Abstract

Abstract In psychiatry clinical trials, an instrument or questionnaire with rating scale is often used to access safety and efficacy of a test treatment under investigation. Statistical analysis based on the collected rating scale is then performed a determine whether there is an improvement in endpoint change from baseline mean scale. This approach needs on absolute change, however, may not actually reflect the performance of the test treatment under study because the change, which may be considered of clinically importance, may fall within the same category in terms of disease severity such as mild, moderate, and severe. In this case, it is suggested, in addition to absolute change approach, a categorical shift analysis be considered to determine whether the endpoint change from baseline has resulted in an improvement in categorical shift, in terms of disease severity shift e.g., from severe to mild or moderate. In this article, we explore the relationship between absolute change approach and categorical shift analysis based on rating scales for assessment of test treatment under study.

Introduction Questionnaires serve as pivotal instruments in evaluating various health domains, notably in assessing mental health within clinical trials. Comprising primarily open-ended and closed questions, questionnaires are designed to elicit quantitative data for statistical analysis and qualitative data for in-depth insights. For example, psychological patients who have anxiety or distress need a standard estimation to evaluate their current psychological health. Experts rely on standardized tools for diagnosis, as patients’ self-reported symptoms may be too subjective. Moreover, in the process of generating questionnaires, open questions and closed questions are as two main question methods used in creating questionnaires [ 3 ]. As the primary types employed in questionnaires, each with distinct advantages; open questions explore broader topics, whereas closed questions solicit specific responses [ 4 ]. For the binary closed question, the simplest answer setting is agree/disagree. There are also some requirements for questions’ setting: To minimize response bias, questionnaires should avoid leading questions, enabling respondents to use intuition for prompt answers. Questions should be straightforward to prevent overthinking and preserve the respondents’ initial, intuitive reactions. Once we have gained an understanding of how the questionnaire is designed and the fundamental rules governing question construction, it is essential to explore how experts utilize questionnaires as instruments in clinical trials. Presently, experts commonly employ two distinct rating scale methods once respondents have completed the questionnaire: absolute change and categorical shift. To exemplify the clinical effect, Thompson employed the Brief Psychiatric Rating Scale (BPRS) as a representative case of the absolute change rating scale method [ 5 ]. It’s important to note that these two methods may not interchangeable; a clinically significant result based on the absolute change rating scale may not necessarily correspond to the same outcome observed using the categorical shift rating scale assessment. At times, absolute change may indeed yield statistically significant differences, but these differences may not translate into clinical efficiency. To accurately gauge the efficacy and precision of questionnaire grade changes, the categorical shift method becomes indispensable as a reliable tool. In the following discussion, we outline four possible scenarios illustrating absolute change and categorical shift rating scale outcomes in clinical assessments. The result of his first test is 70/100, 20 questions are included in this questionnaire and each question is worth 5 points. Hence, each 5 points will be set as one level to estimate where the patient is. The final questionnaire result for diagnosis will depend on which level of the patient in. After treatment, a follow-up questionnaire score determines the level of improvement. If the first-time test grade is 70/100, the second-time test grade is 50/100. Based on the number shown, there are 20 gaps (|50–70| = 20, |second grade – first grade|) which is an absolute value that the expert looking for. A 20-point reduction indicates effective treatment, as evidenced by the absolute change in score. Otherwise, if he gets ‘original positive’ absolute value (second grade > first grade), experts might switch treatments or adjust the dose because an increase in the score indicates a worsening of the condition. Additionally, the categorical shift rating scale method is completely different from the absolute change method. In this case, a patient’s two times scores of 70/100 and 50/100 are categorized in same range: scores from 0–25 represent ‘none’, 25–50 ‘mild’, 50–75 ‘moderate’, and 75–100 ‘severe’. Assign two times categorical changes maintain moderate after a period of treatments. The experts could simply conclude the treatment does not have big improvements on the patient and they might need to adjust the treatment depending on numerical change. This situation remains vague whether the improvements reflected in the absolute change rating scale and the categorical shift represents equivalent clinical outcomes. Hence, the purpose of this research is to compare these commonly used methods: categorical shift and absolute changes for test treatment evaluation under investigation. In the next section, statistical analysis based on absolute change in rating scales will be briefly described. Also included in this section is statistical test for categorical shift before and after treatment. Section 3 compares the two methods in terms of false positive or false negative for various scenarios. Finally, a clinical trial simulation will be detailed in Section 4, and the paper will conclude with remarks that summarize the findings of our research. 2. Analysis of Rating Scales Rating scale is an evaluation tool in clinical trials for qualifying responses and assessing health attributes. Let’s consider Beck Depression Inventory (BDI) as an analysis questionnaire. The BDI is a self-report tool designed to measure the severity of depression symptoms, with the standard cut-offs are as follows: 0–9 indicates minimal depression, 10–18 suggests mild depression, 19–29 corresponds to moderate depression, 30–63 signals severe depression. Higher total scores indicate more severe depressive symptoms. This instrument exemplifies an absolute change questionnaire, with scale groups that align with specific categorical descriptions. In subsequent analyses, we will employ the BDI to substantiate our hypotheses. Absolute Change 0–9 10–18 19–29 30–63 Categorical Shift Minimal Mild Moderate Severe For the evaluation of the efficiency and significant difference between the two rating scale methods, the null hypothesis of interest is to test whether there is a significant difference in mean response between the absolute change rating scale and the categorical shift rating scale. In accessing the accuracy and operating sample size of the absolute change rating scale method, the null hypothesis aims to determine whether there is a difference between the true response rate of the test treatment and the reference value. This test problem is referred to as the one-sample problem and we test for noninferiority. Regarding the accuracy and operating sample size of the categorical shift rating scale method, the null hypothesis examines whether there is a difference in categorical shift before and after the application of the treatment. This issue is addressed using the Stuart-Maxwell test. 2.1 Test for Noninferiority Sample Size Estimation for Absolute Change Rating Scale Let \({x}_{i}, i\in N+\) be the response result from the \(i\) th subject. Assume \({x}_{i}\) could be the questionnaire rating subtraction calculation ( \({t}_{2}-{t}_{1})\) from pre-treatment questionnaire result ( \({t}_{1})\) and after-treatment questionnaire result ( \({t}_{2}\) ). When \({t}_{2}-{t}_{1}>0\) , the numerical positive result means there is a negative treatment response, which is invalid \({x}_{i}\) . Negative \({x}_{i}= \left({t}_{2}-{t}_{1}\right)\) implies treatment validation, example of the BDI illustrates the lower rating score, the better treatment appears. Therefore, that is \({x}_{i}<0\) for responder or \({x}_{i}\ge 0\) for non-responder. Assumed that \({x}_{i}\) is i.i.d. with \(P\left({x}_{i}\right)=p\) , where p is the true response rate, $$\begin{array}{c}\widehat{P}=\frac{1}{n}\sum _{i=1}^{n}{x}_{i} \left(1\right)\end{array}$$ Additionally, to test whether there is a difference between the true response rate of the test treatment and the reference value, the following hypotheses are usually considered: $$\begin{array}{c}{H}_{0} :p-{p}_{0 }\le \delta versus {H}_{a}:p-{p}_{0 }>\delta \left(2\right)\end{array}$$ We are looking for whether there is a statistically significant difference after treatments (better result in BDI questionnaire result). Therefore, we focus on using test for noninferiority to calculate the sample size for when questionnaire using absolute change rating scale[ 1 ]. $$\begin{array}{c}n=\frac{{\left({Z}_{\alpha }+{Z}_{\beta }\right)}^{2}p\left(1-p\right)}{({\epsilon -\delta )}^{2}} \left(3\right)\end{array}$$ Suppose that the questionnaire response rate of the patient population under study after treatment is expected to be around 50% ( \(p=0.5)\) . When \(\alpha =0.05\) , the required sample size for having an 80% power ( \(\beta =0.2\) form power value) for correctly detecting a difference between the questionnaire response rate and the reference value of 30% ( \({p}_{0}=0.3\) ). Also assume that superiority margin e is consider as 10% ( \(\delta = -10\%)\) , the sample size could be $$\begin{array}{c}n=\frac{{\left({Z}_{\alpha }+{Z}_{\beta }\right)}^{2}p\left(1-p\right)}{({p-{p}_{0}-\delta )}^{2}}=\frac{{\left(1.64+0.84\right)}^{2}0.5\left(1-0.5\right)}{{\left(0.5-0.3+0.1\right)}^{2}}=18 \left(4\right)\end{array}$$ 2.2 Analysis Based on Categorical Shift In questionnaire diagnosis process, results are often characterized by shifting across multiple categorical levels, this type of rating scale change is referred as the categorical shift rating scale. It is common to host more than two possible outcomes (common categories include: minimal, mild, moderate, severe) using to diagnose individuals. In this scenario, we consider using the Stuart-Maxwell test[ 6 ] to analyze the sample size. 2.2.1 Stuart-Maxwell test Let \({x}_{ij} \in \{1:Mininal,2:Mild,3:Moderate,4:Severe\}\) , \(i\) th subjects represents the different categorical values and \(j\) th treatment ( \(j\) = 1: pretreatment and \(j\) = 2 :post-treatment) be the categorical observations, there exists 4 categorical evaluations for pretreatment and post-treatment as Table 1 . There are 4 categorical values (i.e., r = 4) related questionnaire results presented Table 2 . To check if there exists treatment effect, we assume power function \(\beta =0.20\) at 5% level of significance. To access the \(\delta\) , we follow $$\begin{array}{c}{\chi }_{\frac{r\left(r-1\right)}{2}}\left({\chi }_{\alpha ,\frac{r\left(r-1\right)}{2}}^{2} |\delta \right)=\beta \left(5\right)\end{array}$$ $$\begin{array}{c}{\chi }_{\frac{4\left(4-1\right)}{2}}\left({\chi }_{0.05,\frac{4\left(4-1\right)}{2}}^{2}|\delta \right)=0.20 \left(6\right)\end{array}$$ By solving Eq. 6, we get \({\delta }_{\alpha ,\beta }=13.62.\) Therefore, the sample size needed for achieving an 80% power is given by \(n={\delta }_{\alpha ,\beta }{\left[\sum _{i<j}\frac{{\left({p}_{ij}-{p}_{ij}\right)}^{2}}{{p}_{ij}+{p}_{ij}}\right]}^{-1}\approx 146\) . 3. Comparison \(\delta\) represents the smallest clinical change that is considered meaningful by researchers. The smaller the \(\delta\) , the higher the accuracy and efficiency of one sample size. Based on these two sample size estimations, we posit two distinct \(\delta\) , each corresponding to a different methodological approach to rating scales. Since we have already finished the analysis of two rating scale method, we fix a \(\delta\) to check sample size and ascertain if there is a clinical difference between the two methods. If two rating scale sample size estimation can be interchangeable in clinical application, which implies there does not exist clinical difference between absolute change and categorical shift rating scale methods. Conversely, a non-reasonable sample size represents the existence of clinical differences. First, we explore the potential of interconversion between the two rating scale methods. Through large sample proportion calculations and using Stuart Maxwell test for sample size estimate, we have determined that the absolute change rating scale method requires 18 samples ( \({n}_{1}=18\) ) with a clinically significant difference \({\delta }_{1}= -10\%\) , while the categorical shift rating scale method requires 146 samples with a clinically significant different \({\delta }_{2}=13.62\) . The chosen fixed \(\delta\) values highlight the sensitivity of our analytical methods. Selecting a more conservative \(\delta\) of -0.1 reflects statistical prudence, representing the lower bound of the effect size under consideration. By focusing on the smaller effect size, we adopt a conservative stance, enhancing the study’s sensitivity to detect minimal yet clinically meaningful differences. This is particularly important when the clinical outcome has high variability or when the intervention’s expected impact is modest. In essence, the − 10% \(\delta\) is not an arbitrary choice but a strategic one, aimed at ensuring robust statistical analysis while acknowledging the clinical imperative of detecting clinical meaningful discrepancy. Given the new − 10% clinical difference for re-evaluating the sample size of categorical shift rating scale method. Keep the r by c contingency table, only change the original \({\delta }_{\alpha ,\beta }=13.62\) to \({\delta }_{2}= -0.1\) . Therefore, the sample size needed for achieving an 80% power is given by \(n={\delta }_{\alpha ,\beta }{\left[\sum _{i<j}\frac{{\left({p}_{ij}-{p}_{ij}\right)}^{2}}{{p}_{ij}+{p}_{ij}}\right]}^{-1}=-1.068742 \approx -2.\) The sample size is not possible equal a negative value which illustrates the difference between absolute change rating scale and categorical shift rating scale significant. Therefore, the questionnaire with absolute change rating scales and categorical shift rating scales are not interchangeable. In addition, further simulation analysis would be considered to estimate two independent variable’s sample size changing tendency. 4. Clinical Trial Simulation In the previous section, we analyzed the clinical differences between the absolute change rating scale and the categorical shift rating scale, concluding that interchangeability between the two methods is not feasible. Consequently, this section will explore the relationship between power, sample size, and clinical difference for these two methods. 4.1 Absolute change Simulation The objective of this simulation was to determine the appropriate sample size for a non-inferiority test concerning the absolute change rating scale method. We assumed the response result \({x}_{i}\) from the \(i\) th subject follows a one-sample design with a large proportion non-inferior test. The absolute change is calculated by the subtraction \(\left({t}_{2}-{t}_{1}\right)\) where \({t}_{1}\) ​ and \({t}_{2}\) ​ are the pre-treatment and post-treatment questionnaire scores, respectively. A negative \({x}_{i}\) ​ (where \({t}_{2}-{t}_{1}<0\) ) indicates a positive response to treatment as per the standards of the Beck Depression Inventory (BDI). The simulation assumed the response results are identically and independently distributed (i.i.d.) with a probability \(P\left({x}_{i}\right)=p\) , where p represents the true response rate. We tested the hypothesis that the true response rate is not inferior to a reference value by a specified non-inferiority margin. Using the non-inferiority margin and the expected true response rate, we calculated the sample size using the formula: \(n=\frac{{\left({Z}_{\alpha }+{Z}_{\beta }\right)}^{2}p\left(1-p\right)}{({\epsilon -\delta )}^{2}}\) , where α is the significance level, β is the power, p is the estimated true response rate, ϵ is the superiority margin, and δ is the non-inferiority margin. We conducted Monte Carlo simulations with the estimated parameters to determine the empirical power of the test across various effect sizes. The simulated power was calculated as the proportion of simulations where the null hypothesis of inferiority was correctly rejected. The results of the simulations indicate a sample size of 18 is sufficient to achieve a power of approximately 0.86 for detecting a true response rate difference from a reference value with 10% non-inferiority margin at a 5% significance level. The simulation framework provided insights into the robustness of the sample size calculation and the sensitivity of the power to changes in effect sizes. From simulation results indicate that there are diminishing returns for both increasing sample size and effect size in terms of power gains. In Table 3 , increasing the effect size from 0.01 to 0.02 increases power more than increasing it from 0.19 to 0.20. This suggests an optimal balance between effect size and sample size to achieve sufficient power without unnecessary use of resources. The detailed results are tabulated as follows, indicating a clear relationship between the effect size, the required sample size, and the simulated power for a given set of parameters. With an alpha threshold of 5% ensuring a 95% confidence interval, we employed Monte Carlo simulation techniques to emulate potential shifts in categorical responses attributable to treatment effects. The simulation’s objective was to ascertain the sample size requisite for discerning a predetermined clinical difference within categorical shift ratings at an 80% power benchmark. Commencing with an initial \(\delta\) of 13.00 and subsequently modulating this parameter, we observed the sample sizes needed to achieve varying power levels. This yielded a discernible trend: as the δ value was amplified, the requisite sample size expanded correspondingly to sustain the targeted power level—this reflects the complexity inherent in detecting more subtle categorical shifts. In Table 4 and Table 5 , discerning a δ of 13.00 required a sample size of 138.9365 to attain a power level proximate to 0.778, whereas a more substantial 𝛿 of 13.40 necessitated an increased sample size of 143.2115 for comparable power. Inversely, as 𝛿 heightened, the β values, denoting the risk of a Type II error decreased, thus enhancing the reliability of detecting larger deltas. Beyond this threshold, the power plateaus, maintaining a consistent level of approximately 80%. These results suggest that the absolute change method does not necessitate a large sample environment for clinical practice. Conversely, the categorical shift rating scale method appears to be more suitable for larger populations. As the precision of power ascends, the identified clinical significance concurrently escalates, necessitating a larger sample size. Therefore, we conclude that the absolute change rating scale is adequate for clinical trials with sample sizes under 100, while the categorical shift rating scale is optimal for trials with a more substantial number of participants. 4.4 Discussion and Limitation From the simulation analysis conducted, it is evident that to maintain comparable power, the absolute change rating scale method requires a smaller sample size than the categorical shift method. Additionally, in the simulations for the absolute change method, the sample sizes consistently correspond to a power of approximately 80%, indicating that the majority of sample sizes achieve just this level of power. The suspended power value restrains absolute change rating scale pursue higher accuracy which limits method’s application sample size. Conversely, for the categorical shift method, there is a positive relationship between sample size and power, suggesting that increases in sample size are associated with proportional increases in power. It does not include the drawbacks of absolute change rating scale, in additional, the categorical shift rating scale could be more appropriate with the larger population size through application. Nevertheless, the categorical shift rating scale grade setting has big gaps between various levels, which is also hard to detect accurate the patients’ psychological situation. For different application scenarios, considering the sample size, accuracy requirement and the patient’s treatment feedback would be reasonable factors to decide which pattern is the best fitful method during clinical trials. 5. Concluding Remarks The core aim of this investigation was to elucidate the differential clinical impact of two distinct rating scale methodologies: the absolute change rating scale and the categorical shift rating scale. Through rigorous simulation and statistical analysis, employing tests for noninferiority and the Stuart-Maxwell procedure, we sought to determine the most suitable sample sizes for each method, while also contemplating their potential for interchangeability in clinical settings. Our findings indicate that the absolute change rating scale demonstrates robust power without the necessity for large sample sizes, reinforcing its suitability for clinical trials where smaller cohorts are typical or when resource constraints limit participant numbers. A stable power threshold of approximately 80% is achievable with sample sizes below 100, making it a pragmatic choice for trials assessing interventions with expected moderate effect sizes. In contrast, the categorical shift rating scale emerges as more pertinent for larger-scale studies. This method’s power is sensitive to sample size increments, suggesting that more substantial cohorts enhance the ability to detect clinically meaningful differences. The method’s reliance on larger samples aligns with trials that anticipate subtle effects or require a more granular analysis of outcome categories. From a clinical perspective, the absolute change rating scale can be advantageous in scenarios where direct changes in a clinical measurement are of primary interest and when these changes are expected to be uniform across the study population. This method simplifies the interpretation of clinical outcomes, as it directly relates to the magnitude of change within a continuous variable. On the other hand, the categorical shift rating scale is especially pertinent in contexts where clinical outcomes are best understood through changes across ordered categories, such as stages of disease progression or severity ratings. The requirement for larger sample sizes is often justified by the need for enhanced precision in detecting shifts that may influence clinical decision-making and patient management. In conclusion, our analysis underscores the necessity of methodological alignment with clinical objectives and study design. While our results delineate clear guidelines for sample size determination, we emphasize that the choice of rating scale must be informed by the specific clinical endpoints and the nature of the intervention under investigation. Future studies should expand on our findings by incorporating sensitivity analyses and exploring the application of these rating scales across diverse clinical conditions and varied trial designs. The ultimate goal remains the enhancement of clinical trial design to ensure that statistical power is optimized, leading to reliable and clinically relevant outcomes. The result of his first test is 70/100, 20 questions are included in this questionnaire and each question is worth 5 points. Hence, each 5 points will be set as one level to estimate where the patient is. The final questionnaire result for diagnosis will depend on which level of the patient in. After treatment, a follow-up questionnaire score determines the level of improvement. If the first-time test grade is 70/100, the second-time test grade is 50/100. Based on the number shown, there are 20 gaps (|50\u0026ndash;70| = 20, |second grade \u0026ndash; first grade|) which is an absolute value that the expert looking for. A 20-point reduction indicates effective treatment, as evidenced by the absolute change in score. Otherwise, if he gets \u0026lsquo;original positive\u0026rsquo; absolute value (second grade\u0026thinsp;\u0026gt;\u0026thinsp;first grade), experts might switch treatments or adjust the dose because an increase in the score indicates a worsening of the condition.\u003c/p\u003e\n\u003cp\u003eAdditionally, the categorical shift rating scale method is completely different from the absolute change method. In this case, a patient\u0026rsquo;s two times scores of 70/100 and 50/100 are categorized in same range: scores from 0\u0026ndash;25 represent \u0026lsquo;none\u0026rsquo;, 25\u0026ndash;50 \u0026lsquo;mild\u0026rsquo;, 50\u0026ndash;75 \u0026lsquo;moderate\u0026rsquo;, and 75\u0026ndash;100 \u0026lsquo;severe\u0026rsquo;. In this case, it is suggested, in addition to absolute change approach, a categorical shift analysis be considered to determine whether the endpoint change from baseline has resulted in an improvement in categorical shift, in terms of disease severity shift e.g., from severe to mild or moderate.
