This paper reports surgical data from 144 patients in Vienna who had persisting sonographically detected “simple” adnexal cysts and were operated on, with subsequent histology used to determine malignancy. Only 3 cysts (one borderline) were malignant, and all occurred in postmenopausal patients; the malignant cysts had mean diameters of 7, 11, and 18 cm. The authors also describe that many “simple” cysts were benign on pathology, while others were associated with chronic pelvic sepsis (including hydrosalpinx and pseudocysts), benign cystadenomas, and a smaller number of endometriotic cysts. This paper relates to endometriosis because 10 of the pathologically benign “simple” cysts were endometriotic.
