Abstract

Abstract Sexually transmitted infections (STIs) such as chlamydia, gonorrhea, syphilis, and trichomonas are a global public health burden, particularly in women, where they are linked to HIV, HPV, stillbirth, neonatal complications, pelvic inflammatory disease, and infertility. Asymptomatic infections pose higher risks when left untreated. This study examines the prevalence and determinants of asymptomatic STIs among Nigerian women. The study is part of a larger cohort involving 11,700 women investigating high-risk HPV, vaginal microbiome, and cervical cancer in Nigeria. Participants, who reported no symptoms of STIs or HIV over 12 months, provided high-vaginal swabs for analysis and serum for testing Chlamydia trachomatis and Treponema pallidum antibodies. The mean age of participants was 39.0 years, with an average sexual debut at 20.9 years. Over half (56.3%) tested positive for at least one bacterial infection. Women with STIs had a lower mean age at sexual debut (20.7 years) compared to those without (21.3 years), though this was not statistically significant. However, debut before age 20 was significantly associated with increased risk of asymptomatic STIs (adjusted OR = 0.53, p = 0.02). Early sexual debut and routine screening are key to reducing asymptomatic STIs and associated complications in this population.

STDs typically present with symptoms including penile or vaginal discharge, dysuria, dyspareunia, vaginal bleeding, or spotting, and lower abdominal pain. However, most STDs either do not produce any symptoms, or they produce symptoms so mild that they go unnoticed 4 , 5 . STDs like Chlamydia trachomatis (CT), Neisseria gonorrhea (NG), Bacterial Vaginosis (BV), Trichomonas vaginalis and Gardnerella vaginalis (GV) can be asymptomatic. Consequently, many infected persons do not know that they need medical care, and as a result, reported cases of STDs only represent a percentage of the actual burden of these diseases 4 , 5 . Asymptomatic STDs create enormous public health challenges because they may be associated with poor sexual education, risky sexual behavior, multiple partners, and unprotected sexual encounters, which enhances STD spread in the population 6 . They are also associated with a range of poor health outcomes, including pelvic inflammatory diseases, chronic pelvic pain, infertility, stillbirth, neonatal morbidity, and mortality 6 – 9 . In addition, asymptomatic STDs are co-factors for infections like HIV and persistent high-risk HPV infection that leads to cervical cancer 6 – 9 . Women are at greater risk of asymptomatic STDs compared to men 9 – 11 . Factors that contribute to this include the vaginal anatomy, oral contraceptive use, higher efficiency of STD transmission from males to females and a lower likelihood of showing symptoms compared to men 9 . Additionally, women face a higher burden of complications of asymptomatic STD that includes pelvic inflammatory disease, chronic pelvic pain, infertility, cervical cancer and ectopic pregnancy 9 – 11 . Most STD diagnosis and treatment initiatives for women have focused on those at increased risk of exposure, such as patients attending specialized sexually transmitted disease (STD) clinics and commercial sex workers 12 . However, a substantial proportion of new STDs in low- and middle-income countries (LMIC) like Nigeria occur in women who do not identify with high-risk groups 13 . Furthermore, many episodes of STDs produce minimal or no symptoms, which reduces the likelihood of identifying those most in need of treatment 3 , 14 , 15 . Globally, the risk factors for asymptomatic STD include sexual debut before 25 years of age, single marital status, poverty, and multiple sexual partnerships 1 , 3 , 16 . Other risk factors include female sex, inconsistent condom use, injection drug use or other substance use, such as alcohol or other drugs (pot, cocaine, ecstasy, crystal meth), past history of STDs, and HIV infection 12 , 15 , 16 . Social and structural risk environments, such as gender and power inequity in relationships, coercive sex, violence, and poverty also contribute to increased risk of asymptomatic STD 17 – 19 . Asymptomatic STDs disproportionately affects people living in LMIC because of a higher prevalence of the risk factors, limited access to healthcare, poor laboratory facilities, and inadequate health workers 1 , 3 , 20 . There are few publications on asymptomatic STD in sub-Saharan Africa. We conducted this study to understand the prevalence and risk factors for asymptomatic STDs among women in Nigeria, a lower-middle income country in sub-Saharan Africa. Methods This study was nested within a prospective cohort study of 11,700 women enrolled in the African Collaborative Center for Microbiome and Genomics Research (ACCME) study of persistent high-risk HPV infection, vaginal microbiome, and cervical cancer in Nigeria. The ACCME study has been previously described 21 . Selection of participants for STD testing. Participants were women enrolled in the ACCME study, living in Abuja, Nigeria’s capital city, who reported that they were free of symptoms of STD and HIV infection over the 12-month period before enrollment into this study between year 2020 and 2021. Sampling and laboratory testing for STDs. We collected high vaginal samples (HVS) from all the women using swab sticks which were inserted into transport media and immediately sent them to the laboratory for processing. We examined wet mounts of the HVS microscopically to identify cells such as Trichomonas vaginalis , clue cells, yeast, and pus cells. We conducted microscopic examination of Gram-stained films and biochemical tests manually using semi-automatic Vitek 2 compact machine. We inoculated the specimens into Blood Agar, Chocolate agar, CLED Medium and Saboroud Dextrose agar and incubated aerobically and anaerobically at 37⸰C for 18–24 hours. Thereafter, we examined the culture plates to identify the colonies. We performed antimicrobial susceptibility testing using the methods of Kirby-Bauer as described by NCCLS, 2019. We collected participant’s blood samples into EDTA and plain tubes, separated plasma and sera into cryovials, and preserved them at -20⸰C until tests for Chlamydia trachomatis IgM and Syphilis antibodies were performed. We detected Treponema pallidum antibodies using Syphilis Ultra Rapid Test Strip CTK Biotech USA, a rapid test for the diagnosis of Syphilis which qualitatively detects IgG and IgM antibodies to Treponema pallidum in whole blood, serum, or plasma according to the manufacturer’s protocols. We diluted participants’ serum or plasma and added them to wells coated with purified antigen. IgM specific antibodies. All unbound materials were washed away, and enzyme conjugate would bind to the antibody-antigen complex if present. The plates were then incubated to allow hydrolysis of the substrate by the enzyme. The intensity of the color generated was proportional to the amount of IgM specific antibody in the sample. The tests used to diagnose STIs in this study were those commonly used for clinical services in Nigeria at the time of screening. In total, participants were screened for Chlamydia trachomatis, Treponema pallidium (syphilis), Trichomonas vaginalis, Gardnerella vaginalis, Neisseria gonorrhea, Escherichia coli, Klebsiella spp., Staphylococcus aureus, Streptococcus agalactiae , and Proteus spp. infections and colonization by Candida albicans. We treated all the women who tested positive for any STD in this study, and we re-tested them to ascertain cure within one month of completing treatment. Data management and analysis We used structured data collection forms to obtain participants’ socio-demographic and behavioral data which were entered into a REDCap database 22 . To compute socio-economic status, we calculated the ‘wealth index’ using the following variables - house ownership and type of house owned (e.g., home, apartment, house, or duplex); source of drinking water (e.g., from outside, well, borehole, piped or bottled); type of cooking fuel; use of separate room for cooking; type of toilet; and ownership of household goods including car and refrigerator. We used Principal Component Analysis (PCA) with varimax rotation to compute factor scores based on the sum of responses to these variables weighted by their factor loading. We used the first component in the PCA that explained (35%) of the variations in the data, to generate a wealth index 23 . The wealth index variable was used to classify participants to low socio-economic status (lowest 40% of the score distribution), middle (middle 30%) and high (highest 30%) socio-economic class. We analyzed the data using Stata (version 17.0). We computed the mean and standard deviation of continuous variables and compared proportions of categorical variables using chi-square tests and age-adjusted odds ratios and 95% confidence interval (CI), However, where we have cells with less than five observations, we used Fisher exact test. In univariable analyses, we selected variables that were significant at p -value < = 0.20 level and included them in our multivariable logistic regression models. We specifically examined the relationship between age at sexual debut and risk of STD by using different cut-points based on the literature and different splines. We set statistical significance at p -value < 0.05 in our multivariable models. Results This study included 247 women with history of vaginal intercourse, none of whom reported symptoms of STD at the time of enrollment and over the preceding 12 months. Their mean (SD) age was 39.0 (9.06) years, mean (SD) age at sexual debut was 20.9 (4.02) years, and mean (SD) age at menarche was 14.2 (1.73) years. Most of the women (77.7%) were married, 13.4% were single, and 8.9% widowed. Most, 63.6%, have received university level education or higher and 46.2% were employed in a professional capacity (teachers, nurses etc.). Most of the participants were Christians (88.7%), have never used alcohol (70.4%), and have never smoked (100%). About half of the participants (53.0%) have ever used contraceptives, 44.1% have had only one vaginal sexual partner, 14.2% have had oral sex, and only 1.6% have ever had anal sex. Most of the women (93.5%) have a history of pregnancy, 81.4% have a history of vaginal discharge while 55.9% gave a history of vaginal douching (Table 1 ). The mean (SD) age at sexual debut among STD positive women was 20.7 (3.98) and this was lower than that of STD negative women which was 21.3 (4.04) years, but this was not statistically significant. However, when we dichotomized age at sexual debut into below 20 years compared to 20 years or more, there was significant inverse association (Adjusted Odds Ratio = 0.53, 95%CI = 0.31–0.90, p-value = 0.02). None of the other variables were significantly associated with STD (Table 2 ). A few of the women were positive for Streptococcus spp. , Proteus spp ., and Chlamydia. None of the participants were positive for Neisseria, Gardnerella , or syphilis (Table 3 ). Discussion In this study of Nigerian women living in an urban setting who voluntarily presented for cervical cancer screening and were asymptomatic for any STD at presentation and in the preceding 12 months, more than half tested positive for at least one vaginal bacterial infection. The infections were more common among women who had sexual debut at less than 20 years of age compared to those who were 20 or more years older at sexual debut. In subset analyses, only S. aureus was found to have significant association with the predictors in multivariable analyses suggesting that this bacteria was the main contributor to the observed effects. There are few studies of asymptomatic STDs in African women and few African healthcare systems offer regular routine screening for STDs for women at any age. The prevalence of chlamydia and syphilis in our study was lower than that found in a 2022 cross-sectional study of 150 asymptomatic women who presented for insertion of intrauterine contraceptive devices (IUCD) in Mwanza, Tanzania 24 . That study reported a prevalence of 22.7% for chlamydia and 5.3% for syphilis but the overall prevalence of STDs in that study was 30.0% which is substantially lower than what we found. These differences may be due to regional variations in the risk factors and prevalence of STDs in the communities. The Mwanza setting is more rural than the site of our study and women seeking IUCD may be substantially different from those volunteering for cervical cancer screening 24 . In another study in New Orleans, US, 1,631 individuals aged between 18 and 29 were screened for gonorrhea and chlamydial infections 25 . The prevalence of chlamydial infection was 10.1%, which is higher than the prevalence observed in our study. The study estimated that 45% of gonorrhea cases and 77% of chlamydial infections were asymptomatic and that 86% of untreated gonorrhea cases and 95% of untreated chlamydial infections were untreated because they were asymptomatic 25 . Another study of 2,224 adult women attending the cytology clinic of the University College Hospital, Ibadan, Nigeria for cervical cancer screening used the cervical cytology smears to diagnose Gardnerella vaginalis , Trichomonas vaginalis , and Candida albicans 26 . The study reported prevalence of 9.76% for Gardnerella vaginalis and 2.52% for Trichomonas vaginalis . The authors concluded that these infections can be easily diagnosed from cervical cytology by identification either of the organism or of characteristic cytological cellular changes, therefore screening for these infections should be included in cytology-based screening for cervical cancer 26 . However, this approach would miss the commonest asymptomatic STDs, cytology-based screening is not widely available in LMICs and is no longer recommended for primary cervical cancer screening in these environments. STDs are complex clinical and public health challenges whose prevention, diagnosis, treatment, and follow-up are associated with wide varieties of social issues including risk of stigmatization and discrimination. These challenges create huge barriers that are further compounded when the STD is asymptomatic which may be associated with fear, denial, and other negative psychological and social factors. The association with early age at sexual debut may be because of limited access to sexual health information, poor sexual hygiene, or engagement in risky sexual behaviors, such as unprotected intercourse in early sexual exploration. Public and sexual health education may be helpful in reducing these risk factors, but they face significant social and political barriers. The CDC currently recommends the Five P’s approach for health care providers obtaining sexual histories 27 . However, availability of healthcare workers and counselors who have the training, time, and resources to conduct effective, sensitive, non-judgmental clinical encounters in LMICs is limited. Given the infrastructure and resources for HIV screening, prevention, and treatment in Africa, it has been suggested that these programs can be used as a platform for STD screening in asymptomatic and HIV negative women. However, this may be challenging because of the high workload of the HIV programs and the risks of stigmatization and discrimination associated with HIV infection. Given the high prevalence of asymptomatic STDs in this study and others from LMICs, limited resources and inadequate number of personnel available for interview-based screening, routine microbiological testing of sexually active women at any encounter with the healthcare system may be the most efficacious approach to reducing the prevalence of asymptomatic STDs in LMICs. Such testing can be incorporated into school health programs and can be required as part of the bundle of peri-schools’ screening for adolescents and young adults 28 . Though school-based STD screening (SBSS) programs do not seem to reduce prevalence of STDs in either the school or the general adolescent population, detection of infections and treatment would reduce the sequela 28 . Studies of large-scale SBSS and similar programs in LMICs are urgently required. Furthermore, there has been little or no studies of natural or therapeutic clearance and re-infection rates of symptomatic and asymptomatic STDs in LMICs. Research in well-characterized prospective cohort of women with multiple testing is required to identify the best time intervals for screening tests and the most appropriate intervals between treatment and re-testing for tests of cure 29 . In conclusion, the high prevalence of STD in this study raises concerns about their population level impact on woman’s reproductive and overall health, policies and practices about STD prevention and screening. Exploring the impact of cultural and societal norms on early sexual debut, modalities and appropriate schedules for population level screening, and determination of tests for cure would provide valuable insights for targeted interventions. Asymptomatic infections pose higher risks when left untreated. This study examines the prevalence and determinants of asymptomatic STIs among Nigerian women.\u003c/p\u003e \u003cp\u003eThe study is part of a larger cohort involving 11,700 women investigating high-risk HPV, vaginal microbiome, and cervical cancer in Nigeria. Participants, who reported no symptoms of STIs or HIV over 12 months, provided high-vaginal swabs for analysis and serum for testing Chlamydia trachomatis and Treponema pallidum antibodies.\u003c/p\u003e \u003cp\u003eThe mean age of participants was 39.0 years, with an average sexual debut at 20.9 years. Over half (56.3%) tested positive for at least one bacterial infection. Women with STIs had a lower mean age at sexual debut (20.7 years) compared to those without (21.3 years), though this was not statistically significant. However, debut before age 20 was significantly associated with increased risk of asymptomatic STIs (adjusted OR\u0026thinsp;=\u0026thinsp;0.53, p\u0026thinsp;=\u0026thinsp;0.02).
