Abstract

Introduction: The diagnosis of endometriosis in patients with chronic pelvic pain is confirmed by laparoscopy and/or imaging. The lack of non-invasive criteria for the diagnosis of endometriosis has motivated the performance of this study, which aimed to identify sociodemographic and clinical patterns in patients with chronic pelvic pain for the non-invasive diagnosis of this pathology. Methods: Cross-sectional analytical study in which data was collected through anonymous surveys from 184 patients with chronic pelvic pain at the Center for Reproductive Medicine and Advanced Gynecological Endoscopy in 2024. Results: The study showed a progressive increase in patients with chronic pelvic pain due to endometriosis as age increased within the range of 18 to 35 years, with a progressive decline after 36 years of age. The prevalence of endometriosis in patients with chronic pelvic pain was 74.46 % with OR 20.40, 95 % CI 4.47-93.13, p = 0.0001, being ovarian endometriosis the most frequent variant. 58 % of patients with endometriosis presented chronic pelvic pain with lumbar irradiation, with OR 3.12, 95 % CI 1.20-8.02, p < 0.05. There is a weak, although statistically significant, correlation between the severity of pain and the degree of endometriosis involvement, presenting a Spearman correlation of 0.28 with Student t = 3.21 and p = 0.0018. Conclusions: Sociodemographic and clinical patterns were identified that can be used in the early diagnosis of endometriosis in the absence of invasive or imaging methods.

La insuficiencia de criterios no invasivos para el diagnóstico de endometriosis ha motivado a la realización del presente estudio, mediante el cual se ha pretendido identificar patrones sociodemográficos y clínicos en pacientes con dolor pélvico crónico para el diagnóstico no invasivo de esta patología. Métodos: Estudio analítico transversal en el que se recolectaron datos mediante encuestas anónimas a 184 pacientes con dolor pélvico crónico en el Centro de Medicina Reproductiva y Endoscopia Ginecológica Avanzada en el año 2024. Resultados: El estudio mostró un incremento secuencial de pacientes con dolor pélvico crónico por endometriosis a medida que aumentó la edad dentro del rango de 18 a 35 años, con una disminución progresiva a partir de los 36 años. La prevalencia de endometriosis en pacientes con dolor pélvico crónico fue de 74.46 % con OR 20.40, IC95 % 4.47-93.13, p = 0.0001, siendo la endometriosis ovárica la variante más frecuente de la misma. El 58 % de las pacientes con endometriosis presentó dolor pélvico crónico con irradiación lumbar, con OR 3.12, IC95 % 1.20-8.02, p < 0.05. Existe una correlación débil, aunque estadísticamente significativa, entre la severidad del dolor y el grado de afectación por endometriosis, presentando una correlación de Spearman de 0.28 con t de Student = 3.21 y p =0.0018. Conclusiones: Se identificaron patrones sociodemográficos y clínicos que pueden emplearse en el diagnóstico temprano de endometriosis en ausencia de métodos invasivos o de imagen.
