We collected 213 valid responses through online questionnaire surveys, which provided data on basic information for analysis. Results Psychological pressure, which could be self-relieved, increased during the epidemic’s late stage (46.9% vs. 67.1%, p = 0.000), but an increase in depressive tendencies was not observed. Females were more likely to work longer hours, spend less time doing research, have a higher fear of going out, and were less likely to return to offline learning. They were also less likely to be depressed. However, males exhibited shorter working hours, greater devotion to scientific research, less fear of going out, and a stronger desire to resume offline learning, but they showed greater depression levels. After adjusting for the effects of other parameters, being male, having longer working hours, and lacking time for scientific research were significant predictors of a depressive trend. However, graduate or postdoctoral students were less likely to be depressive. Conclusions Graduate or postdoctoral students are less prone to depression during the epidemic, possibly due to tutor presence during public health emergencies. Introduction An epidemic outbreak serves as an extraordinary challenge, with hospitals becoming the primary frontline and clinical residents emerging as the main forces in the fight against the epidemic. Given China’s large population, the impact of an epidemic outbreak on clinical residents is substantial. For example, during the global coronavirus disease 2019 (COVID-19) pandemic, young clinical residents, as a primary force in the fight, faced marked challenges as they endured the double pressure of balancing clinical work and personal life. Therefore, their physical and mental well-being require attention. The COVID-19 pandemic continues to spread globally, while other endemic diseases such as influenza, monkeypox, cholera, dengue fever, and measles also persist. These illnesses have high incidence rates and a notable risk of global transmission, posing a serious ongoing threat to public health. Ensuring the physical and mental health of residents is crucial for improving the effectiveness of standardized training during residency and enhancing work competence. A Turkish study in 2024 [ 1 ] included 300 volunteer resident physicians to examine various health issues related to being a physician, such as depression, burnout, fatigue, and hopelessness. It was said 73.7% (221) of the doctors had mild to severe depressive symptoms, 78.7% (236) had mild to severe anxiety symptoms, and 57.7% (173) had mild to severe symptoms of stress. This implies that mental health issues are prevalent among resident physicians. When confronted with public health crises, this situation could become even more severe. Another study from China revealed that the incidence of depression among residents during the epidemic outbreak was 20.8% [ 2 ] , exceeding the usual incidence of depression among healthcare workers (13.4%) [ 3 ] . This higher rate probably occurs because frontline residents feel stressed, worried, and helpless in the face of new patients, lack of medical supplies, and exposure to infection risks [ 4 ] . As young medical staff members, residents are less likely to encounter major stress events. Some even have a student identity. Their life and clinical experiences are not as rich as those of older medical staff. Their self-problem-solving ability is relatively weak [ 5 ] . During public health emergencies, residents show different anxiety and depression levels. Therefore, strengthening the care of clinical residents, promptly addressing their psychological conditions, providing appropriate recognition, and ensuring adequate social support are crucial steps [ 6 ] . To improve the psychological health and problem-solving skills of residents in standardized training in response to epidemic outbreaks, we analyzed the educational concerns of residents in aspects of clinical work, life, learning, and mental stress during the epidemic in China. Using the COVID-19 outbreak as a study background, we conducted an online questionnaire survey. Material and Methods The study subjects were residents working at Peking University Third Hospital. The COVID-19 outbreak served as the study background. The “early stage of the epidemic” was defined as the first three years of strict protective measures and the “late stage” as the mask-only protection period beginning in December 2022. Using an online questionnaire, we collected 213 valid responses, which provided data on basic information, and assessed the impact of the epidemic on clinical work, daily life, learning, and psychological stress. The full questionnaire is provided in Supplementary File. The reproductive health ethics committee of Peking University Third Hospital approved this study. Participation was voluntary and provided their written informed consent online. All responses were kept anonymous. Procedures performed in this study involving human participants or on human tissue were in accordance with the ethical standards of the institutional and/or national research committee and with the 1975 Helsinki declaration and its later amendments or comparable ethical standards. The questionnaire survey was conducted online, voluntarily and anonymously. Each participant could submit only once, and 213 valid questionnaires were collected. The questionnaire was discussed and formulated by the clinical teaching directors. After further modifications and revisions, the final questionnaire included 23 questions concerning basic personal information (9 items), influence of the novel coronavirus epidemic control strategies on the life and work of residents (7 items), and impact of gradual outbreak of the epidemic on the life and work of residents (7 items). All subjects completed the questionnaire independently. SPSS 25.0 software was used for the statistical analysis. χ2 or Fisher’s exact test was used to analyze categorical variables, which are presented as numbers and percentages. Student’s t-test was used to analyze continuous variables, presented as mean with standard deviation (SD). We used the logistic regression method to analyze the factors influencing the depressive trend during the pandemic, and p < 0.05 was considered statistically significant. Results Basic information about residents is listed in Table 1. The ratio of male to female residents was 4:6, with an average age of 29 years. Of the residents, 60.6% were unmarried. The residents were mainly in the third year (30.05%), the second year (26.76%), and the first year (22.07%) of clinical training. Of the residents, 37.08% were postgraduates and postdocs. The subjects of this questionnaire mainly majored in clinical medicine, distributed in all grades, and had a relatively good understanding of the epidemic. Compared with the early stage, the proportion of residents who had no time to carry out scientific research decreased (63.8% vs. 40.4%, p = 0.000) in the late stage, with more residents preferring to resume offline teaching (54.0% vs. 63.8%, p = 0.039). A downward trend in fear of infection (33.3% vs. 19.7%, p = 0.001) and an upward trend in fondness for going out (50.7% vs. 66.7%, p = 0.005) were also observed. Psychological pressure, which could be self-relieved, increased in the late-stage relative to the early stage (46.9% vs. 67.1%, p = 0.000), but no increase in depressive tendencies was noted. Higher seniority was associated with a lower proportion of introverted individuals, longer periods of separation from parents, and more complex work responsibilities during the epidemic. Hence, we conducted a multivariate logistic regression analysis for factors influencing the depressive trend during the epidemic (Table 5). First, all potential confounders that could affect the depressive trend were adjusted to determine whether significant differences were observed. After adjusting for the effects of other parameters, being male, longer working hours, and lack of time for scientific research were significant independent predictors of depressive trends. However, being a graduate student or postdoctoral researcher serves as a protective factor against depressive tendencies. While females exhibit a greater tendency to work longer hours and experience a higher fear of going out, they are less likely to show signs of depression. In contrast, males show shorter working hours and less fear of going out, with greater depression levels. After parameter adjustment, being male and longer working hours are significant predictors of depressive trends. However, graduate or postdoctoral residents were less likely to be depressive. Excessive working hours and health impacts on resident physicians Residents are at a crucial stage in their careers and face high workloads, long working hours, and high expectations from senior physicians and patients. In a study, 71% of residents worked more than 40 hours per week [ 7 ] . This study indicated that long working hours per week was a significant predictor of back pain among resident physicians. In our study, 60% of residents worked more than 10 hours per day during the outbreak of the epidemic. longer working hours was also significant independent predictor of depressive trend. Similarly, Japanese researchers reported that 10.6% of hospital residents work an average of 80 hours per week and have 5.6 nightshifts per month, which leads to increased stress levels and burnout [ 8 ] . In a study aimed at investigating the incidence rate of resident burnout and its related factors, resident physicians worked an average of 75 hours per week, with a burnout rate of 46.3%. The study found that healthcare workers generally had high levels of emotional exhaustion (57.1%) and low levels of personal accomplishment (52.4%) [ 9 ] . Therefore, measures need to be taken to reduce the working hours of resident doctors. For example, the United States has a rule that resident physicians must work less than 80 hours per week [ 10 ] , while the European Union requires residents to work only 48 hours per week [ 11 ] . In 2024, the Japanese government will impose a weekly working time limit of up to 80 hours on residents [ 12 ] . Gender disparities in resident physicians’ mental health In the 2015 systematic review by Mata et al., the prevalence of depression among residents was 28.8% [ 13 ] . The occurrence of anxiety and depression is influenced by gender. Some studies suggest that sex hormones play a role in regulating emotions, making women more sensitive to external factors, which may increase the incidence of anxiety and depression [ 14 , 15 ] . Additionally, depression accompanied by comorbid anxiety disorders is reported to be twice as common in women compared to men [ 16 ] . In 2022, a study in Japan [ 17 ] examined the relationship between excessive workload, burnout, and suicidal ideation among residents in hospitals. 24.1% and 5.6% of residents reported having depressive tendencies and suicidal thoughts, respectively. These percentages tended to increase when respondents worked longer hours. Several factors were significantly associated with suicidal ideation. One of the factors is female sex (reference: male, OR: 2.08, 95% CI: 1.56 to 2.77). In another study, it was mentioned that during the COVID-19 pandemic, residents faced a higher risk of anxiety, depression, and somatic symptoms. Women were at a disadvantage in terms of psychological state. Women resident physicians were more likely to experience somatic symptoms (adjusted odds ratio (OR) = 2.36, 95% confidence interval (CI): 1.33–4.18) [ 18 ] . Our findings of higher depression levels in males during the epidemic outbreak may have resulted from marital status. Notably, 46.3% of women were married, compared to 30% of men (p = 0.016). The presence of a spouse provides crucial support in coping with work-related and life-related stress. Regardless of the underlying causes, greater attention must be given to addressing the psychological pressure faced by male residents during public health emergencies. Mentorship as a protective factor in enhancing resident well-being, clinical competence, and academic output during health crises Beyond family and spousal support, tutor support is also important. We also uncover that graduate or postdoctoral students are less prone to depression during the epidemic, possibly due to tutor presence. Adopting a tutorial strategy is valuable for the physical and mental health of residents during public health emergencies. A study examining general practice residents’ perceptions of their general practitioner tutors—as well as the factors influencing tutor satisfaction—uncovered that general practitioner residents were very satisfied with their general practitioner theory training instructors [ 19 ] . A previous study conducted in our hospital highlighted the advantages of the tutorial system. The study explored whether tutorial system implementation in the standardized training of obstetrics and gynecology residents can enhance the clinical teaching effect, revealing that the total score of the rotation assessment of the group with tutors markedly exceeded that of the nontutor group. Before the tutorial system, the number of scientific research articles published by residents was 0, and after the tutorial system was launched, it was 1–2 articles per year [ 20 ] . The tutorial system is a teaching system that draws on the teaching model of university graduate students, in which students choose tutors and research directions according to their own needs and learn a specific knowledge field [ 21 ] . Unlike the traditional teaching mode, the teacher–student relationship is closer in the tutorial system. It emphasizes teaching students according to their individual characteristics and guiding students’ life [ 22 ] . Since the quality of teachers is crucial for ensuring the effectiveness of standardized training for resident doctors, we recommend selecting tutors for each resident among senior physicians and deputy senior physicians with high medical ethics, a solid theoretical foundation, rich clinical experience, rigorous clinical thinking, excellent academic level, strong communication skills, and outstanding innovation ability. Strengths and limitations The present study has certain strengths and limitations. First, the survey was carried out in the first month of the mask-only protection period in December 2022, immediately following three years of strict protective measures. Currently, the conditions of the residents should have improved significantly compared to that time. Nevertheless, precisely because of its timing, the survey captured an authentic snapshot of the state of resident physicians during a public health emergency—insights that cannot be obtained through current investigations. Secondly, to ensure the response rate of the questionnaires, the resident physicians were not required to complete the Self-Rating Depression Scale (SDS) during the public health emergency, which may have limited the scope of mental health assessment. Conclusion Improving working conditions and personal welfare, ensuring their physical and mental health, and conducting early screening and rehabilitation for affected residents not only benefits residents themselves but also helps to improve job satisfaction, strengthen doctor-patient relationships, and have a significant impact on medical services. Based on the findings of this study, we recommend the implementation of tutorial system in the standardized training of residents. The tutorial system can provide guidance in clinical emergencies while promoting physical and mental health when handling public health emergencies. These illnesses have high incidence rates and a notable risk of global transmission, posing a serious ongoing threat to public health. We defined the epidemic\u0026rsquo;s early stage as the first three years of strict protective measures and the late stage as the mask-only protection period. We collected 213 valid responses through online questionnaire surveys, which provided data on basic information for analysis.\u003c/p\u003e\u003ch2\u003eResults\u003c/h2\u003e \u003cp\u003ePsychological pressure, which could be self-relieved, increased during the epidemic\u0026rsquo;s late stage (46.9% vs. 67.1%, p\u0026thinsp;=\u0026thinsp;0.000), but an increase in depressive tendencies was not observed. Females were more likely to work longer hours, spend less time doing research, have a higher fear of going out, and were less likely to return to offline learning. They were also less likely to be depressed. However, males exhibited shorter working hours, greater devotion to scientific research, less fear of going out, and a stronger desire to resume offline learning, but they showed greater depression levels. After adjusting for the effects of other parameters, being male, having longer working hours, and lacking time for scientific research were significant predictors of a depressive trend.
