, ensuring their diverse healthcare needs and mental wellbeing are adequately addressed, aspects often overlooked in conventional income-based poverty metrics. Additionally, the health status of young couple households and two-generation families, as well as the economic stability of single-person and multigenerational households, warrants prioritized attention in future poverty reduction strategies. 4.2 Limitations This study has several limitations. This study has several limitations. First, as a cross-sectional study, while we confirmed significant associations between family structures and multidimensional poverty characteristics, it cannot establish causal relationships between family structures and multidimensional poverty characteristics. Second, the data and indicator system we used were sourced exclusively from China, thus requiring flexible adaptation for application in other countries with different cultural, social, and policy environments facing similar challenges. Third, as the data in this study relied on self-reporting, it may be subject to recall bias. This may affect the accuracy of measures such as subjective economic status and self-rated health, and consequently influence the accuracy of the research findings. 4.3 Conclusion This study reveals that different family structures exhibit distinct characteristics of poverty in relatively impoverished rural regions. Although China successfully eradicating absolute poverty by 2020, more nuanced approaches are needed to ensure the consolidation and sustainability of its anti-poverty efforts. Single-person households, elderly couple households, and multigenerational families should focus on addressing economic deprivation. The government should provide targeted support for the elderly and offer employment training and skill enhancement programs for the middle-aged and young adults to promote employment and improve their labor skills and economic earning capacity. Additionally, strengthening healthcare services and social security systems, especially serious illness insurance, is crucial to mitigating economic risks. Young couple and two-generation households should address health-related deprivations, focusing on both physical and mental well-being. The deprivation scores were calculated according to the weights of multidimensional poverty index system to explore the poverty characteristics of different family structures. Ordinary least squares (OLS) regression analysis was used to assess the impact of family structure on various poverty characteristics.\u003c/p\u003e\n\u003cp\u003e\u003cstrong\u003eResults\u003c/strong\u003e\u003c/p\u003e\n\u003cp\u003eThe results showed that compared to single-person households, young couple households, elderly couple households, two-generation families, and multigenerational families exhibited significant negative correlations with economic security, education, social capital, and living conditions (\u003cem\u003eP\u003c/em\u003e \u0026lt; 0.05).
