Globally, 118.54 million blood donations occur annually, with 31.5 donations per 1,000 people in high-income countries compared to 16.4 in upper-middle-income and 5 in low-income countries. The WHO (world Health Organization) insists that countries focus on young people to achieve 100% non-remunerated voluntary blood donation (VBD). Therefore, university students are good candidates for blood donation. This review provides a better understanding of the current status of blood donation among medical students. This review systematically searched international databases (PubMed, Scopus, EMBASE, Web of Science, and Google Scholar) with relevant English studies published in English from 2000 to April 2024. study characteristics (study ID, location, study design, sampling techniques, and university), main outcome measures, and findings were extracted.We included 39 cross-sectional Knowledge, Attitude, and Practice (KAP) studies across nine countries involving 13,895 medical students. Most studies were conducted in India. all studies used different self-administrated tools to collect data. Most of the studies had fair to good quality. Positive attitudes varied widely (42%-92.6%), with the highest rates among Indian students and the lowest among Pakistani students. Donation rates ranged from 1–74%, with Nigerian students demonstrating the highest rates. This review, which analyzed 39 cross-sectional KAP studies from nine countries involving 13,895 medical students, found that most students exhibited moderate levels of knowledge, attitudes, and practices regarding blood donation. To bridge this gap, policymakers must prioritize VBD awareness campaigns, and institutions should facilitate donation opportunities and incentivize participation among medical students. Blood transfusion provides essential support for patients undergoing surgical procedures, managing traumatic injuries, treating chronic diseases, and combating cancer. This intervention serves as a critical cornerstone of healthcare delivery, ensuring patient stability and recovery in diverse clinical scenarios. Additionally, beyond its immediate therapeutic benefits, blood donation serves a dual purpose by facilitating therapeutic phlebotomy for individuals afflicted with conditions such as hereditary hemochromatosis, polycythemia vera, and other less prevalent disorders. Therefore, advocating for regular blood donations is imperative to maintain a steady supply of this life-saving resource [ 1 ]. According to the World Health Organization (WHO), it is important that at least one% of the population donates blood regularly to maintain a sufficient blood supply. However, many countries around the world have struggled to achieve this goal. By 2006, more than 70 countries had reported donation rates below this recommended level. This situation creates challenges in meeting the worldwide demand for blood products [ 2 ].For example, in India, where approximately 26.2 million units of blood are needed each year, there is a significant shortfall of about 13 million units annually. This gap underscores the urgent need for coordinated efforts to improve blood donation rates on a global scale [ 1 ]. WHO reports that 38% of voluntary blood donors are younger than 25 years old, underscoring the organization's emphasis on engaging young people to achieve 100% non-remunerated voluntary blood donation. Consequently, university students are viewed as promising candidates for blood donation, particularly medical students who are expected to possess greater knowledge and are likely to become dedicated voluntary donors. Given their future roles as healthcare professionals, the attitudes and behaviors of medical students can potentially influence broader societal perspectives on blood donation [ 1 ]. This review aims to synthesize findings from studies investigating the KAPs of medical students regarding blood donation. By systematically summarizing these findings, this review provides insights into the factors influencing medical students' engagement with blood donation, highlighting areas for further research and suggesting strategies to enhance their participation in promoting this critical public health activity. 2 Methods 2.1. Protocol registration This review was registered on PROSPERO (CRD42024543640) and adheres to the PRISMA guidelines for observational studies 2.2. Eligibility The inclusion criteria for studies were as follows: Study Population: Medical students only, specifically Bachelor of Medicine (MBBS) students at any stage of their undergraduate training. Outcome: Knowledge, attitude, and practice (KAP) towards blood donation. Study Design: Cross-sectional studies. Time Frame: Studies conducted from 2000 to 2024. Language: Studies published in English. 2.3. We excluded studies that focused on postgraduate students, healthcare professionals, or students from pharmacy and dental fields. Qualitative study designs, non-English publications, and editorial correspondence, including letters to editors, case reports, and conference abstracts, were also excluded. Additionally, we omitted studies comparing medical with non-medical students as well as cross-sectional studies reporting pre- and post-intervention effects on blood donation. 2.4. Search Strategy A comprehensive search of international databases (PubMed, Scopus, EMBASE, Web of Science, and Google Scholar) was conducted to identify relevant studies from their inception to April 2024. The search included the following Medical Subject Headings (MeSH) terms: "knowledge," "attitude," "practice," "students medical," and "blood donation. Study Selection The initial search retrieved 1187 articles. After removing 204 duplicate records using Rayaan, 983 articles remained. A.F. and A.E. independently performed primary screening based on titles and abstracts. Any disagreements were resolved by discussion and consultation with a third author. O.A. did a full-text screening, resulting in 66 articles. Out of these, 27 articles did not meet the eligibility criteria and were excluded, leaving 39 studies for inclusion in the final review. 2.6. Data Extraction Data extraction was performed independently by S.M. and O.A. Information on study characteristics (study ID, location, study design, sampling techniques, and university), main outcome measures, and findings were extracted and organized into an Excel table to facilitate quality assessment. Any disagreement was resolved through discussion. Table 1 shows the characteristics and findings of the included studies 2.7. Quality Assessment The quality of the included studies was assessed using the Newcastle-Ottawa Scale (NOS) by A.D. and S.M. This scale evaluates studies based on three broad perspectives: the selection of the study groups, the comparability of the groups, and the ascertainment of the outcome of interest. After summarizing the scores obtained for each question, the cut-off point for a good quality study was 7 out of 10, which was declared based on previous relevant studies. 2.8. Statistical Analysis The included studies were analyzed qualitatively. Because different questionnaires were used in studies and there was a high heterogeneity, conducting a meta-analysis was not possible, and Results were reported narratively, focusing on common findings across the studies. The analysis highlighted patterns and discrepancies in knowledge, attitudes, and practices related to blood donation among medical students. 3 Results 3.1 Literature search Our search strategy retrieved 1187 articles. Duplicates were removed using Rayaan, and titles were screened by two independent reviewers, leaving 66 unique articles for full-text screening. We excluded 27 articles that did not fulfill the eligibility criteria. 39 studies were included in our review. The PRISMA flow diagram is shown in Figure. 1. 3.2 Characteristics of the included studies All 39 articles included in this systematic review were cross-sectional studies that used self-administered questionnaires and were conducted in nine countries worldwide. Of these, 22 studies were conducted in India [ 3 ], [ 4 ], [ 5 ], [ 6 ], [ 7 ], [ 8 ], [ 9 ], [ 10 ], [ 11 ], [ 12 ], [ 13 ], [ 14 ], [ 15 ], [ 16 ], [ 17 ], [ 18 ], [ 19 ], [ 20 ], [ 21 ], [ 22 ], [ 23 ], [ 24 ]. Eight studies were conducted in Pakistan [ 25 ], [ 26 ], [ 27 ], [ 28 ], [ 29 ], [ 30 ], [ 31 ], [ 32 ]. Two studies were conducted in Nigeria [ 33 ], [ 34 ], and one study each was conducted in Saudi Arabia [ 35 ], Syria [ 36 ], Palestine [ 37 ], Dominica [ 38 ], Serbia [ 39 ], Nepal [ 40 ], and Malaysia [ 41 ]. Overall, 13985 participants were included. In all 39 included studies, the researchers used self-administered instruments for data collection. The number of items in different questionnaires varied. The content of the items was different from study to study. Nevertheless, overall, they examined the three components of KAP of blood donation among medical students. The sample size varied from 104 to 987. Table 2 shows the participants' characteristics. 3.3 Quality assessment In this review, three studies were of good quality. The included studies scored well for clearly stating the study aims, design, target population, risk factors, and outcome measurements, as well as result explanations and discussions. Common issues identified included the lack of sample size calculations, addressing non-response rates, missing data within completed questionnaires, and the absence of pilot testing. Table 3 shows the results of the quality assessment of the included studies. 3.4 Knowledge of blood donation towards Of the 39 studies, 26 reported knowledge by correct answers on the minimum age required to donate correct answers ranging from 2.60% [ 27 ] to 99.60% [ 4 ]. Thirteen studies reported the minimum hemoglobin rate required for blood donation correct answers ranging from 32.7% [ 5 ] to 89% [ 8 ]. Out of 13 studies, 2 studies reported knowledge of hemoglobin rates for men and women independently [ 13 ], [ 29 ]. Nineteen studies reported time intervals between donations. Correct answers ranged from 18.30 [ 26 ] to 93% [ 19 ]. Nine studies reported the knowledge of universal blood donors. Correct answers ranged from Lowest: 69.8% [ 18 ] to Highest: 99% [ 6 ]. Twenty-one studies reported the volume of blood donated in each donation. Correct answers varied from Lowest: 9% [ 5 ] to Highest: 98.60% [ 22 ], and 12 studies reported the knowledge of the students' blood type. Blood transfusion diseases such as HIV, HBV, HCV, malaria, and syphilis were reported differently in 12 studies. Table 4 shows 3.5 Attitude Eleven studies assessed attitude by positive attitude percentage, which varies from 42% [ 32 ] to [ 14 ] 92.6%. Attitude levels are shown in Table 5 3.6 Practice The willingness to donate blood in the future varied in studies, ranging from 50% [ 32 ] to 95% [ 21 ]. The percentage of students who donated blood in the past ranged from 1 [ 3 ] to 74.10% [ 34 ]. While six studies reported donating blood as a noble act from 93.70% [ 12 ] to 100%. [ 11 ]. Practice levels are represented in Table 6 . 3.7 Reasons stopping you from donating Some included studies mentioned a question about the reasons for not donating blood, and many answers were reported. Fear of blood and needles was reported in most studies, along with medical ineligibility. Some studies included religion as a cause of donation refusal. 3.8 Source of information Most Eligible studies included a question about participants' sources of information, while educational institutes, TV, and social media were reported as leading causes. Family and friends were reported in a few studies as well. 4 Discussion Medical students exhibit moderate knowledge and awareness of blood donation relative to other health disciplines, along with high willingness to donate. Despite this, their actual donation rates are often low due to barriers such as a lack of information and practical opportunities. A cross-sectional study in Poland was conducted on students in other health disciplines, such as pharmacy, dentistry, and health sciences showed that medical students are significantly more aware of the necessity of blood donation than the rest of the Polish community, and the majority know their blood type [ 42 ]. The results of another study by Giri assessed knowledge among different health branches: medicine and physiotherapy in India. It reported that the overall knowledge of blood donation was good, and the highest knowledge level was reported among medical students (53.1%), while the lowest was among physiotherapy students (20.7%). [ 43 ]. while in another institution, 78% of the students were willing to donate blood in the future, and knowledge for each group of the students was addressed [ 44 ]. Both [ 45 ] and [ 46 ] revealed that despite Malaysian and Nigerian students having adequate knowledge and positive attitudes, the practice was low. A study by [ 47 ]. showed that Nepalese medical students had higher knowledge and practice levels than non-medical students. Measures such as timely education can make a change in their KAP. This study showed that the leading causes of Low practice are a lack of information and direct requests. This aligns with [ 48 ], a review conducted on developing countries. The study indicates that although medical students exhibit a middle level of awareness and knowledge regarding blood donation compared to students in other health disciplines, their actual donation rates are insufficient. This variation demonstrates that increased awareness alone is insufficient to translate in translating knowledge into practice. To effectively achieve this, it is crucial to develop targeted educational programs that integrate practical training and facilitate easier access to donation opportunities. By enhancing these aspects, institutions can improve overall donation rates and contribute to a more dependable and efficient blood supply system by effectively overcoming the awareness and real blood donation practices barrier. 4.1. Limitation One of the limitations of this review is that most of the included studies did not report the overall levels of the KAP of the students. Secondly, all the studies used different self-administered questionnaires, which led to a high level of bias. Future research should employ standardized measures to more accurately evaluate and compare KAP levels, thereby reducing bias and providing a clearer understanding of the factors influencing blood donation practices among students. Conclusion Blood donation is a critical and life-saving resource during emergencies and significantly benefits patients. This review, which analyzed 39 cross-sectional KAP studies from nine countries involving 13,895 medical students, found that most students exhibited moderate levels of knowledge, attitudes, and practices regarding blood donation. To bridge this gap, policymakers must prioritize VBD awareness campaigns, and institutions should facilitate donation opportunities and incentivize participation among medical students. By addressing such issues, we can close the knowledge gap and take action towards increasing blood donation rates and enhancing patient care. Globally, 118.54\u0026nbsp;million blood donations occur annually, with 31.5 donations per 1,000 people in high-income countries compared to 16.4 in upper-middle-income and 5 in low-income countries. The WHO (world Health Organization) insists that countries focus on young people to achieve 100% non-remunerated voluntary blood donation (VBD). Therefore, university students are good candidates for blood donation. This review provides a better understanding of the current status of blood donation among medical students. This review systematically searched international databases (PubMed, Scopus, EMBASE, Web of Science, and Google Scholar) with relevant English studies published in English from 2000 to April 2024. study characteristics (study ID, location, study design, sampling techniques, and university), main outcome measures, and findings were extracted.We included 39 cross-sectional Knowledge, Attitude, and Practice (KAP) studies across nine countries involving 13,895 medical students. Most studies were conducted in India. all studies used different self-administrated tools to collect data. Most of the studies had fair to good quality. Positive attitudes varied widely (42%-92.6%), with the highest rates among Indian students and the lowest among Pakistani students. Donation rates ranged from 1\u0026ndash;74%, with Nigerian students demonstrating the highest rates. This review, which analyzed 39 cross-sectional KAP studies from nine countries involving 13,895 medical students, found that most students exhibited moderate levels of knowledge, attitudes, and practices regarding blood donation.
